NY Podiatrist Explains Why Children's Shoes Should Not be Bought Online
 
Today’s busy parents are increasingly turning to the Internet to do their shopping — from buying groceries  to appliances. But, shopping for kids’ shoes should be left to the expertise of a shoe fitting specialist in a brick-and-mortar store, according to New York-based sports podiatrist Lori Weisenfeld, DPM“I have parents who bring in their kids [complaining] of pain in their toes or ingrown toe nails,” said Weisenfeld. “It can be that the shoe is too small. Kids grow quickly.”

While parents may think achieving a proper fit means simply checking for the right length, you need to think again. There are a range of considerations when fitting a child for shoes. “It’s important to know if your kid has flat feet, or feet that turn in or out,” explained Dr. Weisenfeld, while some children pronate and therefore may require additional support in their shoes. “It’s important to have children evaluated by someone who can check their foot type and direct you to the right shoe, and also important to have a professional such as a podiatrist check them out.”

High Achieving MA HS Grad Aims to be a Podiatrist
 
Jaila Marie Smith never has been shy about setting her goals extremely high. Whether it was attending 12 years in the Brockton school system without missing a day of class, earning membership in the National Honor Society while playing two sports, or being involved in numerous school activities, Smith has accepted every challenge and done it with a smile on her face. Smith will  study pre-med at Howard University in the fall with the aim to be a podiatrist.

“I’ve always had problems with my ankles, so I wanted to do something in a medical field that deals with it, so that’s why I wanted to become a podiatrist,” said Smith, who credits her father, mother April, as well as older brother Marcel for being positive role models in her life. “I visited UCLA in the summer of 2017 and was amazed with all the medical technology they had there. Then I visited Howard because I knew they had a good medical program and I just loved everything about the school.”

A strategic plan is not just a document written by physicians and managers and then filed away for safekeeping. It is a vision for the practice, owned by the practice. For a medical practice to succeed, everyone must engage with and live and breathe the plan. Here are 10 questions to assess your strategic plan.
 
4. Does your strategy put you ahead of the trends? Many strategies place too much weight on current trends. But as the founder of modern management theory, Peter Drucker said, it is not the trends, but “the changes in the trends” that leaders must stay current with. These changes often creep up so slowly that most practices fail to act until it is too late to mount an effective response, let alone take advantage of it. They are held back by sunk costs, an unwillingness to cannibalize a legacy business, or an attachment to yesterday’s formula for success. Instead, you should always look to the edges. How are your patients shopping for healthcare? What are the innovative new entrant competitors doing? What innovations could change your entire specialty?

Query: Billing for C-Arm During Surgery
 
Can I bill for using a C-arm to assist in hardware placement during surgery? The images are all taken while in the operating room. If this is appropriate, do I need a specific modifier for the case? Can I use the same CPT code for the surgery as the intra-op x-ray or does it require a different CPT code?

Response: While I normally do not see the billing of a c-arm during surgery, in this case, the most appropriate code that can be used is code CPT 76000: Fluoroscopy (separate procedure), up to 1 hour physician or other qualified healthcare professional time. When billing, I would add modifier -26, indicating the professional component of the service being billed was “interpretation only.” It is most commonly submitted with diagnostic tests, including radiological procedures.

It would be very useful for all podiatrists working with the elderly to have access to a well thought out approach to assessment and documentation of fall risk. My practice has attempted to get the exact requirements and documentation needs from CMS with only vague answers such as "Just try to make an attempt." If we are going to do this, we would like to make sure it is an exact fit with what is required and not a "best attempt."
