Most Heel Pain Does Not Require Surgery: TX Podiatrist

According to Grace Ouyang, DPM, a doctor of podiatry and podiatric surgery at New Ulm Medical Center, the most frequent complaint most podiatrists hear from their patients is heel pain. The pain can be under the heel or along the side or back of the heel and does not go away with rest and icing. The first step in dealing with heel pain is determining the cause.

Dr. Grace Ouyang

Once a cause has been identified, treatment can include resting from activities that cause or aggravate the pain, different shoes, orthotics, and stretching exercises such as massaging your arch, rolling your foot over a tennis ball or a frozen water bottle, or stretching your calf muscles. Sometimes, when these treatments fail, surgery may be needed, but most people recover from foot pain without surgery.

PODIATRISTS IN THE COMMUNITY - PART 1
IA Podiatrist Opens Midwest Foot and Ankle

Mark Hartman, DPM, of Midwest Foot and Ankle Center, has seen it all. “There was a weekend years ago where we saw six ankle fractures, someone who was gored by a bull, and a black powder burn,” said Hartman, who has been practicing podiatry in Fort Dodge since the 1990s. “It was just crazy. It was a collection of things that nobody would have thought would happen in a 72-hour period.”

Dr. Mark Hartman (photo: Chad Thompson)

Most days don’t involve things like black powder burns, but Hartman said there’s plenty of variety to his work. “Every day is different,” he said. “We literally can have a consult at the hospital in the ICU or go over to the birth center for a consult.” Dr. Hartman went into independent practice earlier this year, opening the Midwest Foot and Ankle Center office.

PRACTICE MANAGEMENT TIP OF THE DAY
10 Ways to Strengthen Your Strategic Plan - Part 3

A strategic plan is not just a document written by physicians and managers and then filed away for safekeeping. It is a vision for the practice, owned by the practice. For a medical practice to succeed, everyone must engage with and live and breathe the plan. Here are 10 questions to assess your strategic plan.

3. Are you focused on a clearly defined target? Push for the narrowest possible segmentation of your target market. You need to aim for the bull's eye to hit the target. Clearly defining and understanding your target market of patients and referring physicians is one of the most powerful things a practice can do to improve its strategy.

CODINGLINE CORNER
Query: EMR versus Written Medical Records

I am looking to start my own part-time practice with traditional “pen and paper” medical records instead of an EHR or EMR. I can’t see any downside with written paper medical records. EHRs are more expensive and written records would be more efficient in clinic. I am expecting to produce less than $90,000 in Medicare monies, so I don’t believe MACRA or MIPS would be a problem. My questions are: 1. From a billing, coding, or legal standpoint, would there be any problems to doing written medical records? 2. Other than storing and holding charts for 7 years, are there any other major laws or regulations that need to be followed? 3. If I do cross the $90k Medicare threshold and do nothing for MACRA/MIPS, what is the penalty?

Archived Codingline Question

Response: 1. From a billing, coding, or legal standpoint, would there be any problems to doing written medical records? No

2. Other than storing and holding charts for 7 years, are there any other major laws or regulations that need to be followed? No, but legibility is key

3. If I do cross the $90k Medicare threshold and do nothing for MACRA/MIPS, what is the penalty? PERFORMANCE YEAR MEDICARE PART B PAYMENT ADJUSTMENT YEAR MAXIMUM -% MIPS PENALTY:
2019 -7% for non compliance in 2017
2020 -9% for non compliance in 2018
2021 -9% for non compliance in 2019
2022 -9% for non compliance in 2020

Paicos served from 1942-1945 in the armed services achieving second lieutenant, Army Air Force as a Glider Pilot. He was stationed in the European Theater, participating in the Varsity Invasion March 25, 1945. Dr. Paicos returned from military service and completed additional studies at Temple University. He opened his practice in Boston, then added an evening/weekend practice in his home located in Stoneham.

Drs. Peter Paicos Jr. and Peter Paicos

He was well respected by his peers and was involved in local organizations, volunteering at many clinics and facilities for those of little means. Pete was known as a fun loving joker; there was always a smile on his face. He was a true gentle man with a warm and generous heart, always willing to extend a hand to those in need. In August of 2006, Dr. Paicos was recognized by the Massachusetts Podiatric Medical Society for sixty years of providing medical care to his patients. His son Peter Paicos Jr., DPM, outgoing president of MPMS, bestowed that honor upon him. Dr. Paicos will be laid to rest on his one hundredth birthday, June 11, 2019.

RE: The Passing of Alan K. Whitney, DPM

Dr. Whitney was truly a gentleman as a teacher and competent biomechanist. His wife Loretta, who worked the school switchboard during the week and on many weekends and holidays, was a lovely individual. In the words of Dr. Whitney, “I like it, go with it.”

QUERIES (CLINICAL)
Query: Best Way to Test for Onychomycosis

Should nail cultures be performed on all possible fungal nails, and if so, what type of culture? Is it best to use a lab or perform the test in-office?

RESPONSES/COMMENTS (NEWS STORIES)
From: Leonard A. Levy, DPM, MPH

Podiatric medicine needs to recognize that it has an extremely important role in preventing mortality by mitigating the risk of falling. Interventions that avoid falls that podiatric physicians should provide demonstrate the importance of podiatric public health and extend far beyond the prevention of disorders that directly affect the pedal extremities.

While such intervention includes advising patients to avoid high heel shoes, sandals, and slippers ("slippers make you slip"), other forms of public health education should be included in the podiatric physician's interaction with patients. Patients should be reminded to get rid of small scatter rugs that they have at home, remove clutter from the floor, and eliminate extension cords that stretch across the floor. In addition, recommending daily exercises that strengthen the legs also should be advised. Also, reduce the height of thresholds between rooms. While these interventions may not have the glamour of surgery and other medically complicated procedures, they can/will be life-saving.
