IL Podiatrist Discusses Plantar Warts

Podiatrist Leonard Kuzmicki of Spring Valley Foot Clinic emphasizes that plantar warts are no different from any other warts except in location. Because they are on the bottom of the foot, these warts grow inward instead of protruding from the skin. The virus gets in through some opening in the skin and begins growing. “Once they enter the skin, they set up these little cauliflower-like growths,” Kuzmicki said, “and all they do is they live in the outer layer — the epidermal layer — of the skin.”

Contrary to popular belief, Kuzmicki said, plantar warts do not put down roots. The black “pepper spots” often mistaken for roots are actually tiny scabs from capillaries that have broken inside the wart. That’s right — the growth has its own blood supply, courtesy of the patient.

Dr. Howard Liebeskind, team podiatrist to the US World Cup and National Teams, recently completed final podiatric preparations for the team's departure and participation in the 2010 FIFA World Cup South Africa  during the " 2010 Send off Series" held in East Hartford, CT and Philadelphia.

Dr. Liebeskind has been team podiatrist to the U.S. Men’s and Women’s National soccer teams for the past 18 years, serving the United States Soccer Federation through a combined eight FIFA World Cups. He currently sits on the Medical Advisory Board of the US Soccer Federation and is a member of the American Academy of Podiatric Sports Medicine.

ON THE LECTURE CIRCUIT

PA Podiatrist Lectures at Diabetic Symposium in Trinidad & Tobago

Dr. Lee J. Sanders joined a distinguished faculty at the First Diabetes Symposium sponsored by the Trinidad and Tobago Medical Association and Johns Hopkins International, on May 16th in Port of Spain, Trinidad.  His lecture was entitled Diabetic Foot Disease and Foot Care in the Rural Setting. The program was part of the Trinidad and Tobago Health Sciences Initiative (TTHSI); a collaboration between the Ministry of Health, the Ministry of Science, Technology and Education, the University of Trinidad and Tobago (UTT), and Johns Hopkins Medicine International (JHMI).

Dr. Sanders expressed his concern for the burden of diabetes and diabetic foot disease in the Caribbean black population. In Trinidad and Tobago, foot problems account for approximately 14% of hospital admissions and 29% of bed occupancy by people with diabetes. The habit of going barefoot is common. There is a very high rate of diabetes-related lower extremity amputations. Mortality rates after lower extremity amputations in Barbados are among the highest in the world. Sepsis and cardiac disease are the most common causes of death. Prevention strategies are urgently needed!

Before you sort of jump off the diving board, you want to make sure that there is water there to catch you.

RE: Painful Dermatitis in 17-Year Old (Jared Clifford, DPM)

It worries me when Dr. Clifford says he is going "to try a course of topical steroids."  Is it not incumbent for a podiatrist who doesn't really know what the diagnosis is to save the patient the time, money, and delay by sending him to a real specialist in skin diseases, i.e., a board certified dermatologist who has years and years and years of training and only treats skin diseases, not just part time like we do? In addition, twice in the report, viral etiologies were mentioned. Isn't it contraindicated to give steroids in a viral disease as it may further suppress the immune response and cause continuation or exacerbation? If you're going to insist on "trying" yourself, perhaps an anti-viral like "Amantadine" or "Zovirax" may be helpful. If antibiotics and antifungal failed, it would make sense, because a virus wouldn't respond to those treatments.

If your patient has been using the Neosporin for any length of time, have him just stop using it and watch for any changes in the skin. I have seen some major dermatitis reactions from Neosporin. The Neosporin is most likely not the cause, but I would still remove it from his current therapy. The problem, as I am sure you already know, with the steroid approach is you are treating the problem without focusing on the cause. Dr. Howard Lewis was my attending when I was a resident. He would make me buy him lunch if I did that during residency.

I see conditions that look like this often in my practice. My 16 y/o son also had it. I believe it to be a severe tinea infection with reactive/allergic skin changes. I have the patient soak in warm water with a high epsom salt concentration. Have the patient apply betamethasone diproprionate cream, along with an anti-fungal cream. Sometimes we have the patient add Neosporin cream.
 
This is done 3x per day, and the patient is seen back in one week. Usually, it is about 80% better, and if so, I have the patient continue for another week. I actually have never seen it fail.

Many PM News readers have performed tendon surgeries and applied casts in the OR or PACU. I would venture to estimate the number of procedures in this country to exceed 500,000 each year. Few, if any, injuries result. It seems repulsive to think that a member of this profession would allege a violation of the standard of care and present such weak arguments to support his case. Bravo, PM News for publishing the transcript and allowing us to see, in black and white, the utter lack of integrity and honesty Dr. Stabile exhibits for a paltry few thousand dollars; a sum he could perhaps earn practicing medicine honestly and responsibly. And bravo again, for exposing this trash to the greater podiatric community. Dr. Stabile deserves our collective scorn.
