Jamaican Podiatrist Discusses Gout
"At least twice a month, a patient will limp into my surgery with a very painful, inflamed big toe and the cause will be gout. The goal in treating gout is to relieve the symptoms and prevent further attacks. Relief is given in the form of non-steroidal anti-inflammatory drugs (NSAIDs) and these are prescribed. To prevent further attacks, medications such as allopurinol help to lower the levels of uric acid," says podiatrist Angela Davis.

"Patients are advised to elevate the foot as much as possible and to use an ice pack to help reduce the swelling while waiting for the medication to take effect. A podiatrist can make the patient orthotic devices to wear within the shoe to redistribute the pressure away from the affected area. Lifestyle changes such as weight loss and dietary adjustments are essential," says Davis.

Dr. Michael Hoenig, 28, recently moved to Fargo for a podiatry residency. For him, an apartment made sense. "I don't know what to expect from my first year of residency, so I don't want to deal with buying a house and fixing stuff up," he said. His one bedroom with a den feels spacious after living in a studio. "I just liked that it was new," he said of the complex.

QUERIES (CLINICAL) |
Query: Recurrent Fissures
My patient is a 58 year old female with extremely painful, recurrent fissuring on the soles of the feet which began several years ago. The fissures are not related to hyperkeratosis and are not seasonal. They are often accompanied by swelling. They occur intermittently with brief periods of remission. She has a history of ankylosing spondylitis for which she is currently taking Simponi, but has been on other meds for this in the past. She admits to eczema, but no psoriasis or dandruff. She also has a gluten allergy.

Recurrent Fissures |
Prior treatments include topical and oral antifungals, topical corticosteroids, Medrol dosepaks for acute flares, and wound care with various types of dressings to retain moisture including burn dressings. She keeps the fissuers covered and moist for comfort. Thus far, she has found no permanent solution. Any ideas would be appreciated.

The scenario described by Dr. Borreggine is only one glimpse of what is evolving with our healthcare system, and we are affected by it not just as healthcare providers but potentially as patients.
The physician I have been seeing for several years had his practice taken over by a corporate entity. In the past, if I asked him for a referral, he would give me a list of really top doctors in the specific specialty. Now, he only recommends doctors whose practices are also controlled by the same corporation. My insurance does not restrict me from seeing any doctors outside of this corporate network, but I rely on my PCP's advice since I might not know the strengths and weaknesses of every specialist in my area. I still trust my PCP, but admit that there is a little voice in the back of my head that questions whether he is being forced to make referrals by the "company office" as opposed to recommending whom he feels is the best in any given specialty.

The problem of the identity crisis of podiatrists being considered "physicians" by MDs and DOs is our lack of unified comprehensive education and training. Until all podiatry graduates undergo the same residency training and testing that the MDs go through, we will never be accepted in mainstream medicine. All NPs and PAs are accepted by MDs and DOs as sub-specialties and endorsed by them in training and work habit. NPs and PAs do not, for the most part, work independent of a physician in most states.
Podiatrists, despite their extensive education and training, assert their independent and unique identity which runs counter to physicians' control. We will never be endorsed by hard-lined, stoic, physicians. The future of global acceptance of podiatrists as physicians is doubtful, I'm afraid.

Dr. Gurvis has a number of valid concerns about the difficulty of securing our records and the illusion of security. Most hackers and thieves of information are looking for financial information; this information is found in our practice management systems.
Most of us have used practice management systems for many years. Blaming the issues we face protecting our data should not be laid at the feet of EHR. We need computer security to protect our billing records as well as our...

When a drug is studied, there might be multiple endpoints, and while both may seem related, in some instances, they are not. The drug in this study significantly reduced serum cholesterol levels. It seems logical that since there is an association between hyperlipedemia and heart attacks and strokes, this is all that is needed to place this medication on the shelves of drug stores. Unfortunately, this may not be the case. There have been other drugs that lower cholesterol but have not been shown to reduce cardiovascular incidents. There still is a piece of the puzzle that it missing, and unless the drug in question can be shown to reduce the risk for having MIs and CVAs, it may not be a good drug of choice for patients and physicians.
