IA Podiatrist Treats Athletes from his Own Experience as a Triathlete 

Dr. Kirk Neustrom's experience as a triathlete helps him treat foot and ankle problems at his podiatry practice in West Des Moines, and satellite offices in Winterset, Carlisle and Adel. He is the medical director for this weekend's Dam to Dam race and also serves on the medical team for the Des Moines Marathon. He has worked on the medical teams for races across the country, including the Ironman, and marathons in Boston, Chicago, New York City, and Los Angeles.

"With the weather finally breaking and people getting more active, I see a lot of heel pain, plantar fasciitis, Achilles tendonitis, stress fractures, shin splints, knee pain. Most are overuse injuries because people come out to train and don't have the right equipment. Injuries come from the theory of "toos": too much activity, too soon, and too frequently."

First Endowed Scholarship Established at WesternU Podiatry College

Western University of Health Sciences’ College of Podiatric Medicine received its first endowed scholarship commitment for the next generation of podiatrists. OsteoMed L.P. has made the first of five installments that will total $20,000 to create the OsteoMed Endowed Scholarship.

TX Podiatrist Receives Distinguished Alumnus Award from Rowan University

Dr. Marque Allen of San Antonio received the 2010 Distinguished Alumnus Award from Rowan University on May 14. Allen, who earned his bachelor’s degree in biological sciences from the Glassboro, NJ-based institution in 1991, accepted the award during Rowan’s annual undergraduate Commencement ceremony.

Allen is a partner in Sports Medicine Associates of San Antonio and is board certified in foot surgery and ankle and reconstructive surgery by the American Board of Podiatric Surgery. He serves as the foot and ankle consultant for the NBA’s San Antonio Spurs, the WNBA’s San Antonio Silver Stars, the AHL’s San Antonio Rampage, the University of Texas-San Antonio, Trinity University and multiple high schools throughout southern Texas. Last  fall, Allen and his wife, Yvette, presented Rowan with the first donation to fund a scholarship at Cooper Medical School of Rowan University.

QUERIES (CLINICAL)

Query: Maceration, Pitting Keratolysis, and Hyperkeratosis

I have a 60 year old female patient with 14 year history of B/L sweating feet with maceration, pitting keratolysis, and hyperkeratosis. The onset was from sitting in a hot tub. She has tried Botox injections, electrophoresis, topical astringents (Drysol, Betadine) oral antibiotics, T-stat, and gentian violet. The oral antibiotics have decreased some pitting, but after three months of this, it is still evident. Any advice?

Given the history of this patient, a 60% chance of success from a spinal cord stimulator is a great possible outcome. If it does not work, one of the last options would be to undergo a regional sympathectomy and weight its benefits vs. complications.

I had a patient who developed CRPS after surgery. Pain management tried absolutely everything with no success. They were about to do the spinal cord stim when I came across the option of trying hyperbaric treatments, which are currently being studied for this problem. After one week of treatment, her pain went from a 15 (on a scale of 10) to a 4. The other day, she called me to say that she had gone roller skating and was living as normally as she did before the CRPS. It’s certainly worth a try before the cord stim.

If autonomic ganglion ablation has been ruled out, I manufacture an alternative topical treatment I'd be glad to furnish a sample of to your patient. She applies the formulation over the ipsilateral lumbosacral area, as well as over the superficial courses of the nerves travelling distally. The formulation is all-natural and was designed to treat pain that did not respond to other modalities. Applied in this fashion three times daily in the beginning, I have used this to significantly reduce narcotic intake in these types of cases.

I would elect for an aggressive TMA with TAL. This patient probably has advanced contributing co-morbid factors. The remaining digits would only be a source of breakdown in the future and probably have compromised small vessel disease-- the calcifications affect the distal forefoot and are not limited to just the hallux. A good TMA with a maintained parabola of the remaining metatarsals afford a better weight-bearing surface, and fitting a forefoot prosthesis is very manageable.

With that amount of soft tissue destruction and bone exposure, I would take the surgery back as far as the Lis Franc joint, go back to an area of better blood supply, and maintain and fashion a viable plantar flap. Keep the patient in the hospital until bone cultures and biopsies are back, and discharge with the appropriate IV antibiotics, strict off-weight-bearing, and daily dressing changes.
 
Of course, the disclaimer must be that this is a limb salvage, and it is an attempt at saving the patient's leg. If this fails, a BKA is the only other option.

RE: Chaetomium Onychomycosis (John Strisower)

Mr. Strisower has presented an interesting request: "we will need to know the subspecies of chaetomium (there are dozens of subspecies) isolated from the patient."  The lab that I use rarely provides the name of the subspecies, except for the dermatophytes. Perhaps, there is a specific wording that we should use to get this information? The requisition form (from the lab) that I use has a mycology area in which I can choose PAS with histologic exam and/or fungal culture & ID and/or fluorescent KOH. Is it possible that clinical laboratories are not reimbursed from insurance companies for such specific identifications?  If I am understanding this post correctly, unless we can identify the organism to the highest level of specificity, we cannot be certain of the effectiveness of laser therapy in the treatment of onychomycosis.

RE: Medical Marijuana and Liability Issues (Michael M. Rosenblatt, DPM)

While I very seldom disagree with the esteemed Dr. Rosenblatt, I must take issue with his conclusion that "It is probably legal for podiatrists to prescribe medical marijuana to treat foot and ankle disorders." The last time I looked, the prescribing and/or dispensation of Tetrahyrocannabinol (THC) in any form was still a violation of Federal law. State laws which purport to permit medical use of marijuana, or other substances containing THC, do not, repeat do not, supercede the federal law.
 
I take no position on whether "podiatric pot" is a good idea medically.That's for the physicians to decide. But the one thing podiatric pot definitely is not, is legal. Just because you won't be prosecuted for doing something doesn't mean it's lawful to do it. So, as the esteemed Dr. Rosenblatt correctly concludes, if the use of podiatric pot is medically and/or legally questionable, why do it at all?

Reading Dr. Rosenblatt's discussion of medical marijuana jogged my memory back to an out-of- the-office conversation I had long ago forgotten.

During the debate over California's first decriminalization of marijuana for medical use during the Clinton administration, I told an older gentleman that I was glad I would never be faced with such a prescription dilemma. He then confided to me that he himself had faced the problem of gangrene-caused auto-avulsing digits. He assured me that no drug, narcotic, or otherwise, touched his agony, but marijuana did. I find that astonishing, and now I wonder how well it might work for the problem of painful neuropathy, which is so common. Food for thought.
