GA Podiatrist Provides Tips on Preventing Fungal Infections
Sarepta Isaac, DPM of Serenity Podiatric Concierge says if you want to keep your feet healthy and pain-free this summer, choose your flip-flops or flats carefully. "You want one with a sturdy sole, a nice arch, a sturdy heel," Isaac says. Dr. Isaac says summer is primetime for fungal infections. Isaac says sharing shoes, even with someone you know, could leave you with itchy, irritated feet. "You may not know your best friend has a fungal infection," she says. "Their feet look great, but you get a fungal infection."

It's the same thing with running around barefoot in wet places like pools and locker rooms. "Do not walk barefoot," Isaac says. "Because, often, we're tempted to walk from one side of the pool to the other, or jump into that gym shower. That's an intense breeding ground for fungus as well."

TX Podiatrist Warns of Nail Salon Dangers
Your beautiful mani-pedi could give you an ugly infection - or worse. Look around the salon to see how clean it is. "The true test is the bathroom. If it isn't spotless, you shouldn't trust that the stations are," Gabriel Maislos, DPM, a podiatrist affiliated with Memorial Hermann Southwest Hospital, said. Aestheticians never should cut cuticles, which serve as skin barriers to protect your nails’ growth. Nor should they remove calluses with a metal cheese grater-like device.

Whirlpools and tools can spread blood-borne diseases–such as HIV and hepatitis–via microscopic nicks and re-used instruments. Even invisible nicks expose you to the possibility of infection, which also is why you shouldn’t schedule a pedicure within two days of shaving your legs. And forget about trendy pedicures that have flesh-nibbling fish to remove dead skin. “The fish use their teeth, which could create cuts – and who knows what germs they’re carrying,” Dr. Maislos says.

PA Podiatrist will pay $75,000 and Stop Prescribing Schedule II Opioids for 2 Years
A Langhorne podiatrist will pay $75,000 to resolve allegations that he improperly prescribed opioids to a patient from 2014 through 2016. Dr. Ronald Klein will also stop prescribing Schedule II opioids, which present a high risk of abuse, for two years, according to the terms of a settlement announced by U.S. Attorney William M. McSwain on Monday.

Deadly Falls by Seniors Soar
New research shows fatal falls have nearly tripled in older Americans in recent years, rising to more than 25,000 deaths yearly. The new analysis from two studies published in the Journal of the American Medical Association included 16 years of U.S. vital statistics data on adults age 75 and over.
Fatal falls increased from 8,600 in 2000 to 25,190 in 2016. Separate CDC data show they climbed even higher in 2017, to almost 26,440 fatal falls in Americans aged 75 and older.

RE: Myrmecia Wart
This photo is of a lesion on a patient who presented to my office for treatment of what he thought was a verruca on his toe that had been present for a few months. The somewhat atypical appearance, increased bleeding upon debridement. Bryan Markinson, DPM's recent article in JAPMA The Misdiagnosis of Acral Lentiginous Melanoma prompted me to send a shaving for biopsy and not treating the apparent verruca.

The dermatopathologist diagnosed a "Myrmecia wart with focal squamous atypia, transected". The findings were "a myrmecia inclusion body wart; however, one fragment showed enhanced cellular atypia suggesting an ensuing significant squamous dysplasia, potentially arising in a myrmecia wart. There are rare reported cases of Bowen's disease resembling and possibly arising in myrmecia warts." Bowen's disease is the earliest form of squamous cell skin cancer. The patient is currently being further evaluated by a dermatologist.

Dr. Whitney was my instructor at PCPM in 1972. He was a true gentleman and very professional in both actions and appearance. His lovely wife, Loretta, also worked at PCPM. In addition to being a very good instructor, he was a very gifted artist The profession has lost a truly great man.

My best of times with Dr. Whitney was taking a few trips with him and my best friend, Dr. Steven Gubernick, to the casinos after clinic during my last two years at PCPM. He loved the slot machines. He never lost or won much, just wanted to have a good time. He was like a grandfather to us. I loved the conversations we would have in my car on the way to Atlantic City. He was very funny and always had us laughing. My buddy and I called him "Big Al". There will never be a replacement for him. RIP Big Al!

RE: The Passing of Alan K. Whitney, DPM
Dr. Whitney was fun in class, countering Dr. Schoenhaus' statement that the rearfoot controlled the forefoot. His drawings were legendary. In clinic, my responsibility one day was to dispense a pair of orthotics that another student had ordered. The patient was limping from a mishap playing basketball the previous day. I placed the orthotics in his athletic shoes and asked him to walk the hallway to see if the orthotics created any pain.
Dr. Whitney observed this fellow limping. When I "presented the case" to Dr. Whitney, he asked me in his thick New England accent, "Why is that guy limping?" After explaining about the basketball injury, he replied: "Then, why the hell are you asking him to walk?" That was my next lesson...avoid tunnel vision!
I enjoyed having AKW as a clinical and classroom instructor. After 37 years of practice, one of his books still resides on a shelf in my office. Rest in Peace, Dr. Whitney. My condolences to your family.

Regarding the comment by Dr. Borreggine, I disagree. The definition for CPT code 28122 makes no mention of plurality. If more than one tarsal or metatarsal bone requires an exostectomy (e.g., 1st metatarsal base and cuboid), then multiple units could be billed. However, if each of these osteophytes were accessed through the same incision (e.g., 1st met base, medial cuneiform), then only one unit would be appropriate.
