NH Podiatrist Returns from Medical Mission to Eduador
Kevin Murphy, DPM of Core Podiatry recently traveled to Ecuador on a mission trip with the group Hands Across the World. The group travels annually to the Orellana region of the country to perform free surgeries on patients who would not otherwise be able to afford the procedures.

Dr. Murphy was part of the team who screened patients, and he performed nine surgeries during the week. In between surgeries, he assisted other physicians wherever needed, from starting IVs to cleaning instruments. “I really enjoyed being a part of the team that provided care to people who truly need it,” said Dr. Murphy. “We were able to bring care to a part of the world where those services are just not available otherwise.”

Letting Toenails Grow Too Long Can be Problematic: IL Podiatrist
People want their toenails to look great in the summer. But many people allow their toenails to grow too long, which can cause discoloration, blisters, and other problems, according to Loyola University Medical Center podiatrist Dr. Coleen Napolitano.

If a toenail is allowed to grow too long, it can jam the end of the shoe, causing a microtrauma with each step, Napolitano said. Thousands of such microtraumas can discolor the toenail.

TX Podiatrist Comments on "Streak Running"
Hundreds of runners live by a simple principle: Run every day. Period. Some of these “streak runners” call themselves “streakers,” and to avoid the forbidden skipped day, they’ve persevered through flu, whooping cough, and even the eye of a hurricane. The United States Running Streak Association defines a streak as “at least one continuous mile (1.61 kilometers) within each calendar day under one’s own body power (without the utilization of any type of health or mechanical aid other than prosthetic devices).”

Dr. Jeffrey Ross, a sports medicine podiatrist in Houston, doesn’t think streak running is a good idea, but he still says we can’t automatically say it’s bad or good for everyone and it depends on the individual. Research is needed to reach more definite conclusions, he says.

Surgery by TX Podiatrist is 2,000th Performed at Victory Medical Center
Victory Medical Center Beaumont performed its 2,000th surgery this week. The 2,000th surgery was performed by Dr. Mark Bruyn, a leading podiatrist in the area. In addition to podiatry, Victory Medical Center Beaumont specializes in complex spine, neurologic, orthopedic, gynecologic, cardiothoracic, and ENT surgical cases as well as general surgery and pain management.

Query: Billing Strategy for Arthroeresis
I do the vast majority of surgical cases out of a single specialty surgery center and a multispecialty outpatient center. I usually go to larger hospital only for cases involving ASA 3 patients. Going to an outpatient hospital OR is a hassle for my office, my patients, and me. The final bill for the facility is much greater at the hospital. The problem that I have is that the use of an arthroeresis device for STJ/MTJ stabilization is much less invasive than calcaneal osteotomies. I like to have the option of utilizing that device on certain feet. I typically make the decision intra-operatively.
At my facility, the administration has asked me to take those cases somewhere else. That really only leaves me with the hospital. It really kills me that I am financially forced to take patients to a large hospital and spend tons more money on their care with all the hassles involved; not to mention risks of an inpatient hospital setting (MRSA, etc.). I do not have a problem going there for patients who need it medically, but to make a move based on financial reimbursement somehow sits wrong with me. Does anyone know a billing strategy that will allow facilities to be paid for an arthroeresis device?

Query: Partial Rupture of Ligament
I am trying to bill a pneumatic walking boot given to a patient for a partial rupture of the plantar fascia. Does anyone know what the ICD-9 code is for the partial rupture of the plantar fascia? I can find a rupture of a tendon, but not a rupture of the ligament.

Response: I recommend ICD-9 845.19 (sprains and strains of ankle and foot, other). There is no specific code that describes a partial or complete rupture of the plantar fascia.

I agree with Dr. Benjamin’s sentiments regarding the California podiatrist. I would like to comment on his statement regarding the podiatrist Congressman from Ohio. That Congressman is a neophyte in the obfuscate terrain of Washington, DC. He is yet learning the political “lay of the land” on Capital Hill. To expect him to move political mountains at this point is unrealistic. This Congressman is on two key committees, and given time, he will prove to be a valuable asset to the profession of podiatry. If an incriminating finger is to be pointed, let it point to the APMA Legislative Committee which has been less than stellar in addressing a number of important areas of legislation for our profession.

I would like to respond to Dr. Benjamin's position statement if it is really necessary for a podiatrist to be in representative government in Washington, D.C.
Dr. Lee Rogers may not have been my political "cup of tea" as compared to Dr. Brad Westrup; however, the presence of any podiatric physician, nurse, or medical professional is essential in order to help educate and inform other legislative colleagues of the challenges to our patients and medical practices on a daily basis. We all have seen how legislative bills are rarely read these days. Their input is essential on all levels of government (local, state, and federal) to help structure and guide changes in medical care that will affect how we all deliver care to our patients in the future.
I guess that if there were more doctors, nurses, and other medical specialties, we would not see the disaster that seems to befall federal and state management of healthcare exchanges, improper administration of the VA toward veteran healthcare, delays on healthcare initiatives and reimbursement rates yearly, lack of fixes on SGR (sustainable growth rate) within the Medicare program, lack of healthcare tort reform, etc. The founders of this country were made up from all walks of life. However, if you feel that the 90%+ legislators who are attorneys are providing benefit to your patients and the nation overall, continue to vote for them. As the saying goes, "people deserve the government they vote for!"

This happened once before under a GOP governor back in the '90s for 6 years in Illinois and then just recently in 2012 under the Illinois SMART Act where 64 health agencies or medical professions including dentistry, optometry, psychiatry, and podiatry were cut or had services reduced in the Medicaid program, all in the name of saving money. Of course, we knew differently. The Illinois legislature looked to eliminate podiatry completely, except for children 19 and under, but an IPMA delegation including our esteemed executive director, Michael Hriljac, DPM, JD, testified in front of the Illinois legislature back in 2012, and fought to have any diabetic patient on Medicaid to be seen by a podiatrist. Recently, we lobbied the Speaker of the House and got the Illinois House and Senate to reverse their decision. So, where was the Illinois State Society? Right on top of the issue every moment and step of the way!

The Illinois Podiatric Medical Association testified in the Illinois capital, when the cut was first proposed. Originally, it was proposd to cut all podiatric services. That was changed to allow podiatrists to treat Medicaid recipients with diabetes, and children under 21. To make things worse, back in '92 or '94, the state did the same thing and it cost them far more. You'd think the elected officials in Illinois would have studied a little history.
