The New York College of Podiatric Medicine (NYCPM) coordinated one of the three clinics offered entitled, “Identifying Lower Extremity PAD Peripheral Arterial Disease.” The clinic involved a podiatric medical screening, general evaluation of orthopedic foot deformities, dermatologic conditions, taking pedal pulses, and using a Doppler to locate pulses if not palpable.

CRHC Adds IA Podiatrist to Medical Team
 
Clarinda Regional Health Center (CRHC) is adding a new podiatrist to its medical team. Hospital officials announced Friday that Nathan Shumway, DPM has began his practice at CRHC.

Dr. Shumway earned a Doctorate of Podiatric Medicine from Des Moines University. Shumway then went on to complete a three-year surgical residency at the Veterans Affairs Eastern Colorado Healthcare System, where he specialized in reconstructive foot and ankle surgeries.

MEDICARE ADVANTAGE IN THE NEWS
Blue KC Exiting MA Market by 2025 Due to 'Regulatory Demands'
 
Blue Cross and Blue Shield of Kansas City (Blue KC) is leaving the Medicare Advantage (MA) market at the end of 2024, the insurer announced recently. The company blamed “heightened regulatory demands and rising market and financial pressures” for the decision but said it is still focused on employer-sponsored health plans, and Medicare supplement and Affordable Care Act plans in the state.
 
“We explored every alternative path for our MA members and are disappointed we must exit this line of business,” said Erin Stucky, Blue KC President and CEO, in a statement. “We value our MA members and are committed to providing uninterrupted, quality service to our current MA membership through the end of 2024.”

QUERIES (NON-CLINICAL)
RE: Stealing Associates
 
We were approached by a company for practice acquisition. After we signed the non-disclosure agreement (NDA) and handed them our financials, they asked for associates contracts and collections by each associate. After a month of negotiations, they came back with an excuse that our practice makes a lot of money and if they acquire our practice, they will have to pay the associates higher salaries. 
 
One week after I informed our associates about the possible merger, one associate suddenly resigned without telling us to where the associate was going. A month after, the other associate resigned, and we found out that they were hired by the group. They used our financials basically to destroy our practice and to steal our associates. We just want to make sure that this does not happen to other practices. Any word of advice is appreciated.

RESPONSES/COMMENTS (CODING AND BILLING) - PART 1A

The simple, most accurate and direct answer would be NO! However, support and justification for this answer is necessary. If a patient has PVD with ulcers that need to be debrided, and the ulcers are painful necessitating the administration of a nerve block such as a local sympathectomy of the posterior tibial nerve in order to perform the debridement(s), the necessity of the nerve blocks does not justify their reimbursement. Why?
 
The CPT codes that are used for the debridement of necrotic tissue out from within the base of the ulcer (i.e., 97597, 11042, 11042, 11044) are all classified as minor surgical procedure codes that have a post-operative global period of “0” days. When one accesses the “Global Policy for Minor Surgeries (0 or 10 day follow-up),” the 3rd item that is listed is the following: “ALL intra-operative services such as a local anesthesia, injections, dressings, casts, splints, post-op shoes, wires, pins, supplies, etc. that are a necessary part of a surgical procedure are included in the surgery fee.” I don’t think that it can be any more specific than this.
 
However, for those that are skeptics and don’t believe the above, here is more proof and support. Just access the NCCI edits. The CPT code of note that is used for the nerve block is CPT code 64455 which is defined as the following: INJECTION(S), ANESTHETIC AGENT(S) AND/OR STEROID; OTHER PERIPHERAL NERVE OR BRANCH. When the NCCI edits are obtained, ALL of the ulcer debridement CPT codes (i.e., 97597, 11042, 11043, 11044) are the Column 1 code(s) to CPT code 64455, the Column 2 code. Therefore, since the administration of the nerve block is directly related to the debridement of the ulcer(s), CPT code 64455 is bundled with ALL of the ulcer debridement codes and is not separately reimbursable.

I would say, not likely... if not no. This would be looked at as an attempt to get paid for a nerve block for the debridement (which is not separately payable) vs. sympathectomy. You certainly can try with the appropriate ICD for vascular insufficiency and very distinctly and clearly document the reasoning behind such a block.

I would expect denial, and you would need to appeal and explain with your notes the clinical reasoning and medical necessity. I would be surprised as it will appear to be "gaming the system" to get paid for the block. Unfortunately, not all of our well-meant attempts to assist the patients are paid for, PT blocks being a big one in many cases of vascular and neurological pain.

Respectfully to Dr. Maleski, the One Board proposition wasn't developed for his generation. It is developed for the current and future generations of podiatrists who are ALL "surgical podiatrists." Since 2003, according to CPME standards, EVERY residency graduate has been trained in podiatric medicine AND surgery. Whether a podiatrist chooses to do surgery after residency is irrelevant. Just like all OB/Gyns or ophthalmologists are trained to perform surgery and some decide to focus on a medical practice afterward, they are still board certified in what their residency training was. 
 
Other medical specialties don't have 2 boards. There is only one recognized board for every MD specialty, many containing a mixture of surgery and non-surgery. Subsequently, your privileges at the hospital or ASC are determined by your education, training, and current experience (surgical cases).
