Dr. Allen continued on to the Westside Podiatry Reconstructive Foot and Ankle Fellowship at Westside Hospital in Plantation, FL.

VA Podiatrist Discusses RCT on OTC Antifungal
For years, podiatrists have been carrying OTC anti-fungal products in their offices. Scott Malay, DPM was the lead author of a randomized controlled trial (RCT) to evaluate the efficacy of Tolcylen to treat onychomycosis. He decided to use PAS to evaluate the cultures because he believes they are more accurate than conventional methods. The study had a total of 69 participants that had debridements in both groups and Tolcylen applied to the toenails in the test group. At the end of the nine months, 54% of the treatment group had PAS negative tests, meaning there were no signs of fungus in the nails.

The study is available online in Infectious Diseases and Therapy. It is an international, open access, peer-reviewed (single-blind) publication. Another OTC study of Tetra is pending.

Podiatrists a Major Presence at NCVH
Podiatry was a major presence at the 2025 National Cardiovascular Horizons (NCVH) meeting in New Orleans. Sessions included a section titled "Podiatry’s Role in PAD Care" and "Podiatry and Wound Care". Podiatry has had a growing presence in this meeting over the past few years, allowing collaborations between the podiatry and vascular world to grow.

Query: Short Proximal Phalanx
Over the 36 years of surgery, I have done many osteoclasis procedures on metatarsals—but never on a proximal phalanx alone. A gradual lengthening is preferred over a single stage lengthening due to vascular concerns.

Query: CPT Code for Keller Bunionectomy
What is the proper CPT code for the so-called Keller bunionectomy. I'm going round and round with my coder and some recent CPT changes. It is their position that a Keller is two osteotomies (resection of the medial 1st Met. head and proximal phalanx).

Response from thedoctorline.com: The term "Keller" bunionectomy was removed from CPT on 1/1/2017. In fact, all named procedures were removed like Austin, Mayo, McBride, etc. The proper CPT terminology for CPT 28292 - Correction, hallux valgus with bunionectomy, with sesamoidectomy, when performed; with resection of proximal phalanx base, when performed, any method.
You stated you did "resection of the medial 1st Met. head and proximal phalanx", this is CPT 28292. The position that a "Keller" is two osteotomies is not appropriate at all, the procedure is removal of bone in two different bones. I applaud Dr. DeHeer's initiative on the New Student Enrollment Campaign. Many well-intended and stellar colleagues in academia obtain other doctorates after their DPM degree to facilitate recognition and parity among their international peers, but they never cease to promote and advocate for their chosen profession. On the other hand, there are other colleagues who may be disillusioned, disappointed, or even feel like second class citizens, thus continue to advocate out loud for a degree change and a supposed plenary license when, if we're completely transparent here, it's all about the money.
If you belong to that second group, you have all the right to seek alternative professions or go back to MD or DO school and seek the specialty of your calling. Yes, the medical profession is a calling and not a means to an economic end. If you do what you love, work hard, and are passionate with the calling to be a podiatrist, you will be just fine.

I have served as a medical director for a large physician group as well as the podiatry directory in the hospital's Orthopedic & Spine center. I assisted in group development and physician recruitment and contracting. When contracting, we reviewed MGMA data which is designed around group model compensation. Their data does not reflect accurate private practice model salaries. In other words, MGMA tabulates for compensation taking into account downstream revenues. The MGMA compensated podiatrist would be at a higher tier (300k+) vs the independent podiatrist range of 150-200K. MGMA assumes an estimated average of 100k loss per provider on the professional component service, given high cost of overhead in a system practice.
Therefore, in all truth, the group model compensates on downstream revenue (i.e., OR revenue, MRI revenue, Admission revenue, etc.). Unless a private physician has access to investment in a surgical center or the like, compensation for private practice physicians cannot keep pace. As far as recruiting, one need only look at our...

RE: Podiatry’s Greatest Generation

Several of us have written about the podiatrists who mentored us in podiatry, mostly guys who were unselfish with their knowledge and time. As Tom Brokaw wrote, they stopped the greatest threats to mankind and returned home without their comrades. When they spoke, we listened and many would not talk about what they had been through. My father was a medic attached to the Marines in the South Pacific hitting a number of beaches in the first wave. When he was a 19-year-old kid, he was deciding which 19-year-old kids were going to live and which ones were going to die on that beach. The only thing he told me was that on bloody battles like Guadalcanal, he took more cigarettes and morphine for the dying. He didn’t worry much about addiction on that beach. In return, the kids he didn’t know gave him letters to send home. That’s all he ever told me.
We respected and tried to emulate. They set us straight when we complained and we knew what was expected of us. I worked...

Dr. Jacobs has somehow gotten the impression that someone is arguing for less than 3 years of residency training for podiatrists, so he continues to argue that we need 3 years of training. I don't know how he has gotten the impression that any of us want to reduce the length of residency training. I have not seen anyone express that opinion and I have certainly not expressed it. He is arguing, strongly, against no one. And he regales us with an, admittedly, heroic recounting of his treating a life-threatening necrotizing fasciitis and making sophisticated decisions regarding IT. Good for him. I admire him. But how many podiatrists or for that matter, MDs/DOs want that kind of practice?
Most podiatrists would gladly refer that patient to him or the other minority of DPMs or MDs/DOs who want to treat that high risk, critically ill patient. We speak in this forum about the danger of prospective students reading negative opinions and criticism about the profession. But what do you think is the impact on that student with reading that treating this type of patient is what they will be doing as a podiatrist? How does that affect their inclination to apply to podiatry school? My opinion is that we need a few heroic foot surgeons like Dr. Jacobs in each community who do these types of cases, with the rest referring those cases to them. All DPMs need not be created in his image. We need a variety of podiatrists to treat the cross-section of patients who see podiatrists, and their training should be appropriate for what they actually do treat.
