IL Podiatrist Discusses Cycling and Foot Health
With the weather warming up in Chicago, many residents will take to bicycling to work or recreationally. Podiatrist Dr. James Lavell understands the convenience of cycling around the city, but urges cyclists to be aware of their foot and ankle health.

"The foot is the contact point with the cycling pedal. Because of this, foot problems are not uncommon with bicyclists. Most of these complications can be easily fixed and prevented. As long as the bones, muscles, and joints of the lower extremity are properly aligned with each other, there is an efficient and pain-free excursion of the pedal," said Dr. Lavell.

Common Sense With Flip-Flops Prevents Injuries: NC Podiatrist
From poolside to an evening out on the town, for many people, flip-flops have become a summer staple. This popular style of footwear, however, may not be the best choice for every situation, an expert warns. “While flip-flops are perfectly fine — and far better than going barefoot — in a number of settings, they do have some drawbacks,” said Dr. Christina Sigur, a podiatrist and instructor in orthopedics at Wake Forest Baptist Medical Center.

IL Podiatrist Sentenced for Videotaping Naked College Wrestlers
Dr. Ninos Danny Jando, the Illinois foot doctor who pleaded guilty to secretly videotaping Muskegon Community College wrestlers while they showered, was sentenced to nine months in jail Monday. Jando has already served nearly half that sentence – 133 days – since his arrest in January 2013. He said his time in jail has given him a chance to reflect on his actions. “I never really thought about … the consequences of my actions,” he said. “I’ll feel this guilt and remorse for the rest of my life. I’m disappointed in my life. … I don’t want to disappoint myself now. That’s why I vow never to repeat these actions,” Jando said.

Query: Tibial Sesamoid Fracture Coding
What is the correct way to code a tibial sesamoid fracture (ICD-9) and the fracture care (CPT)?

Response: Is it a closed or open fracture? The reason I mention it is that come October 1, 2014, in ICD-10 you must state whether it is open or closed, displaced or not displaced, etc. so it may be a good thing to start documenting that detail. ICD-9 tibial sesamoid fracture coding is "fracture of other tarsal and metatarsal bones, closed; other" - ICD-9 825.29.
If it were an open fracture, then it would be coded as "fracture of other tarsal and metatarsal bones, open; other" - ICD-9 825.39. If it were a stress fracture, it would be coded ICD-9 733.94 - "stress fracture, other bone." The fracture care coding is based on the services you provide:
CPT 28530 - closed treatment of sesamoid fracture or CPT 28531 - open treatment of sesamoid fracture, with or without internal fixation.

I read about the new graduates who do not have a residency and feel sad, and frankly embarrassed that we, as a profession, have done this to them. I have a great idea. Have whoever makes the decision on opening new schools open even more of them. That will work out great, right? That is what happened over the last several years. Are you kidding me? I spoke with others about this a few years ago, and I hate to say I told you so, but….It is ridiculous that we allowed that to happen, and now have students who are being cheated out of advanced training, with huge debt and no way to get the training to pay it off.
First, we need to strongly look into who allowed that to happen and do something about that leadership. It is completely irresponsible to...

RE: Bigger Heels Equal Bigger Tips for Casino Waitresses: CT Podiatrist

Being from Las Vegas, the cocktail waitress capital of the world, this has been something we deal with on a regular basis. Nearly all of the casinos have adopted a "reasonable and comfortable" shoe policy. Caesars Palace was one of the first casinos to allow a more casual sandal, as many of their waitresses were "older" and unable to wear the high heels.
Several years ago, one of the things we as podiatrists were doing was to write shoe modification letters for our waitress patients so they would be allowed to wear a lower heel or an open toe. Over the years, the casinos saw the problems the high heels were causing for their waitresses and nearly every casino began to allow a more sensible shoe to be worn. Most of the waitresses have gone to a low dance heel and many of the problems have been eliminated.
One of the last holdout casinos, the Imperial Palace, finally relented as well just before they closed down for remodeling and "rebranding."

RE: Staff Performing Routine Foot Care

There is a direct complication when it comes to staff performing ANY service that insurers require that physicians do themselves. RFC is one of those, but there are others as well, like removing sutures, putting on and removing casts and braces, and even minor surgery. When Government insurance is involved, these rules become the force of Government itself. None of us has the resources to defend ourselves against vengeful Government, seeking to destroy us as an individual for their charge of "fraud."
Certainly RFC will remain a necessary and valued service as our society ages. Many podiatrists still continue to offer this treatment, which is basically a non-covered service that patients still expect Government to pay for. We have an uphill job explaining to patients (and their families) that fraudulently billing Government for non-covered services, no matter who does them, is illegal.
The entire issue of staff performing non-covered services requires expertise of our best advisors on the legality of billing. This means using and properly executing "Advance Beneficiary Notices" with complete, correct knowledge of their use. There is the mistaken belief that just because you don't accept Medicare for a service "excuses" you from following Medicare law, say if the patient submits the bill instead of you. My bottom line is that we should be offering training and seminars on the PROPER ways to manage non-covered services of all kinds. It's either that, or you run the risk of getting yet again into serious trouble. It's a mine-field out there.
