TX Podiatrist Pioneers Use of New Non-Opioid Post-Op Painkiller 
 
Showing its leadership in addressing the opioid crisis, the UT Health San Antonio Multispecialty and Research Hospital is the state’s first to approve use of a new non-opioid medication for pain following orthopaedic and podiatric surgeries. The new drug suzetrigine, brand name Journavx, is not a controlled substance and avoids opioid-related side-effects. It was approved by the U.S. Food and Drug Administration in January and first became available in April.

The type of post-surgical pain associated with orthopaedics is the target indication for the newly approved medication, said Lee C. Rogers, DPM, clinical associate professor of orthopaedics and chief of podiatry at UT Health San Antonio. “It could be spine, or a total joint, or a bunion,” Rogers said. “This is a way to use innovative and effective pain management approaches that avoid opioids and their side-effects.”

NY Podiatrist Discusses Common Injuries to Feet From Ballet
 
Despite ballet’s long and storied history (its origin dates back to the courts of the Italian Renaissance), scant research exists around the lasting effects of going en pointe. However, as you can imagine, balancing one’s body weight on two points no larger than a couple of quarters does take its toll—especially on young, developing feet, as the bone structure isn’t skeletally mature until about the age of 17, says Nelya Lobkova, DPM, a board-certified surgical podiatrist in New York City.

A common ailment among the patients Dr. Lobkova sees—people who did ballet as kids and teens but didn’t become professional dancers—is nail trauma. “If the impact and the friction on the nail from early childhood was strong enough and bad enough because of the shape of their toes and how they came up or came down [from en pointe], it will change the nail forever,” she says. Going en pointe can also lead to the development of corns or hammertoes, according to Dr. Lobkova. Additionally, going en pointe can exacerbate bunions, but doesn’t necessarily cause bunions.

NY Podiatrist Discusses How to Choose Women's Sandals
 
According to Bruce Pinker, DPM, a board certified podiatrist and foot surgeon, you'll want to look for a sandal that offers arch support, cushioning, durability, and breathability. "Supportive sandals should have comfortable arch support, appropriate outsole traction for the intended activity, added cushion, durable uppers, adjustable straps, and shock absorption. They should also fit securely with ankle straps for added support and a secure fit," he explains.

The podiatrist also says to pay attention to how your toes sit at the end of the sandal, as well as the fit of the shoe around your entire foot. "Summer sandals should possess secure ankle straps to accommodate long walks. Also, there should be no overhang and there should be a comfortable, supportive arch," Dr. Pinker says. "Sandals are not suitable for a person's feet if they fit too tightly and cause blister formation, fit too loosely so that the footwear slides around when you walk, or fit in such a way that the foot hangs over the edge of the footwear."

5. Hold five-minute stay interviews
 
Exit interviews happen too late. Stay interviews—quick, quarterly check-ins that ask, “What still excites you here? What might tempt you to leave?”—surface fixable irritants early. In one case study on hiring during the Great Resignation, a practice saved two senior coders after discovering their chief complaint was a squeaky chair and thermostat wars. Block 10 minutes every Friday for one informal chat. Bring a pen, not a form; the goal is conversation, not paperwork.

Query: Amnionic cell membrane injection/umbilical cord injection for muscular skeletal injuries
 
What is your view of the use of these products for Achilles tendinitis and plantar fasciitis. There was a article by the FDA in 2021 which stated that amniotic products are only to be used for experimental treatments. However, when reviewing this, this is the only article that I have located that says that it should be experimental.
 
What is your thought on whether we can offer this to our patients as long as in the consent we spell out that the FDA has approved the use of this item for human use, although it is experimental in terms of its use for these conditions.

Your post, I believe, is lacking the more recent article published. Consider reviewing the bibliography for many other articles that have been written. The best advice is contact your healthcare attorney and/or medical malpractice carrier before starting to use this in your practice. I have about 40 years experience with hyaluronidase injections for scar tissue. It's an invaluable tool. I generally create a "cocktail" with haluronidase, triancinalone acetate, Decardon, and Marcaine: 1/4cc, 1/4cc, 1/4cc,1/2cc, respectively. It's not comfortable to give the injection without giving a local anesthetic block proximally. I advise they could experience a post-injection flare from the steroids. I have had universal success with this injection, but sometimes less than 100% relief. Still, every patient has been happy they had the injection. I have never had an adverse response.

RE: Compensating for Podiatric Surgeons Who are Not Trained In General Podiatry

There has been a great deal of discussion on how to compensate for a lack of training in general podiatry at some of our 3-year residency programs. The issue is that many who complete these programs are masters in foot and ankle surgery but are not trained in how to manage patients with "corns, calluses, warts, ingrown toenails, and other "bread & butter" pathologies.
 
When I did my residency many years ago, we treated patients at a hospital-based general podiatry clinic several days a week, managed all foot emergencies in the ER, and assisted podiatric surgeons and orthopedic surgeons in the operating room. Many programs today, as has been well pointed out, are strong in surgical training but lacking in general podiatry training. The root of the problem may be that these programs may not have standing general podiatry clinics. If that is the case, these programs can work with general podiatrists and have residents rotate through their offices in order to master the non-surgical aspects of our profession.

It is gratifying to see the last two messages from Drs. Pressman and Tomczak supporting the need for some kind of dual track 3-year residency training program to best meet the training needs of both predominantly surgical and medically oriented podiatrists. We are all proud of what our profession has accomplished in the development of the standardized 3-year surgical residency model. It seems to be producing excellent foot surgeons that are well respected in medical delivery systems, well employed, and well rewarded financially. But these two individuals seem to agree with me that it is time to catch our breath and assess whether these training programs are the best that we can do in training ALL the podiatrists that the U.S. population needs to provide for their foot care needs in the decades ahead.  
 
My main concern is that a resident’s time is well spent training in the work that they will be doing after their 3 years are up.  Believe me, I know that restructuring what was years in the making will be difficult. But perhaps some programs are already well suited to offer more clinic-based outpatient training for residents heading for more medically oriented office-based practice? And where does wound care, which occupies so much of podiatry practice, fall in this dual track system? I’d like to hear opinions of other PM News readers.
