PODIATRISTS IN THE NEWS - PART 1
NJ Podiatrist Discusses Gout

Gout is a painful form of inflammatory arthritis that affects over eight million Americans according to the national Gout & Uric Acid Education Society. Gout represents a build-up of uric acid in the joints. "It's very similar to the process of making rock candy, where you boil water and add sugar until you can't add anymore," Podiatrist Dr. John Brandeisky explained. "When you super-saturate a solution, it crystallizes as the temperature cools. In the case of gout, conditions in the blood serum lead the joint-lubricating fluid to become high in uric acid.

Dr. John Brandeisky (photo: Russ DeSantis)

"Because the joint is located farther away from the body's warmer core temperature and is therefore cooler, uric acid crystals form in the joint and are pointy like knives — so pointy that when white blood cells are repeatedly sent to attack these 'foreign' substances, the crystals actually impale and kill the white blood cells," he said. "Gout is the great mimicker of a raging infection around the joint, though it won't respond to antibiotics or get better," he said.

PODIATRISTS IN THE NEWS - PART 2
Proper Standing Posture Prevents Painful Problems: NY Podiatrist

Poor posture while standing can introduce or exacerbate a whole other set of potentially pain-causing issues. “Our bodies tend to go to the pattern of less stress,” says Emily Splichal, DPM, a Manhattan-based podiatrist and movement specialist who is the author of Barefoot Strong. In other words, you’ll assume the position that is easiest, and sometimes that results in poor posture.
Dr. Emily Splichal

Luckily, there are easy ways to make sure that your standing isn’t just sitting 2.0. First, you want to start with the feet. “They are the foundation to our posture,” says Dr. Splichal, which means women will want want to ditch the heels while standing desk-side. A two- to three-inch heel will tighten the fascia—the connective tissue web that envelops the bones, muscles, and tendons—and will put stress on your joints, she says.

PODIATRIC GRADUATES IN THE NEWS
Barry Graduate Delivers Keynote Speech at Youth Academy Ceremony

Dr. Michael Johnson delivered on a promise when he graduated in late April with a degree in podiatry from the prestigious Barry University in Miami. Johnson, an alumnus of the Reichert House Youth Academy in southeast Gainesville, was the keynote speaker and one of three inductees into the Richard Baxter Hall of Fame at the 2015 Reichert House Youth Academy Baccalaureate Ceremony held last Wednesday at Mount Carmel Baptist Church.

Michael Johnson

During his speech, Johnson also emphasized the importance of setting and reaching goals. He ended his speech by saying that young black men are not as an endangered species as some proclaim them to be. "I want them to realize that black men are more than capable of accomplishing and doing great things," said Johnson, when asked what he wanted the graduates and younger students to take away from his speech. In July, Johnson will begin a three-year residency at the Charlie Norwood Veterans Administration Medical Center in Augusta, Georgia.

PODIATRISTS AND PEDORTHISTS
PA Podiatrists Use Pedorthist for Better Patient Outcomes

When Carlisle podiatrists Drs. Mark Pinker, Mark Golec, and Ashley Herczeg, with Pinker & Associates, determined it would better serve their patients to have licensed pedorthist/orthotic-fitter, Randy Stevens, provide his services at their office, they didn’t hesitate.  So every Wednesday, Randy, of Randy Stevens Family Footcare in Harrisburg, PA, makes a ‘practice-call,’ and provides his services to the appropriate patients. “We have always utilized Randy’s services/skills, but recently I believe we all came to the realization that it would be much more convenient for our patients to see him in a familiar setting,” says Dr. Golec.

Randy Stevens, licensed pedorthist/orthotic-fitter, middle, is fitting Dr. Mark Golec’s patient, seated left, with custom-molded diabetic shoes and custom-fabricated diabetic inserts to accommodate the bilateral valgus foot deformities, as Dr. Golec, right, overlooks.

Both parties feel fortunate to have worked out this setup. “A good working relationship with a licensed pedorthist/orthotic-fitter is invaluable,” says Dr. Golec. Randy echoes Dr. Golec’s sentiment and adds that patients get to witness the team approach first-hand for the greater good of their foot and ankle care. At the end of the day, it’s all about the foot/ankle health of patients, and that is how this whole thing started.

INTERNATIONAL PODIATRISTS IN THE NEWS
One in Five People Walk Less Than 15 Minutes a Day: UK Podiatry Study

A fifth of us are now so lazy that we walk for less than 15 minutes each day, a new study has revealed. And the reason? We're all hooked on driving. Research carried out by the College of Podiatry found that despite being well aware of how little we exercise - and even acknowledging the fact that we each should walk more - the temptation to jump behind the wheel rather than go by foot is almost always just too much to bare. The survey found that nearly half of Brits choose to drive to work, while almost 40 percent go by car to visit friends and family who live just a stone's throw away.

Emma Supple

Podiatrist Emma Supple, from the college, said, "It is a shame to see that so few of us are failing to use our own two feet to get about. One in five parents drive their children to school despite more than a quarter saying their child's school is less than a mile away. Walking for just 30 minutes a day can bring so many health benefits; it is a great way to improve fitness gently and can be a suitable form of exercise for many people. The findings also suggest that we don't seem to be setting a good example for children, showing that walking can be a great way to travel."

MEDICARE NEWS
Medicare Vies to Keep ACOs on Board With More Flexible Rules

More flexibility is coming for Medicare accountable care organizations (ACOs) under a final rule the CMS published Thursday. The revisions are intended to strike a balance between maintaining the program's rigor and making sure providers continue to participate.

The Medicare Shared Savings Program will offer a new track to take on more financial risk of patient care, and it will allow Medicare ACOs to avoid penalties beyond the initial three-year term. The CMS will also issue future guidance on benchmarking and rebasing issues that have been sources of contention for many providers.

CODINGLINE CORNER
Query: Code for a 2nd MPJ Arthrodesis?

Is there a CPT code for a 2nd metatarsal-phalangeal joint arthrodesis? I see that CPT 28750 is only for the 1st metatarsal-phalangeal joint.

Hyim Baronofsky, DPM, Park City, IL

Response: The only code available for this is an unlisted CPT code: 28899. With the claim, send an explanation of what you did and the op report. It is also suggested that you might describe an alternative procedure (with assigned RVU value) which you feel might be equivalent in value.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS)
From: Kathleen Neuhoff, DPM

I am 62 and many would consider me a workaholic, but I would NOT have scheduled a surgery that would have kept me from my grandchild's graduation. If it had been an emergency, I would have had a colleague cover for me or done it after the graduation.
