MEDICARE NEWS |
Justice Department Cracking Down On Fraudulent MA Coding, Billing
The Justice Department is pursuing legal action against several major Medicare Advantage payers and providers for alleged fraudulent risk adjustment coding practices, according to The Washington Post. Physicians told the publication it is common practice for payers and health systems to "data mine" a patient's medical history if that individual is covered by Medicare Advantage. That data is then used to encourage physicians to maximize profit through such practices as documenting the maximum number of diagnoses or ranking physicians by how they code patients compared to their colleagues.
One Justice Department lawsuit against Palo Alto Medical Foundation, a subsidiary of Sacramento, CA-based Sutter Health, alleges the provider altered medical records to make patients seem sicker than they were. By adding risk adjustment codes to the Medicare Advantage patient's file, the provider could pocket additional reimbursement— equating to millions of dollars in overinflated bills. The lawsuit alleges an outside consultant found Palo Alto Medical Foundation submitted 8,000 false codes in 2012 and 2013. The provider settled the case in August 2021 for $90 million and admitted no wrongdoing or liability.

RELEVANT RESEARCH |
Evaluating the Effect of Omega-3–Rich Fish Skin in the Treatment of DFUs
This is the second of 3 planned articles reporting on a prospective, multicenter, randomized controlled trial assessing the efficacy of fish skin graft in the management of diabetic foot ulcers (DFU) in comparison with the standard of care (collagen alginate dressing).
Results: As of the time of this writing, 94 patients had completed the protocol. At 12-week follow-up, healing was achieved in 63.0% of index ulcers (29 of 46 patients) in the acellular fish skin graft group compared with 31.3% in the control group (15 of 48 patients) (P =.0036). In both groups, the mean time to healing was 7 weeks. The median number of applications of the fish skin graft to achieve healing was 6.
Conclusion: A clinically and statistically significant difference in healing was observed between patients treated with acellular fish skin graft and those treated with a collagen alginate dressing. The data support the completion of this prospective randomized trial.

PRACTICE MANAGEMENT TIP OF THE DAY |
4 Tips on Physician Practice Succession Planning
There are several options available if you are a private practice owner looking for advice on how to establish a succession plan. Here are four tips on getting started:
4. Communicate your succession plan
The last thing you want to happen as you create your succession planning is for rumors to spread about your departure. Rumors can become misunderstandings if you are not clear about who will be running your medical practice and when the transition will take place. You risk patients leaving and referring physicians sending patients elsewhere if they are concerned about the stability of your practice.
Once your plan is in place and the time is right, make sure to tell your patients and referring physicians and assure them that your successor will provide the same level of service they have come to expect while working with you. Communication is also essential with the person you are eyeing to be your successor. Don’t let them jump ship for another job without communicating that you are considering them as the eventual head of your medical practice.

RESPONSES/COMMENTS (PM EDITORIALS) |
RE: "Who Wants to Be a Podiatrist?"
From: Ron Freireich, DPM
I just read Dr. Block's Editorially Speaking "Who Wants to Be a Podiatrist". I was somewhat shocked but not surprised by the number of students in the applicant pool, just over 500 students applying for the 2022 school year. With nine podiatry schools in the country, that's around 55 students per school, if they are all accepted. This reminded me of survey results posted in the May 9, 2022 edition of PM News where only 13.27% of the 889 responding podiatrists would recommend podiatry as a career for their child.
Sabrina Minhas, DPM, Immediate Past President of the Pennsylvania Podiatric Medical Association (PPMA) suggests these action steps that an individual practitioner can take to address this problem: “Please reach out to your local college advisors and schedule either an in-person or Zoom event to promote awareness of our profession." I'm wondering why our national and state associations as well as the leaders at the nine podiatry colleges are not getting to the root cause of this major problem prior to taking any action. Maybe we should start by surveying the 87% of the podiatrists who would not recommend their child choose a career in podiatry.
