FPMA County Component Holds First Annual Walk-a-thon
The Miami-Dade County component of the FPMA had its first annual walk-a-thon on Saturday, June 2 at Zoo Miami to benefit the Dr. Clifford O'Connor, DPM Foundation. The motto of the event was Healthy Feet to Step Towards the Future" as a means to educate the community on what we do as podiatrists.

Approximately 400 were in attendance, including State Representative Robert Asencio from District 118. Vendors raised $12,000, which will benefit Barry University students via scholarships.

AK Podiatrist Running for House Seat
There is a primary clash between a trio of Republicans: Chugiak’s Craig Christenson, former Eagle River representative Nancy Dahlstrom, and Eagle River attorney Bill Cook. Christenson is a retired podiatrist, while Dahlstrom served in the State House until 2010 before resigning to take a job in the Gov. Parnell administration.

Dr. Christenson is a retired USAF Colonel with significant leadership/management skills. He commanded three medical squadrons at both the clinic and hospital levels. Following a 23-year U.S. Army and Air Force career, he was appointed as Deputy Commissioner, Alaska Department of Health and Social Services.

UK Podiatrists Promote Foot Health Month
The College of Podiatry is urging people to discover how simple it is to keep their feet in peak condition this Foot Health Month, which takes place throughout June 2018. “Having healthy feet enables us to keep fit and well and that’s why we all need to look after them,” says Emma McConnachie, a podiatrist from the College of Podiatry.

“Without healthy, pain-free feet, we can’t walk, do sports, or even relax properly. Imagine feeling as if you are walking on sharp stones each time you took a step. That is how some foot conditions can feel. “A basic knowledge of foot health care is essential for everyone and that’s what this year’s Foot Health Month is all about.”

IL and WI Podiatrists Comment on Summer Footwear
Flip-flop season has finally arrived, and it’s terrible for your feet. “Flip-flops are my least favorite type of shoes worn in the summer,” said Sarah Dickey, DPM, president-elect of the Illinois Podiatric Medical Association and a podiatrist at Loyola University Health System. “These provide zero structural support or protection for your feet.” And flip-flops aren’t the only culprits. Podiatrists say many of the season’s most popular shoes are some of the worst options for maintaining healthy feet by preventing injuries and avoiding heel, leg, and back pain.

“This time of year, particularly in the Midwest, brings in new patient complaints,” said Dr. Alex Kor, a podiatrist in Milwaukee. “Typically, the average consumer is not very well-versed on what’s a good shoe … and if you throw in a mix of activities, it makes things worse.” Kor, a spokesman for the American Podiatric Medical Association, and Dickey say customers should avoid flexible-soled shoes that allow too much bending in the midsection, near your arch.

Query: What does "EACH" mean for CPT 27686?
CPT Assistant 2009 says the term “each” in CPT 27686 refers to each tendon. I interpret that to mean that each tendon lengthened in that incision would be coded. For example: through a single incision, I lengthen gastroc, soleus, and plantaris tendons — I would CPT 27686 three times. (Or it could be TP, FDL, and FHL through same incision). I have heard other people say you just code it once, no matter how many tendons were lengthened. That doesn’t seem consistent with the wording in the CPT description, which says “multiple tendons (through same incision), each”.
If each tendon is coded separately, does each CPT 27686 get a -59 modifier? Also, if multiple tendons lengthened through the same incision on both legs, should the -50 modifier be applied? If so, where? CPT 27686 three times total (if 3 tendons lengthened each leg), each with 50 modifier? CPT 27686 three times each side with Rt and Lt modifiers? If 3 tendons on right and 2 tendons on left, just add 50 modifier to two of them? Please clarify this.

Response: Here’s the simple answer:
CPT 27685 – single tendon
CPT 27686 – multiple tendon, same incision
So if you lengthen one tendon use CPT 27685 and multiple tendons you would use CPT 27686. Therefore, the case you described you would just use CPT 27686 one time.

Query: Local Anesthetic Shortage
The staff member who orders my supplies has informed me that our usual venues for lidocaine and bupivacaine are claiming they are unavailable, possibly until the next calendar year. Does anyone have a reliable source for these injectables, or do I operate under ethyl chloride anesthesia?

RE: Florida’s New Law on Controlled Substance Prescribing

H B 21, signed into law by Gov. Rick Scott on March 19, 2018, imposes a number of legal requirements on healthcare practitioners who prescribe controlled substances. A prescriber must consult "the database" to review a patient's controlled substance dispensing history before prescribing or dispensing a controlled substance. This requirement applies to all controlled substances, not just opioids. There is now a substance prescribing continued education requirement for each physician to be completed by Jan 31, 2019, and then prior to each subsequent licensure renewal. Details of this new law are outlined in this article:

We, as podiatrists, are all worried about the dwindling number of pre-med applicants applying to podiatry school. Could part of the problem have anything at all to do with the 50% passing rate for ABPS certification? Orthopods don't have that problem. Or is it possibly the chance that there might not be a position available for residency? The low passing rate is much easier to fix than the residency crisis, which is less of a crisis now. Another impediment to getting talent to apply to podiatry school could easily be addressed by increasing the pass rate so that it is no longer such an embarrassment.

I recently interviewed associates with three years of residency training. The spectrum of graduates training was from excellent to below average. One may ask how a graduate of three years of surgical training could be average or below average. The answer is that many programs just do not have the surgical load or variety that is necessary to come out as a fully trained “surgeon”. Many whom I interviewed felt they needed an additional year as a fellow to feel confident. Sadly, three years of training may NOT adequately prepare graduates for practice and or sitting for the boards. From what I saw from the current crop of residents is that many were under-trained and not ready to become full scope podiatric “surgeons”.
