APMA STATE COMPONENT NEWS - PART 2
NY Podiatrists Honored for Efforts at Free Medical Clinic

Friends of Night People’s (FONP) Free Medical Clinic is an incredible act of service that Western Division/Buffalo area members are involved in. Since 1994, this clinic has served countless individuals who are homeless, destitute, and less fortunate. During this time, the following members from the New York State Podiatric Medical Association (NYSPMA) have committed to regular service and made it a personal mission to care for these individuals’ feet.

Back row (L-R): Drs. David Perelstein, Ted Fitzpatrick. Front row (L-R): Drs. Jeffrey Carrell, Kenneth Goldstein, David Davidson, James Hanna, and Roy DeFrancis.

In many cases, these podiatrists have saved not only patients' feet and legs, but also patients’ lives: Drs. Ted Fitzpatrick, Ken Goldstein, James Hanna, Joe Sciandra, Jeffrey Carrel, David Davidson, David Perelstein, Roy DeFrancis, and Bob Rudewicz. They were recognized at the 8th Annual Party for the People at Buffalo Iron Works.

PODIATRISTS IN THE COMMUNITY
EHI Named One of the Fastest Growing Companies in Atlanta

Atlanta Business Chronicle has released its annual ranking of Atlanta’s Fastest-Growing Private Companies (Pacesetters). To qualify, a company must be privately held; based in the 20-county metro Atlanta area and not a subsidiary of another company; has experienced a two-year growth in sales of more than 50%; and 2017 revenues between $1 million and $300 million. Extremity Healthcare, Inc (EHI) had 56 million dollars in revenues for 2017, has 330 plus employees, and over 50% growth.

Dr. David Helfman

CEO David Helfman, DPM accepted the Pacesetter award on behalf of EHI. Village Podiatry Centers (VPC), a division of EHI, is the largest foot and ankle surgical practice in the Southeast and one of the largest podiatry practices in the country. VPC has over 40 doctors in more than 30 offices throughout the metro-Atlanta area, Middle Georgia, and Tennessee.

INTERNATIONAL PODIATRISTS AND FOOWEAR
Aussie Podiatrist Discusses Most Common Footwear Issues

“The two main issues with people’s footwear are poor fit and heel elevation,” says Dr. Hylton Menz, a podiatrist and professor of biomechanics at La Trobe University in Australia. Some of Menz’s research on older adults shows that too-short or too-narrow shoes can lead to foot deformities and increased risk for weakness and falls, as well as growths like bunions, corns, and calluses.

Dr. Hylton Menz

The healthiest footwear for both older and younger adults should fit well and have a low, broad heel, a thin and flexible sole, and some kind of lace, strap, or Velcro to ensure the shoe stays firmly attached to the foot, he says. Sandals and flats often fit this description, though many don’t. And if you’re wondering about the benefits of orthotics, the research is mixed.

MEETING NEWS
AIMFAS Holds Bi-Annual Lecture/Cadaver Seminar

The Academy of Minimal Invasive Foot and Ankle Surgery (AIMFAS) held its bi-annual 3-day lecture/cadaver seminar in conjunction with L.S.U. Medical School this past weekend in New Orleans. The conference attendees and lecturers included podiatrists and orthopedic surgeons from as far away as Australia.

(L-R) Drs. Burton Katzen and El Ali Kohen

The seminar featured two days of lectures and a cadaver lab day at the medical school. Among the featured speakers were Academy President Burton J. Katzen, DPM, who lectured on the Katzen Wilson Saw Bone Presentation and Moroccan orthopedic surgeon El Ali Kohen, MD who lectured on minimal invasive surgical techniques for the treatment of heel spurs and Haglund's deformity.

QUERIES (MEDICAL/LEGAL)
Query: Switching Practice, Manners, and Ethics

I’m planning to leave a group practice to join another group in the same city. What are my obligations or good practice to notify my boss and my patients? Is it bad manners to tell my current patients where I am going? What is the proper ethics and manners in this situation?

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Joseph Borreggine, DPM

I do not know if this individual is an outlier or is the typical norm for an ABFAS candidate, but it should raise eyebrows for the entire profession if this issue is a frequent problem. The facts are plain and simple... the cost and time involved to reach this pinnacle of certification may be in excess based on the supposed high quality education and training that every podiatric student receives prior to this achievement.

It is my understanding that podiatry is equal to allopathic and osteopathic medicine less a few educational courses and post-graduate rotations. And as a specialty requiring all DPMs to be trained as “foot and surgeons”, passing of said exam should be a piece of cake. Alas, it is not. The pass rate the first time a candidate takes this exam is much less than 50%.

The second or third time it is taken, the results are appreciably higher, but these multiple attempts to pass an exam which should reflect the candidate’s aptitude is unnecessary. Our orthopedic colleagues seemingly take a similar exam and have a much higher first pass rate as compared to our comparative board exam.

I find this disheartening in light of the fact that podiatry is and has been fighting for parity. This concern from this ABFAS Diplomate is valid and should be investigated. If not, then it should fe explained so that prospective ABFAS candidates can understand more thoroughly how this process really works.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From: Don Peacock, DPM

With ABFAS board certification discussions, we sometimes miss a very important point. The boards give credence to our surgical achievements and that’s all. Our greatest achievements will be in helping people, not in ABFAS certification. Lacking certification will not prevent a good podiatrist from making a great living. The huge effort to obtain and remain board-certified for the purpose of hospital privileges is becoming less important as well. I perform most of my surgeries in the office setting where hospital privileges or board certification is optional.

Whether we're board-certified or not, our surgical income means little with respect to our financial health. The bulk of reimbursement for a successful foot surgeon remains non-surgical. With current cuts in procedure-based and fee-for-service models, it would behoove all of us to move more surgeries to the office setting where reimbursement is better and turnover quicker.

In the overall scheme of things, board certification is not as important as being a good doctor, having patients that like you, and doing a good job at treating those patients. That is how you will be successful. The board certification is the icing on the cake. It is not the cake.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS)
From: Ivar E. Roth DPM, MPH

I would like to take exception to Dr. Harvey’s comments about Saturday office hours. I too did not have Saturday office hours(for over 30 years), until recently. My kids are off in college now and I decided that I wanted to expand my practice.

The only practical area for growth was to open up Saturdays, which I decided to do. Patients LOVE the convenience and it is our “casual” dress attire day. It is a big hit and I plan on opening up Sundays also now. Of course, I cannot work 7 days a week and I do not plan to. It will be a great opportunity to hire some younger practitioners who are eager to work and improve their finances. I see this as a win/win for everyone. I am curious as to how many  other practices out there are open on Sunday?
