CA Podiatrist Helps Runner Win NCAA Championship

His real name is McClellan Verne Fleet, but he’s better known in these parts as Mac Fleet.  Mac Fleet was on his way to becoming one of the next greats among the Men of Oregon. But a foot injury derailed most of his sophomore season, and just as Fleet was working his way back, another, much worse, injury surfaced on his other foot. “I was at a point last year, over the winter, where I actually thought I’d never be able to run, not even competitively,” said Fleet.

He had developed plantar fasciitis in his left foot, and he could no longer do what brought him to Oregon. Finally, there was a break. On his way back up to Oregon in the fall, he saw foot specialist Dr. Amol Saxena in Palo Alto, California. He was given a cortisone shot, a shot that injected new life into Fleet’s running career. “Immediately in the next couple of days, I knew that something was different,” said Fleet. “I knew that I’d be able to start running again.”

Bigger Heels Equal Bigger Tips for Casino Waitresses: CT Podiatrist

Cheryl Haase has been serving cocktails for more than two decades, and the 52-year-old waitress says she has a list of foot ailments to prove it. The miles she has walked carrying trays of drinks through the Foxwoods Resort Casino in high heels have sent her over the years to chiropractors and podiatrists for injections to treat inflammation. Now, she and a union representing her fellow cocktail waitresses at the country's largest casino are fighting for the servers to wear shoes of their own choosing.

A Connecticut podiatrist, Dr. Eric Levine of Norwich, said he frequently treats casino waitresses who wear high heels. He said many who come to him with painful conditions return to work with a recommendation to not wear high heels for medical reasons, but some waitresses confess they won't give them up easily. "Several have told me they're not going to stop wearing them. 'The higher the heel, the larger the tip,'" he said.

Leonard LaRussa, doctor of podiatric medicine, has passed the Physician Wound Care Certification Examination, according to the American Board of Wound Healing (ABWH). Passing this exam confers special competency via Certificate of Added Qualification in Wound Care.

This is LaRussa's second year as president of the Georgia Podiatric Medical Association. He  has served the Georgia Podiatric Medical Association as their Carrier Advisory Committee representative since 2008. He Is a Georgia Delegate to the American Podiatric Medical Association, and has been appointed by Governor Deal to serve on the state Board of Medical Examiners.

NY Podiatrist Honored as Oldest Yankees Fan

The New York Yankees recognized their oldest living fan recently, a Black Brazil-born New Yorker who, at the age of 111, can say, more than anyone else, “been there, done that.” Dr. Bernando LaPallo can say, for instance, he saw, from a seat in the bleachers, Babe Ruth, Jackie Robinson, and scores of other baseball legends play.

LaPallo says he was born in Brazil on Aug. 17, 1901, and later moved to the United States with mother Mattie Carr and Bernando LaPallo, Sr. He said he studied culinary arts in Paris and then pursued a career in healing arts. He studied massage at The Swedish Institute in New York and later became a reflexologist and podiatrist.

QUERIES (CLINICAL)

Query: Unusual Lesion

This patient presented to my office simply because his neurologist noticed the lesion and wants it checked. The patient states that this has been present about 20 years, and he recalls no history of trauma. With the exception of cervical disc problems, he is in good health. The lesion has grown very slowly since it was first noticed. It has always been black. 

Undiagnosed lesion

It has always had the raised border. There has been no treatment. He denies easy bleeding or pain. There is no itching. The smaller non-pigmented lesions have also been present. Under a dermatoscope, there are halos of bluish and whitish areas. I took a punch biopsy.

RE: Recurring Skin Lesions

Radiographs would be helpful to determine underlying factors contributory to the redevelopment of these lesions. However, I suggest it 
is possible that the third metatarsals are elongated relative to the adjacent second metatarsals. Such elongated metatarsal positions may be contributory to such biomechanically-induced lesions on the plantar aspect of the third toes seen despite toe arthroplasties being previously performed.

If the third metatarsals are elongated relative to the adjacent metatarsals, I suggest orthotic management, or shortening osteotomy of the third metatarsals if orthotics fail.

RE: Recurring Skin Lesions

This is a case of biomechanical surgical iatrogeny. These are not recurrent lesions as Name Withheld has labeled them; they are transfer lesions. The surgeon has stripped the distal segment of the 3rd digits in question from being functional in closed chain. The patient has morphed mechanically to a compensatory equilibrium via Wolf’s and Davis’ laws. That is giving a new location of the digits weight-bearing purpose which they are not biologically capable of assuming, resulting in this pathological dermatological reaction.

Perform a biomechanical evaluation of the digits, beginning with functional foot typing, and take a biopsy of the lesion. Send the specimen to a pathology lab (such as Bako) that can give you a biomechanical microscopic reading of the slide. Utilizing that data, develop a non-operative and/or operative plan of care to resolve this painful and disabling problem.

I assume the licensure you refer to has to do with orthotists/pedorthists and their scopes of practice, and not municipal or other commercial licensure issues. Some states require the entity interfacing with the patient to be licensed. However, that does not appear to be your scenario. Since no employee or official from the lab ever interfaces with the patient, you are the only entity subject to licensure.

RE: Compounding Pharmacies (Marc Katz, DPM)

I certainly appreciate and understand Dr. Katz' concern. With all due respect to Dr. Jacobs (lecture appreciated), just do a Google search on compounding pharmacies, and the Dept. of Justice or FDA. Read the blogs from the reps, and see the commissions they discuss. Read about the different ways some of these upstart pharmacies attempt to bypass Stark Law to enrich physicians. I venture to guess that a few of you have been approached with arrangements that do not pass the "smell test".  
 
I have been happy, as many others, with results from compounded topical Rxs. My advice would be to use a local pharmacy you trust. I personally destroy the pads I have received from reps and pharmacies from out of state. I feel this niche of the industry popped up and expanded too fast. It is greed that drove this, not anything else. These compounded creams have been around a long time; and they did not become so popular because they are affordable. How many ultrasound companies are detailing you right now? I assume that Dr. Katz decided to stir the pot, because he saw it bubbling over. I agree.

Let me start by saying that the tone in Dr. Brower's comment on PM News was full of criticism, negativity, and bitterness. I am very shocked to hear such a closed-minded comment. Dr. Brower, you need to keep up with your readings of medical journals. The percutaneous bunionectomy that I perform was published in JBJS a few years ago. It was a five-year follow up of 118 feet with 91% satisfaction rate.

RE: Unmatched Residency Placements Currently Stand at 92

I want to  point out the injustice done to podiatric medical graduates. As of the last report, there still are 92 qualified graduates who remain unmatched but deserve a residency slot. Several practicing podiatrists have voiced their acknowledgement of this unfair scenario.

I am a graduate of 2012 who passed his NBPME steps 1 and 2 in my first attempt and stand in the middle half of my class. I was not successful in attaining a residency spot last year or this year because of the extreme shortage of residency spots. I believe I prepared myself really well for interviews this year and had 24 interviews. The residency directors who interviewed me told me how well I had performed in the interviews. However, I still did not match. This is all due to the drought of the number of enough residency positions. I understand that this has happened in the past, but the podiatrists could practice without residencies back then. Not so anymore.

I believe all the podiatric organizations and the schools are to blame for this debacle. Something has to be done for the 92 graduates. These graduates are in $250,000 debt, on average. All of us have no way of paying back our student loans or gaining residencies that we were promised if we passed our board exams and graduated from podiatric schools.
