Feet Should be Inspected Daily: DC Podiatrist 
 
Suddenly swollen feet can indicate high blood pressure, said Kirk Geter, DPM, the chief of podiatry at Howard University Hospital. Swelling can also be a sign of gout or kidney problems, Dr. Geter added, while tingling and numbness in the feet can indicate diabetes.

So each day, you should inspect the bottoms and tops of your feet — as well as between your toes — and wiggle your toes around, Dr. Geter said. Look for cracks in the skin, redness, changes in nail color, numbness, sores that won’t heal, and swelling, he said. “And if something doesn’t feel right or look right, then you probably need to get it checked out,” he added.

Tips for Easing Patient Collections in Your Practice
 
Starting in 2023, some credit-reporting firms will remove unpaid medical debt of less than $500. Asking patients to pay their medical bill may become more difficult when there are fewer financial consequences. Here are some strategies to ease the burden of patient collections and improve cashflow and revenue:
 
6. Prepare to issue refunds. - Collecting the amount the patient is responsible for before services are rendered is never an exact science because amounts are always an estimate. This means practices may need to issue refunds if they collect more than what the insurance company deems the patient ultimately owes, says Rozmin Bapat, president of CodeRite Healthcare Consulting
 
She suggests running a monthly refund report from within the practice management system. Then check to ensure those refunds are legitimate, meaning that a payer didn’t accidentally overpay the claim. Also check to determine whether there are any pending claims to which the refund could be applied. Once the practice is sure the patient is owed that money, the clock starts ticking in terms of when it must issue that refund, says Bapat. Each state has its own specific requirements for other payers, she adds.

Query: Subsequent Debridements
 
I have been getting insurance denials for subsequent CPT 11042 after one has been previously performed. Regardless of what level of tissue is debrided, my biller is saying I can only bill CPT 11042 every 60 days. The biller is telling me that if I debride any level of tissue within those 60 days, I should bill a CPT 97597 until 60 days has passed. Then after 60 days, I can bill the CPT 11042. This does not seem appropriate. Should we appeal?

Response: To start, your biller is wrong. With that out of the way, CPT 11042 has no global period. Always code to the depth of debridement. Always code what was performed whether it is covered or not. To substitute another code just to get it covered is fraudulent. The denial is more likely due to the diagnosis code, modifiers, OR the payer wants to see the records before they pay. It is difficult to offer concrete advice without the name of the payer, diagnosis codes billed, and modifiers used if any.

I am retired but I would like to share with the new graduates a modification (of which there are many) of an old proverb which I hope is relevant and will help them in their careers and life:
 
"If you know that you know, stay humble.
If you know that you don't know, you are wise.
If you don't know that you know, you need a vacation.
If you don't know that you don't know, you are dangerous."

I get frustrated every time I see a post regarding diabetic insoles with therapeutic footwear! APMA has been working on pushing a bill through Congress to ease the requirement of an MD/DO signing off for patients. Most patients never see a physician but only see the PA/NP. How many of the readers of PM News have contributed to APMA PAC?; how many have contributed to their state association, and finally how many of the readers are members of APMA/state association? 
 
Please join your state association/APMA. Please keep up with APMA news. Please support your PAC. Many of the diabetic insole questions are answered regularly by APMA.
