Dramatic Reduction in Amputation/Death in Dialysis Patients Receiving Podiatric Care
Tze Woei Tan, USC Vascular Surgeon, and David G. Armstrong, Professor of Surgery and Director of the Southwestern Academic Limb Salvage Alliance at USC were first and second authors of a study evaluating the role of podiatry in people on dialysis. The study, the first of its kind, evaluated more than 50,000 patients.

Those that received podiatric care prior to developing a diabetic foot ulcer were approximately 18% less likely to die or have an amputation over four years of study follow-up. Dr. Tan was awarded this year's Marvin Levin Award for top oral abstract in the lower extremity diabetes session. Dr. Tan is a recipient of an NIH K23 Career Development award focusing on limb preservation in high-risk communities, on which Dr. Armstrong serves as a mentor.

Bill Enables 401(k) Match for Doctors Paying Off Student Loans
Under provisions in the “Securing a Strong Retirement Act” (H.R. 2954), graduates who cannot afford to contribute to their employer-sponsored retirement plan would no longer be forced to forego the important employer match for retirement contributions. The bill, passed by an overwhelming 414–5 margin in the House, has moved to the Senate.
The “Securing a Strong Retirement Act” permits retirement plans such as employer-sponsored 401(k) plans to make matching contributions to workers as if their student-loan payments were retirement-plan contributions. Qualified student-loan payments are broadly defined under the legislation as any indebtedness incurred by the employee solely to pay qualified higher education expenses of the employee.

4 Tips on Physician Practice Succession Planning
There are several options available if you are a private practice owner looking for advice on how to establish a succession plan. Here are four tips on getting started:
3. Prepare your successor (and yourself) for success
Once you have finished your private practice succession planning, chosen a successor and taken care of the legal arrangements, you will need to train your successor to successfully run your medical practice. I cannot stress enough the importance of this step and yet it so often is overlooked after the attorneys have drafted the legal documents and departed the practice. Even if your successor is passionate about your practice, you should set up a training plan that exposes them to every area of your medical practice so they can learn the critical management tasks that you may take for granted.
In addition to training your successor, you should also plan your own exit strategy. Give your successor room to learn and grow while you are still in a place to offer advice, but be prepared to start giving them more control as you approach retirement. This can prove to be difficult for passionate private practice owners, but it’s essential to gradually let go and allow your successor to take ownership when you’re ready to depart.

Query: Defining Flexor Tenotomy
The coding team and I have been going back and forth regarding the use of CPT 28010 and CPT 28011. My understanding of these codes is that CPT 28010 would be performed on one toe on a foot. CPT 28011 would be used in the event that I perform the procedure on multiple toes on the same foot. The coding team’s view is that CPT 28010 is used when I perform the procedure on the flexor longus tendon and not the flexor brevis tendon. But for the use of CPT 28011, both flexor tendons would need to be cut on a single toe. Therefore, CPT 28011 could have multiple units on one foot. CPT 28010 could also be appended with multiple units. Some clarification would be helpful.

It is not unusual for a physician or even a coder to interpret these two CPT codes as the following: CPT code 28010 should be used if a percutaneous tenotomy is performed on one toe on a foot, and CPT code 28011 should be used if a percutaneous tenotomy is performed on multiple toes on the same foot. Is this correct?
I think that the Medically Unlikely Edits or MUEs for CPT codes 28010 and 28011 need to be accessed in order to obtain clarity regarding the appropriate way to bill these two CPT codes. The Medically Unlikely Edits or MUEs for CPT code 28010 is 4. This supports the fact that this CPT code can be performed and billed for up to 4 times on one foot on a given date of service. The Medically Unlikely Edits or MUEs for CPT code 28011 is also 4. This supports the fact that this CPT code can be performed and billed for up to 4 times on one foot on a given date of service.
Therefore, CPT code 28010 can be billed when a percutaneous tenotomy is performed on one, single tendon on one toe. This procedure can be performed on up to four toes on the same foot on the same date of service. CPT code 28011 can be billed when percutaneous tenotomies are performed on multiple tendons on the same toe on the same date of service. CPT code 28011 can be performed and billed when this procedure is performed on up to four toes on the same foot on the same date of service.

Query: Papilloma
This patient entered my clinic recently with complaints of the right foot being enlarged and some pain. Her medical history is unremarkable related to the condition of her right foot. It has been going on for over 10 years with no doctor helping her.

I would like to answer Dr. Fellner’s questions from a senior practitioner’s perspective. First, I would say that most states, if not all, would find this advertisement as illegal and practicing podiatry/medicine without a license. I would state that any practitioner that sees this should check the state law for pedicurists and, if appropriate, file a complaint against the establishment and the pedicurist involved with the state licensing board for pedicurists.
Concerning salaries, Dr. Fellner has a choice of going into practice himself with no one but himself to determine his income. From what I can see, many of our newly-minted colleagues work for a hospital or orthopedic group, and I am sure their salaries (from what I am told) are quite generous. There are lots of private practice opportunities out there, and it may require a move to another state to find the high paying job. Like any profession, the better you are qualified and perform, the better your chances that your salary will compensate you well. The bottom line here is the efforts you put out in life generally equal the benefits you will receive.
