FL Podiatrist Discusses Podophobia (Fear of Feet)
Every couple months, Julie Schottenstein, DPM, a podiatrist who runs the Schottenstein Center in Miami and Hallandale, FL, encounters a patient with podophobia who’s reluctantly seeking treatment for a foot concern they tried to ignore. “There’s a lot of tears,” Schottenstein says. “People are panicked, with a lot of fear.” Some turn white in the face and are so distressed they need to lay down; they don’t want their feet touched, so they jump at the doctor’s exam.
|
Dr. Julie Schottenstein |
Dr. Schottenstein recalls one person who tried to bolt out of the exam room and had to be coaxed back inside. Another put their shoes back on and said, “I don’t think I can do this.” “It’s like a trauma has catapulted them into this thing,” Schottenstein says. Schottenstein makes it a point to proceed slowly and carefully, explaining everything she’s about to do. “We talk it out,” she says. “Then I go, ‘Okay, now I’m going to do what we talked about. Remember, I'm doing this and doing that.’” She encourages patients to look out the window at the pretty waterfront view, and to do breathing exercises.

PODIATRISTS AND SPORTS MEDICINE |
GA Podiatrist Avoids Cotton Socks for Marathons
For most people sweaty feet are simply the result of wearing the wrong shoes or socks—or spending hours outside on a hot day being active. The problem is worth addressing because fungus likes sweaty feet, opening the door to infections like athlete’s foot, which usually begins between the toes. “If you put a cotton sock on a foot, you stick it in a shoe, and then you get that dark, damp, moist environment, where it stays wet all the time, fungus grows,” says Jay Spector, DPM, a podiatrist at Atlanta Sports Podiatry in Johns Creek, GA. “It’s just like if you have a leak in your bath you’re not aware of, and you have sitting water for a while.”
|
Dr. Jay Spector |
Socks play an important role in the sweaty-feet equation. Dr. Spector is a marathon runner, and he has learned that the key to cutting back on sweat-drenched feet is wearing acrylic socks, which wick out moisture. Merino wool works similarly—but he avoids cotton. With lightweight wool or sweat-wicking socks, “I’ll do a 20-mile run, and I take my socks off, and it’s as if I just put them on,” he says. “If I were to wear cotton socks, they’d weigh 10 pounds.”

PRACTICE MANAGEMENT TIP OF THE DAY |
5 Issues Physicians Should Consider Prior to Selling Their Practice
Recent legislative and rulemaking activity at both the federal and state levels, along with changes in certain deal dynamics, are sure to impact physician practice transactions going forward. Following is a list of five new issues physician practice owners should consider in advance of a transaction. Many of these items should be addressed in advance of going to market or accepting indications of interest from potential buyers.
3. Is your CTA compliance house in order? The Corporate Transparency Act (PDF) (CTA) requires that Beneficial Owner Information (BOI) Reports be filed by January 1, 2025 for reporting companies in existence as of December 31, 2023; for entities formed after December 31, 2023 and before January 1, 2025, BOI Reports should be filed within 90 days of formation. A physician practice most likely will be exempt by qualifying as a large operating company. This exemption requires that the entity have more than $5 million in annual revenue as shown on its most recent tax return and more than 20 full-time employees who work 30 hours a week. Leased employees do not qualify. Ensuring that you have analyzed the applicability of the CTA reporting requirements and your organization’s compliance is paramount as buyers will require new representations to that effect.

From: Alan Sherman, DPM
And as far as APMA Policy Proposition 2-24, amen to that. May we again have the foresight and strength to standardize podiatry to the model that general medicine has established, so that we may fully integrate into the greater healthcare system and fulfill our destiny as a medical specialty.
Alan Sherman, DPM, Boca Raton, FL
