PODIATRISTS AND SPORTS MEDICINE
Zumba Can Cause Compartment Syndrome: CT Podiatrist

There are some 12 million people worldwide who are taking up this cardiovascular dance called Zumba. However popular this new dance exercise is, beware of injuries that are caused by the high impact as one moves in a flying position. "One may not notice when they have "zumba'd" their own bodies into unnatural and inadvisable positions, often landing in an imbalanced position. Many are being treated for severe heel pain and, in some cases, a potentially more serious condition of "compartment syndrome" from the high-impact," says podiatrist Dr. Robert Weiss.

Dr. Robert Weiss

According to Dr. Weiss, "Compartment syndrome can be a serious condition that involves increased pressure in a muscle grouping. This can lead to muscle and nerve damage, or a problem with blood flow, which is a medical emergency. The symptoms are decreased sensation, severe pain that gets worse, and weakness. In other cases, there can be ankle strains due to lateral movements or, even worse, fractured bones. It's most important to know your limits. Pre-warm-up, even exercise with stretching and strengthening, is of value, but do not get carried away while in motion and Zumba with care and caution!"

PODIATRISTS AND SPORTS
WI Podiatrist Puts Together (Mostly) Podiatric Ultra-Marathon Team

When running a marathon, Dr. Tyler Wishau usually juggles. It’s a skill he has found to attract sponsors for his races. This time, the Carthage graduate is juggling schedules. “There are a lot of logistics, from picking up my teammates from the airport to practicing and shopping for our gear,” said Wishau, a podiatrist, who has organized a team of six to run in the Ragnar Relay Series from Madison to Chicago Friday and Saturday. The team consists of four podiatrists and two Ph.D. candidates. In all, they will run about 200 miles.

(L-R) Drs. Tyler Wishau and Sean Betesh

The other teammates are Jacob Mahaffey, Carthage graduate and first-year PhD. candidate at the University of Wisconsin-Madison; Dr. Chris Bourke, a podiatry resident at Penn Presbyterian Medical Center; Dr. Sean Betesh, a podiatry resident at Yale University; and Dr. Jeffery Weber, a podiatry resident at Detroit Medical Center.

PODIATRISTS IN THE COMMUNITY
IL Podiatrist Would Not Discourage Anyone from Pursuing a Medical Career

Although born in Chicago, Dr. John Sigle was raised in Springield. He loves the people more than anything. “We are a small community that takes care of each other,” he said. “Just look at the way we pulled together when the tornado hit our city. This is a special quality that most cities do not possess. This is a city that cares.”

Dr. John Sigle

Sigle would never discourage anyone from pursuing a career in the medical field. “There seems to be a lot of negative press about entering medicine,” he said. “Despite the regulations that are underway, medicine is not terrible, and physicians do not hate their lives. In fact, the career is fulilling and often a source of joy.”

SUNSHINE ACTS NEWS
Sunshine Act Will Soon Provide a ‘Rich Database’ for Compliance

Pharmaceutical and medical device manufacturer payments to physicians and teaching hospitals are on the verge of being publicly available under the Physician Payments Sunshine Act, which could change the dynamics of fraud enforcement, the management of conflicts of interest, and the public perception of physicians and hospitals. The CMS database with the payment information — including ownership and investment interests — will be open for business on Sept. 30, 2014.

“This can be a rich database for compliance people,” said Bret Bissey, who is senior vice president of compliance services at MediTract. “It’s a huge opportunity to bolster conflict-of-interest policies and procedures and to educate physicians who haven’t been totally transparent about disclosures that will be on the Sunshine database.”

CLINICAL CASE OF THE DAY
RE: Fungal Nail? Think Again.

This is an interesting case that recently presented to my office. This elderly woman presented with a 4-year history of a "fungal nail." If you look closely at the nail bed, you can see pigment extending into the eponychium. This is Hutchinson's sign and is pathognomonic for melanoma until proven otherwise.

Melanoma in a Nail Bed

At the initial visit, I biopsied the matrix and nail bed which demonstrated melanoma, Clark's level II. I won't go through the subsequent management here, but I wanted to show the case as it might have interest on this forum. Also, it serves as a wake-up call for all of us.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Visionary Medical Systems
From: Richard Rettig, DPM

If you are a user of Visionary Medical Systems, by now you received the certified letter that they are sunsetting their practice management system. They are offering replacements, but can only preserve limited data. I did a lot of research and was 99% complete in switching over to a different company (which was difficult to work with). Then I discovered that the original  writer of the Visionary code has produced a new cloud-based modern PM system with an improved scheduler, eligiblity checking, and is ICD-10 ready. See it at ncpractice.com.

The most important news is they can transfer over all your demographics and templates and A/R and referring doctors - virtually EVERYTHING. Using it is very similar to Visionary, so there is very little you have to learn. I reversed horses mid-stream, and am so happy I did. The owner was even able to re-write the automatic transfer of data to Medinotes (Yes, I still use that too!). They have been so very accommodating at every level; I could not recommend them any higher.

RESPONSES/COMMENTS (NEWS STORIES) - PART 1
From: Richard Benjamin, DPM

Can we finally put this issue to bed. The people of California District 25 have spoken for the second time. It is time for Dr. Rogers to go back to what he does best, and that is podiatry. Has any benefit come from having a podiatrist from Ohio in Congress? Please correct my ignorance if I am missing something.

RESPONSES/COMMENTS (NEWS STORIES) - PART 2
From: Jeff Kittay, DPM

What? Did I really just read another article about some state eliminating podiatry from Medicaid as a "cost cutting" measure? They tried that in NY in 1977 and found that foot care costs tripled or quadrupled, since foot problems didn't stop, Medicaid patients just went to more expensive ERs for their care. They tried it here in MA in the late 1980's, with the same result. Where was the IL state society when this was being discussed in the legislature? Does no one read history?
