Stretching Can Reduce Hyperpronation: CA Podiatrist
 
“One of the most common functional [foot] deformities is hyperpronation, or flat foot, which is known to cause many conditions like bunions, hammertoes, and plantar fasciitis,” explains LA-based podiatrist Albert A. Nejat, DPM. “Stretching the feet, but mainly the calves and hamstrings, can be very beneficial in reducing hyperpronation and other issues.”  If you often wake up with stiff legs and feet, try this exercise before even getting out of bed.

“In a seated position, place the mid-portion of a non-elastic strap on the bottom of your forefoot. You can use a leather belt, yoga strap, or even a towel,” says Dr. Nejat. “With a slightly bent knee and a straight back, gently pull the ankle up until you feel a pull in the back of your calf. Hold for about 20 seconds on each side, and try not to bounce the leg.” Slowly bend and extend the knee for 20 seconds on each side. Finally, with your leg extended, bend your torso toward your knee to also get a stretch in your hamstring.

Shoe-Buying Women Need to Know Their Foot Type - NY Podiatrist
 
 "Women need to know their foot-type first, and be realistic about expectations of shoe type," says Krista Archer, DPM, a New York-based podiatric surgeon who appreciates both foot health and footwear. It's absolutely necessary that you learn about your own foot before you make a purchase. As Dr. Archer says, "a swooping generalization can't be made" for all feet, so make sure you get yourself checked out if you're experiencing pain.

"The higher the heel, the more severe the slope of the mid-foot, the greater extension of the big toe, the more load going through the ball of the foot, then the heavier the more load is placed on the forefoot," Dr. Archer warns. So do your research on your own foot, and read up on shoes that are good for you.

If you feel stuck in a lousy deal with your payers, you can renegotiate. Here are some steps to renegotiate your contracts and improve your revenue. 
 
4. Hold the line - Now that formal negotiations have begun, hold the line. Remember, negotiations start with a “no.” Payers will push back and even refuse to open up talks. Press on, and don’t give up. Stay focused on your mission and purpose. The key to any successful negotiation is knowing what you want. Be specific with your desires. Don’t let their fear tactics intimidate you, and always be comfortable exercising your right to veto.

Query: Hospice Billing and Coding
 
My patient with a leg wound secondary to cancer just entered into a “palliative care” program administered by a local hospice company. She says that the hospice company told her that I can continue to treat her leg wound even if her condition is related to her hospice diagnosis (cancer) since she is not in actual hospice, but in “palliative care.” I am hesitant to continue to treat her, but both she and her home health nurse insist that this isn’t typical hospice and I can still treat her although her condition is related to the reason she is being seen by the hospice company. Can I continue to treat her and bill Medicare and is any special modifier  needed?

Response: As far as I know, a hospice is hospice. That being said, if you are not treating the cancer itself, you can bill for other services and append your billing lines with the GW modifier. Your treatment should reflect the patient's overall status. If the patient has a limited life expectancy, then using expensive wound skin substitutes may not make any reasonable or medical sense. Maybe a simpler dressing to keep your patient from getting infected would suffice. Document your train of thought and rationale for doing what you are doing as this can be a tricky time and you don’t need any hassle from some long lost relative.

Does anyone have any experience with injecting Botox or Dysport for plantar fasciitis? The research I've seen seems to be promising. Just wondering if anyone has recommendations or any pros or cons about using it.
