TX Podiatrists Troubled By Public’s Confusion About Specialty

Notwithstanding a daunting caseload of nearly 7,000 patients, Dr. Anna E. Sanchez and her husband, Dr. Michael E. Roth, are troubled by public confusion about their medical specialty: Podiatry. It’s not a crisis of identity, but rather a hurdle of understanding.

There’s little need to attract more patients to the Del Rio Foot Clinic, but the husband/wife team fears that many victims of foot problems aren’t being seen simply because of misconceptions. “When you say ‘podiatrist,’ “people are thinking ‘psychiatrist,’ ‘pediatrician,’ or who knows what else,” Sanchez lamented.

“You can characterize us as lower extremity physicians,” Roth chimed in. “Other doctors send us patients with foot problems.” So, the initials behind Sanchez’s and Roth’s names are D.P.M., Doctor of Podiatric Medicine. “We work from the lower spine down, including the foot,” Sanchez added. To be licensed to cover that turf, the foot docs underwent rigorous training, and continue their education today.

CA Podiatrist Recommends Best Sandal Features

Sexy summer sandals that aren't murder on your tootsies are gaining a toehold in the fashion world. "Footwear has come a long way in the last 10 years," says Rich Johnson, owner of Foot Solutions in Riverside. He notes the proliferation of brands, mostly European, that offer flexibility and support for arches and long bones of the forefoot.

Otherwise, cautions Dr. Jan Tepper, a podiatrist in Upland and a board member of the California Podiatric Medical Association, you'll be prone to injuries and pain that can kick up in the feet, ankles, lower back, hips and knees. The best sandals, he says, should have straps across the top of the foot behind the toes, in front of the ankle and behind the heel. "The bottom of the sole should be as close to a tennis shoe as possible, not a smooth surface like a flip-flop."

Forbes Magazine has ranked podiatrist number 15 on the list of the highest paying professions, outpacing both lawyers and dentists.

Mean Annual Wage: $118,500
One-Year Change: 6.5%
Top-Paying Industry: Outpatient Care Centers ($129,310*)
Top-Paying State: Oregon ($176,740*)
Top-Paying Metro: Orlando-Kissimmee, FL ($184,780*)

Diagnose and treat diseases and deformities of the human foot.

*Remember, these are mean salaries and that they give no indication of how distant the outliers at either end of the salary scale for any occupation might be. There are plenty of lawyers that earn a lot more than the average $113,660, and surely there are dishwashers who earn a lot less than $16,190.

Query: Treatment for Intra-epidermal Pustular Dermatitis

I recently received a biopsy report on a patient who had been exhibiting a recurrent, itchy, pustular rash on both feet in a typical moccasin distribution. She had been treated by other providers many times in the past with various forms of topical antifungals and corticosteriods with no success.

Intra-epidermal Pustular Dermatitis

The biopsy report diagnosis was intraepidermal pustular dermatitis. A comment was added that the histological pattern seen was consistent with palmoplantar pustulosis, which is considered by some authorities as a variant of psoriasis. PAS stain was negative for fungal organisms. I would appreciate some advice on a treatment regimen for this patient.

I am honored to be acknowledged by both Sharon Day and PM News. It should be pointed out that some of the story was not entirely accurate (such as saying she could get back in 6 weeks) and the reality is that if she didn't have such a competent, caring and conscientious DPM who referred her, I may not have had the opportunity to help in her successful return, (she took 4th in the NCAA finals jumping 6 ft). She had a difficult fracture, the type that I have also seen, which sometimes results in non-union, despite good fixation and post-operative care. Sometimes it is better to be lucky than good.

RE: Scope of Practice Question (John Renard, DPM)

I congratulate Dr. Renard on the respect you have garnished from the vascular community and your local hospital to expand your privileges compared to what most other podiatrists in your area may not be performing.

In order to protect yourself from legal issues in performing a procedure you are not sure you are licensed to perform, I would suggest asking your state board of podiatry for a declaratory statement. It probably would work best for you to obtain the proper training, both in an academic setting and with your local vascular surgeon first, and then go to the state board and ask for the declaratory statement, or special privileges for this procedure.

It would be entirely up to your state laws. Here in Florida we are able to treat soft tissue problems of the entire lower leg, only bone issues distal to the tibial tubercle.

My partner and myself have been harvesting thigh grafts for the purpose of coverage of lower leg wounds for many years. We too were trained by a surgeon outside of podiatric education. This is an easy procedure to perform and has a shallow learning curve. Any surgeon capable of doing foot and ankle surgery who is properly trained
could master this technique.

I would simply check your state law and if allowable, encourage you to get the training.

When I first came to this city to interview for positions, I was taken into the O.R. by a local podiatrist to observe a case. I assumed at the time that I had permission, but in retrospect, that may be up for debate. He was placing a skin graft on the ankle of a person with a Brown Recluse spider bite who had been in-house for a while for the condition. I was astonished that he took the graft from the thigh with the dermatome, with no general, vascular or plastic surgeon in sight. He obviously knew how to operate the dermatome and the placement of the graft was without incident, but it left me curious how he got privileges for something that is clearly out of our scope of practice here in Texas. I also know how to use a dermatome but would never consider taking the graft without help as it could cost you your license and would be indefensible in the courtroom.
