MI Podiatrist Uses HBOT to Heal Decade-Old DFU
For 13 years, Kevin Campbell suffered with a wound that would not heal. Three years ago, he sought help from Dr. Heidi E. Monaghan of Midland Podiatry Associates. “I was to the point of having it amputated, but Dr. Monaghan said she wouldn’t give up on me,” Campbell said. The wound, though originally caused by the trauma of an accident, was diagnosed as a diabetic ulcer. While treatment did reduce the size of the wound by half, it did not totally heal.

Campbell’s treatments included debridement, which involves removing damaged or infected tissue. He also had specialty dressings applied, including biological skin substitutes that work like skin grafts. From Campbell’s point of view, the most unusual treatment was hyperbaric oxygen therapy. Campbell had a total of 40 treatments in the hyperbaric oxygen chamber. While his wound is classified as healed, Campbell knows he will have to closely manage his health for the rest of his life.

Keep Flip-Flops Dry: MA Podiatrist
Just because they’re fun to wear, doesn’t necessarily mean flip-flops are functional. Most of them lack the right support to stabilize your foot; this will cause pain if you’re walking a lot. Plastic flip-flops are more likely to cause blisters which can get infected. Also, try to keep them dry after you swim. “Some people even wear them in the water. Then they wear them all day and don’t dry them out. Mold starts growing and your foot is susceptible to fungus and mold,” said Dr. Benjamin Dickert, a Springfield podiatrist .

Believe it or not, the Consumer Product Safety Commission reported there were more than 25,000 emergency room visits in 2014 because of flip-flops! Dr. Dickert said there are a few brands of flip-flops he’d recommend though, including Spenco, Vionic, and FitFlop.

House Bill Seeks ICD-10 Grace Period
With the October 1 ICD-10 deadline rapidly approaching, another member of Congress has introduced legislation that would establish a “grace period” following the compliance date during which providers could not be denied Medicare/Medicaid payments because of coding errors.
Rep. Gary Palmer (R-AL) has sponsored the “Protecting Patients and Physicians Against Coding Act” (H.R. 2652) which calls for a two-year grace period so that providers can “focus on patient care instead of coding and receiving compensation for their care while ICD-10 is being fully implemented.” With ICD-10’s five-fold increase in codes compared to ICD-9, Palmer warned that the code switchover creates significant administrative challenges for rural and small town providers in particular, who lack the resources to fully prepare for the ICD-10 implementation.

AMA Supports EHR Training for Medical Students
At its annual meeting on Monday, the American Medical Association adopted a new policy to provide medical students with “hands-on” experience with electronic health records to improve patient care and increase the accuracy of clinical communications. Specifically, AMA’s policy recommends that medical students—with appropriate supervision—learn as part of their education, how to document patient encounters and enter clinical orders into patients' EHRs.
Towards that end, AMA has committed to working with medical school accreditation bodies to support U.S. medical schools, as well as residency and fellowship training programs, in teaching students how to use electronic devices in the examination room and at the hospital bedside. The new policy calls for “determining the characteristics of an ideal software system” for teaching EHRs to students that could be used at medical schools and teaching hospitals.

Choose the simplest, most straightforward surgery that will do the job. Consider met head plantar planing, simple osteotomy, or other straightforward small procedures. The goal is prevention and improvement, not perfection. Using biomechanics to assess how to alleviate pressure is key. Take care to not cause transfer lesions. Also, consider delling-out of the orthotic under the lesion, and fill with Poron; then use a 1/16th inch Spenco/soft topcover.

This is a perfect case to apply the Leventen formula while performing the distal osteotomies using minimally invasive surgical techniques. As far as the forefoot is concerned, without seeing the patient, I would consider a 2nd and 3rd MIS distal osteotomy of the right foot and a 2nd, 3rd, and 4th MIS osteotomy of the left foot. Also, evaluate to see if a gastroc recession is needed which may preclude the osteotomies. Performed with minimally invasive techniques, there is minimal healing time and disturbance of circulation, and the results in healing sub-met ulcers and especially preventing reoccurrence are almost miraculous.

It would be helpful to have pictures of the feet and measurements of at least the dorsiflexion of the foot. Dr. Monroe Laborde does a lot of gastroc recessions and other soft tissue procedures for these high-risk diabetic patients. The gastroc recession has proven to be an extremely valuable procedure for taking weight off the ball of the foot.
MIS metatarsal osteotomies and MIS toe straightening surgeries are excellent for correcting and preventing metatarsal head ulcerations. The treatment of toe ulcers, crooked toes, and metatarsal balancing with or without dislocated MPJs is a topic that...

Kudos to Dr. Jacobs for citing this paper. This reminds me of what one of my professors taught us at the New York College of Podiatric Medicine, over 30 years ago. He said "don't be lulled into making snap diagnoses."
All too often, when a patient presents with symptoms of peripheral neuropathy and a history of diabetes, we are all too quick to put down in the chart, "diabetic neuropathy." A significant number of patients with diabetes present with symptoms associated with peripheral neuropathy. As the article Dr. Jacobs recommended points out, in some cases the cause might be a vitamin B12 deficiency. As Dr. Richard Mann has often shown, the symptoms can also be caused by a vitamin B1 (thiamine) deficiency. Hence, it is prudent before initiating a plan of treatment for these patients, to order vitamin B12 and plasma thiamine levels.

I sympathize with Dr. Benjamin’s plight. These policies are pervasive in the EMR industry. I purchased an EMR program 6 years ago, for a rather large sum, which was located on MY server and the data backed up on MY external drive. Unfortunately, that firm filed for bankruptcy and was acquired by a new entity. Due to the reasons mentioned by Dr. Benjamin, I switched vendors as well. I have now been locked out of MY software still located on MY server unless I pay an outrageous sum. I am not sure what I paid for if I cannot access the software or my data. There are three years of medical records present in that software that were entered prior to their acquisition. I do not take being held hostage kindly. It is time for legislation and/or EMR licensing rules that require a read-only file be provided at no additional cost in the event of a termination.

I tried it on just to experience the ridiculousness of it all. My foot had absolutely no platform to rest on. I have no clue why or how this came to be, but nevertheless, it stands as the most "outrageous shoe" I've seen, probably in my entire life.
