Avoid Summer Injuries from Walking Barefoot: OH Podiatrist
Abigail Elliott, DPM, a board-certified podiatrist with Knox Community Hospital, explained how to care for feet, what warning signs to watch for, and how to select proper shoes. “Recently, we've been seeing a lot more puncture wounds, meaning patients are stepping on things because they're walking barefoot,” Dr. Elliott told the Mount Vernon News. “Just in the last week or two, we’ve had quite a few patients with glass in their foot or a splinter because they've been walking barefoot.”

Walking barefoot in the summertime can be something people look forward to, but Elliott said it’s best to wear shoes outdoors. “You really shouldn’t do that just to avoid injury to the foot,” the podiatrist said. “Walking barefoot can expose you to viral infections like plantar warts or fungal infections like athlete’s foot or ringworm.” Even around the house, socks, slippers, or a pair of flip-flop sandals are recommended, she said. This is especially important for those with diabetes, where the foot can be less sensitive and more prone to slow-healing wounds.

Warmer Weather Brings Out Foot Fungus: MI Podiatrist
According to Conway McLean, DPM, "With the increasing temperatures of summer upon us, activity levels are rising as well. Moisture levels naturally rise, creating a wonderful place for fungus. These are opportunistic infections, and are out and about, simply part of our environment. To flourish, they simply need the right opportunity. A closed-toe shoe, a sweaty foot, a shoe material that doesn’t breathe, all these are part of the complex equation leading to the development of a fungal infection of the foot."

"Although tinea pedis has been the subject of innumerable jokes, it can be a serious matter. Besides, who wants to live with a fungus among us? Although some forms of athlete’s feet are quite subtle and easy to miss, medically-speaking, living with these organisms is generally not a good idea. Treat your feet well and keep them fungus free; they’re your ticket to your next walk," says Dr. McLean.

Query: Medicare Advantage Challenges
BCBS Medicare Advantage in Michigan is stating that CPT 64455, J3301, J1100 are not approved payable codes by Medicare so they denied my claim. This is what I billed:
CPT 64455 RT G57.61, M79.671
CPT 64455 LT G57.62, M79.672
J1100 A5 (our NDC modifier code, which works on all other insurance carriers)
J3301 A6
I appealed the claim and they sent back my appeal, requiring more documentation. I called Provider Services and they stated they need the office note (which I faxed with the original appeal) and the LCD from Medicare stating this is a payable code. According to their information, this is not a Medicare-approved code. I have not had this happen with any other insurance carrier. The really odd thing is I just billed a different patient with the same insurance the same codes (right only), and it was paid. Any suggestions?

Response: 1) This is a legitimate code. 2) The description of this code is for multiple injections. The descriptor states injection(s)–That little “(s)” is the problem. So what constitutes injection(s)- multiple on one foot or both feet? I think the podiatric community considers this to be on one foot since we sometimes inject two neuromas on the same foot. Is this fair? No, but that’s how it is. The use of the RT and LT triggered the denial. That is why you were paid on your other claim, not on this one.
So options would be to appeal indicating arguing the injections were on separate feet and it should be paid. The other option for this carrier is only to inject one foot per encounter which gets you paid… at the patient’s inconvenience.

RE: Tall People at Increased Risk of Peripheral Neuropathy

Analyses were performed using data from the U.S. Veterans Affairs (VA) Million Veteran Program in 222,300 non-Hispanic White and 58,151 non-Hispanic, Black adults in the U.S. It was concluded that tall people have an increased risk of peripheral neuropathy and skin and bone infections but a lower risk of heart disease, hypertension, and elevated cholesterol.

I found the two sides of the current state of podiatry income very informative. A few years ago, I decided to ”give” my 40 year old private practice away as it was time to retire. I did not want to leave my wife with a vacancy in her medical center. I came up with a novel idea of charging the new podiatrist $50,000 for all my equipment and all my charts and goodwill and require that he signed a three-year lease with my wife. I would stay on board to help with any transitions necessary and set up the new guy with all my current staff and found them a super capable office manager to take him into the future. I also told the new guy that I would accept $1500 a month until the debt was paid.

Dr. Block asks, “Who wants to be a podiatrist?” Dr. Fellner discusses how non-professionals appear to charge more for routine foot care than podiatrists.
Dr. Block notes that the applicant pool for podiatric medical schools is too small. That is interesting considering the fact that the applicant pool for medical schools is hitting record highs: This demonstrates that the lack of sufficient applicant numbers is a podiatry issue.
We can encourage prospective students and tell them about the wonderful services we provide as podiatrists, but I have to be honest about the challenges...
