NY Podiatrist Uses Combination of Steroids to Treat Plantar Fasciitis

Plantar fasciitis doesn't have to disable you. A shot of cortisone is common but a new study in the British Medical Journal says it may not work in the long run. Dr. Johanna Youner says that the study used a short-acting steroid that works only for a few days.

"When I do an injection, I use a combination of both the short-acting and long-acting steroids, which works for weeks instead of days," says Youner  She says that she gets excellent results.

Buying  Running Shoes "On Sale" Can Be Costly: NY Podiatrist

Not all running shoes are created equal; picking the wrong shoe for your particular foot/body type can be an injury-inducing waste of money. Dr. Chris Bromley is a podiatrist who practices in Poughkeepsie. Oftentimes, he treats recreational athletes whose foot and ankle injuries can be traced to improper choice in footwear. He understands the sometimes confounding landscape of the athletic shoe industry, and he does his best to educate his patients on how to make proper footwear choices.

Dr. Chris Bromley

“The first mistake most people make,” Bromley said, “is going to the store and buying whatever is on sale. Don’t buy the bargain stuff. Spend a little more money on (proper) shoes, and you’ll spend a lot less on co-pays (for doctor’s office visits).” Go to an athletic specialty store with a staff that is knowledgeable in proper fit and foot type. And, Bromley recommends, go at the end of the day. “Feet are more swollen at the end of the day,” he said.

PODIATRISTS AND HUMANITARIAN CAUSES

IL Podiatrist Inspires Through Her Charitable Foundation

An inspiring widow, podiatrist Dr. Gina Santori, guides the Santori Charitable Foundation started by her late husband, Richard. Gina traveled to Cambodia last year to see what the hospital they support needed. She also toured the Santori school her husband started and reports that although the people in the village have no running water, electricity, they smile and made her a “fabulous lunch of fish, chicken and rice. They killed a chicken for me, and are so generous and warm. I will be getting them solar panels to power computers … and I hope they can weave scarves I can sell for them in America.”

Dr. Gina Santori

Gina also is busy with memorial education scholarships in Richard’s name. This amazing woman received a second degree black belt in karate. She practices mornings before she goes to the hospital to do surgery.

FL Podiatrist Honored as Volunteer Physician of The Year

The Palm Beach County Department of Health held its 34th annual volunteer awards Friday, May 18 at the Airport Hilton, honoring the dedication and commitment to better health in Palm Beach County. Presented by the master of ceremonies, WPEC-TV 12 news anchor Eric Roby, this year’s theme “Volunteer$ Are Pricele$$” acknowledged honorees for outstanding contributions to the programs that promote and protect the health and welfare of all county residents. The event honored Volunteer Physician of the Year, Dr. Ira “Ike” Jacobson.

Dr. Ira Jacobson

Jacobson is a podiatrist and has served the Glades area for 22 years. He is also a member of Project Access. Jacobson has an ongoing policy in his office that if any patients are uninsured, he accepts only what the patient or the family can afford no matter what treatment is necessary.

Specialty Boards Set Time Limits for Certification

In an effort to prevent physicians' indefinite use of the term "board-eligible," all member boards of the American Board of Medical Specialties will limit the time that physicians have to undergo board certification after completing their residencies. Some of the ABMS' 24 member boards already have certification time limits in place; the rest will complete a transition to time limits by Jan. 1, 2019.

According to an ABMS news release, the organization has never recognized the term "board eligible"—a term some physicians use to signal to patients and prospective employers that they intend to seek certification, the release notes. Establishing time limits, according to the release, "makes it legitimate" for physicians to claim eligibility while preventing the term from being abused. Most of the time limits for the 24 boards range between five and seven years.

QUERIES (EMR)

Query: Allscripts My Way

My hospital is making an EHR available to the community docs at a very reasonable buy-in. Does anyone have information or personal use with Allscripts My Way? I would appreciate any insight.

QUERIES (NON-CLINCAL)

Query: Ovation Medical

In June of 2010, my front office person received a call from J. W. of  Ovation Medical offering to send us "samples" of their pneumatic walkers to try at no expense. About a week later, we received a large box from Ovation. Within a few days, she received a card from J.W. saying that he, "Appreciated her giving him a chance to earn her support." Since I had not requested these devices and was not interested in using them, I decided to not open the container.

In less than 3 weeks, we received an invoice from Ovation Medical for $132.42 for three pneumatic walkers plus S&H. We called and complained that we had not ordered these devices and they eventually had Fed Ex do a pick-up. Last week, we again received a "sample" from Ovation Medical, and I suspect we will be receiving an invoice soon. Has anyone else had similar experiences with Ovation?

RESPONSES / COMMENTS (NON-CLINICAL) - PART 1

RE: Exparel
From: Allen Jacobs, DPM

I was recently introduced to a long-acting local anesthetic, Exparel, which might be helpful in providing pain relief following surgery. It is a bupivacaine liposome injectable suspension which has been studied for application in bunion as well as soft tissue surgery.  It is indicated for post-operative analgesia and provides prolonged analgesia for as long as 72 hours following bunionectomy. It offers an alternative to pain pumps and reduces the need for additional analgesics. It is injected directly into the surgical site.

Disclaimer: I have no relationship with the manufacturers, Pacira Pharmaceuticals, and merely wish to pass on this information to the PM News readers.

RE: Vision 2015, Vision 20/20 (Ed Davis, DPM)
From: Robert Scott Steinberg, DPM

Dr. Davis' comments are insulting. His vision is closer to 0/0! If orthopedic residencies are 4-5 years, (you know that covers every bone and joint in the body), how can he say our profession needs 4-5 year residencies?

We have lost ground in biomechanics, thanks to the orthotic companies selling us out (and our still supporting them). The APMA is only focused on their damn surgical boards. There is no reason for us not to take back that ground. Our biggest stumbling block is our colleagues who want to charge for orthotics, but relegate impression capture to an office assistant, using the pathetic argument that it's good practice management.

A permanent place in Medicaid requires the passage of Title XIX, something the APMA has thus far not been able to do. But, I don't place the blame at the feet of the APMA, I place the blame at the feet of every single podiatrist who cannot produce copies of multiple e-mails to their congressman and senators.

RE: Tablet PC Recommendations (Lori Weisenfeld, DPM)
From: Marc Garfield, DPM

Your best bet at this time is probably the Fujitsu Lifebook. Toshiba stopped making PC convertible tablets, but the M7xx series was excellent and still is useable, if you can find one. I have thrown out several HP tablets and would not recommend them. Motion and Tough Book are ridiculously overpriced and underpowered.

Lenovo, and ASUS cut more corners and are not as well-suited for all day EMR use (I have not used them, but looked into them previously. There may be some recent changes, but for the most part, computer companies are chasing the IPAD market now), but could be worthwhile if you had a reasonable deal on one compared to...

Editor's note: Dr. Garfield's extended-length letter can be read here.

RE: Unethical Behavior by Foot Solutions (Cam White)
From: Marc Katz, DPM

Who really cares about Foot Solutions or Cam White or Novascarpa? There is nothing you can do about them or their behavior. How about concentrating on our own practices and behavior? Treat patients properly and prove to them and the whole medical community that podiatrists are the experts for all things feet! Do that by building yourself up, not putting others down. Stop the whining already and use your time more wisely. If you are not sure how to accomplish this, I recommend going to one of the most valuable and positive meetings in podiatry, AAPPM.
