PODIATRISTS AND FOOTWEAR |
NY Podiatrist Has Novel Way to Make Dress Shoes Breathable
Wearing tight shoes that don’t allow for good air flow—like dress shoes or boots—can exacerbate sweating. Instead, look for lightweight athletic shoes with mesh on the outside, advises Danielle DesPrés, DPM, a podiatrist and foot and ankle surgeon in New York. “Mesh has airflow,” she says. Shoes without it won’t be as breathable. Many of Dr. DesPrés’ patients are doormen and security workers—people who are on their feet all day, in the heat, in heavy shoes. Their feet tend to swell up throughout their shift, making their shoes tighter and more uncomfortable. Yet they’re required to wear black shoes, she says.
|
Dr. Danielle DesPrés |
So Dr. DesPrés came up with a solution: “I’ve told some of them to get a pair of sneakers that are really comfortable and supportive, because they often have other arch and foot problems too,” she says. “Then spray paint them black”—covering up light-colored mesh, brand names, and other markings—“and now you have your black shoes.” The workers who have tried it, she adds, assure her it does the trick. Some companies also make supportive black sneakers that could pass as dress shoes.

PRACTICE MANAGEMENT TIP OF THE DAY |
5 Issues Physicians Should Consider Prior to Selling Their Practice
Recent legislative and rulemaking activity at both the federal and state levels, along with changes in certain deal dynamics, are sure to impact physician practice transactions going forward. Following is a list of five new issues physician practice owners should consider in advance of a transaction. Many of these items should be addressed in advance of going to market or accepting indications of interest from potential buyers.
2. Will your transaction require notice to or approval from a state agency? Fifteen states (California, Colorado, Connecticut, Hawaii, Illinois, Indiana, Massachusetts, Minnesota, Nevada, New Mexico, New York, Oregon, Rhode Island, Vermont and Washington) have enacted some form of legislation requiring notice to or approval from state agencies for certain healthcare transactions. The applicability of these state laws generally turns on the type and size of the transaction as well as the amount of business the buyer has in the state. Knowing in advance whether these statutes could be implicated is important both in terms of evaluating buyers and planning timing - the notice and approval requirements can add anywhere from 30 to 180 days to a transaction timeline.

RESPONSES/COMMENTS (INSURANCE ISSUES) |
RE: Podiatrist "Specialist" Co-Pay
From: Keith L. Gurnick, DPM
PPO-insured patients with Anthem Blue Cross, Aetna, Blue Shield, Cigna, HeathNet and UnitedHealthCare often have office visit co-pay requirements that are stipulated on the front of the insurance cards and also on the insurance carrier websites. Most of these companies financially "punish" their subscribers (our patients) for seeking care from a covered and contracted in-network specialist by charging a higher co-pay for a specialist than when they are seen by a primary care doctor. Traditional Medicare seems to treat all doctors the same and does not differentiate based on specialty or degree.
However, when it comes to the patient's office visit co-pay, some of these same companies that are listed above do not treat the podiatrist co-pay equally. As an example, Blue Shield of California recognizes podiatrists as specialists, but takes the co-pay as if we are primary care (which is usually substantially lower). It is inconsistent with what is on the insurance card and the company website and makes it confusing for the billing department and the front office when money is collected at the time of the visit. It would be much more efficient for billing if the insurances would simplify things and have only one category of "co-pay" or clarify to the doctors and to their subscribers how they will "categorize" and address the co-pay for each specialist.

Congratulations to Randy Crampton, DPM on his retirement. I, like many others, entered this profession due to his influence. He warmly welcomed pre-med students into his office, and took time to promote the profession. Enjoy the retirement Randy, it's much deserved.
