Dehydration Dangerous with Diabetes: MI Podiatrist
According to Conway McLean, DPM, "Dehydration is quite dangerous to individuals with diabetes and carries many potential complications. Dehydration thickens our blood, leading to higher blood glucose concentrations. When someone’s blood sugar is high, their kidneys will try to remove excess sugar from the blood. They achieve this by pulling it out of the blood and excreting it into the urine, removing too much water in the process. The result is that both issues, high blood sugar and dehydration, both lead to the other and both are dangerous to the health of our kidneys."

"Diabetics tends to have kidney problems which can be exacerbated by any increase in the concentration of blood, making them work harder to remove waste. The complications of diabetes are significant and too often lead to disability, hospitalization, or worse. When it comes to management, diet and exercise are front and center, as they should be. But the role of simple, everyday water often gets overlooked. Diabetics need to pay close attention to their hydration levels since this is crucial for managing their blood glucose and also in preventing diabetic ketoacidosis, " says Dr. McLean.

UK Podiatry Service Wins International Recognition for Diabetic Foot Care
The Journal of Wound Care Awards 2025 recently awarded a Welsh health board with the Best Diabetic Foot Intervention Award. The Cardiff and Vale University Health Board Podiatry service team triumphed over strong international competition, with runners up from Italy and Singapore. The award celebrates the success of the Diabetic Foot Emergency Early Triage (DFEET) clinic, launched in 2023. The initiative was previously recognized as a finalist in the 2024 Diabetes Quality in Care Awards and Health Service Journal Awards.

NY Podiatrist Discusses What to Look for in Walking Shoes for Older Women
According to Bruce Pinker, DPM, a board certified podiatrist and foot surgeon, our feet change as we age — and it's important to pay attention to how our feet fit in shoes to avoid long-term damage. "Wearing the right walking shoes can prevent or alleviate these issues by providing proper cushioning, shock absorption, and arch support," Pinker explains. "Offering a proper width and length can also be helpful in preventing the development of bunions, hammertoes, and neuromas."

Dr. Pinker says that women over 50 should look for footwear that fits properly — making sure the shoes aren't too cramped or narrow in the toe box. He also advises to make sure you look for "adequate cushioning, shock absorption, and arch support," Pinker adds. "Extra wide sizes are appropriate for those with wide feet. Adjustable straps for shoes and sandals are recommended to provide a more stable and secure fit," Pinker says. "For flat feet, it is best to look for walking shoes or sneakers with added medial arch support. Meanwhile, for those with high arches, finding a shoe with a firm heel counter and a somewhat deep heel cup is recommended for a preferred fit."

Query: Sending Records to DPM Who Was Previously in Our Practice
Does a patient need to sign a record release authorization form if they are seeing an established DPM at their new facility? A patient had surgery from a DPM who was in our practice at the time. That DPM is now at a new location, but we still have the patient's records. Can I just forward the records to that DPM's new location, or does the patient need to sign a record release auth allowing us to forward their records to the new facility?
Response from thedoctorline.com: The answer is yes. The previous practice owns the records and they can only be accessed by those healthcare practices within that system treating that patient and the patient themselves or whomever else they authorize access to. The patient can gain access to those records by signing an authorization and they can either be sent to the patient or the patient may choose to have the records released to the new practice.
While the patient is seeing the same doctor, from a HIPAA and entity perspective, they are not. At least that's the way it works here in NY several times, when doctors moved from one hospital system to another. Lots of hoops to go through, but in essence it was no big deal. By all means, check with your professional liability perspective. I will be surprised if you receive a different answer.

There surely might be other considerations, and we do not know if this is for functional issues or purely cosmetic concerns, and not limited to the size, length, and appearance of the RIGHT foot. Why not offer the patient the idea to simply shorten the hallux and the 2nd toe at one surgical setting?

Rather than make this complex, just do arthroplasties of the 2nd and 5th toes and make them all the same length. It’s much better and easier with less complications and the patient will be happy.

Once again, Dr. Sherman presents an inaccurate portrayal of the current state of podiatry graduates, resorting to ad hominem attacks on me rather than a true representation of state of podiatry education today. Here is St. Louis, there are very busy practices with a great number of podiatrists doing serious wound care on complex patients. There are a number of podiatrists doing ankle fractures, reconstructive surgeries, TARs, and engaged in advanced wound care. There are practices with greater than 20 inpatients. In fact, as I wind down my practice, I am now a minor player compared to many well-trained younger podiatrists.
My practice is not the exception. I suggest Dr. Sherman actually read JFAS, or attend the lectures at ACFAS, or familiarize himself with the advanced care on complex patients now provided by many 3-year and/or fellowship-trained podiatrists.
I will not dignify the misrepresentations of Dr. Sherman by any further responses on this subject. He can have the last word. My discussion on this subject was intended to call attention to the outstanding education our three-year residents and fellows now enjoy. My most recent communication was intended to illustrate the status of patient care responsibility inherent in many podiatry practices in 2025, and how proud we as a profession should be with regard to the fact that our residents are so well trained to manage complex foot and ankle pathology.
