Don't Ignore Foot Problems in Children: MD Podiatrist
 
Many children suffer from foot problems that often go undiagnosed. There are many symptoms parents can watch for, says Dr. Bradley Lamm, a Baltimore-area foot and ankle surgeon and fellow of the American College of Foot & Ankle Surgeons. Many parents think children can outgrow foot problems, but Lamm said it is important to get treatment.

"A common misconception I see in patients who previously visited a physician or pediatrician is that children will grow out of foot conditions like pediatric flatfoot, which is simply not true. Many children may have in-toe or bow legs, which can be normal, but others may be abnormal. A comprehensive evaluation by a foot and ankle surgeon is necessary to determine the diagnosis and whether it needs treatment, monitoring, or nothing at all. The long-term consequences of not treating these conditions can include pain, joint arthritis, muscle weakness, overloading of the opposite foot and ankle, deformity, inactivity, and mental frustration," Says Lamm.

"Your foot’s arch moulds itself to whatever shape you put it in, so if you live in Converse, then you’ll have flatter feet than someone who has built up their arch support by wearing heels more often. Likewise, someone who wears heels every day may struggle to wear flats again, because the instep is now so high and the Achilles tendon has shortened."

On June 23rd, author and podiatrist Peter Allton will launch his book Undefeeted by Diabetes: a Step-by-Step Guide to Keep your Feet Healthy for Life. The book is aimed at those with diabetes, their family members, friends, and those at risk of developing diabetes.

As someone who lives with Type 2 diabetes, Peter knows how important education is to managing the condition. When his daughter was diagnosed with Type 1 diabetes at the age of 11, Peter decided to take action, start the global Undefeeted movement, and promote good foot health for diabetes sufferers. Peter has 27 years of experience as a podiatrist, having qualified as a podiatrist in 1988. He has since treated over 200,000 feet.

Anthem Touts Telemedicine
 
Anthem has announced LiveHealth Online, a non-emergency telemedicine option for members in Connecticut. The program was launched late last year to certain segments of Anthem’s membership. Participants can use their smartphone or computer to introduce a live video visit with a board-certified doctor. Doctors are available 24 hours, 365 days a year, including holidays, Anthem says. A LiveHealth Online visit costs $49 but because out-of-pocket expenses for Anthem member visits are calculated “real-time” at the start of the visit, many members will pay less, depending on their health plan benefits, according to a prepared statement. 
 
Anthem is making a version of LiveHealth Online available to doctors in its provider network, starting with primary care organizations. ProHealth Physicians, Connecticut’s largest independent primary care practice, will be the first medical group in the state to implement the Practice Edition. Anthem says it is in active discussions with several other large physician organizations across the state.

Dr. Udell appropriately indicates that the ordering of vitamin B12 levels may be appropriate in the patient with diabetes. Several other factors should be considered. 
 
Vitamin B 12 deficiency is commonly encountered in diabetic patients not taking metformin. Jawa, et al. (Endocrine Practice, 16, 2010 ) noted in their study that as many as 48% of patients with diabetes not taking metformin demonstrate low levels of vitamin B 12 and that B12 deficiency is common in the diabetic patient. Carnell (Clinical Nutrition 94, 1 2011 ) addressed the possible unreliability of...

Dr. Stanley's scenario underscores that doctors using EHR systems where the data is stored "in a cloud" should make paper back-ups of patient records.
 
Many doctors lulled into EHR because of stipends for making "Meaningful Use" are becoming disenchanted because of audits, complex time-consuming systems, and difficulty satisfying the requirements at upper levels. Hence the pool of doctors buying these programs might shrink, causing many of these vendors to go bankrupt. The government regulates and taxes the companies that make and sell EHR software. Why it did not place a requirement on these companies that protect the interest of doctors and their patients is anyone's guess. Be it as it may, we have to protect our patients and ourselves from losing their records. The only way is to keep a paper back-up in addition to what is "in the cloud."

I hear about medical students and residents spending less and less time with patients. I think the AMA should be more concerned with the lack of time spent on patient care.
