Wearing Heels Alters Center of Gravity: NY Podiatrist

The phrase "commuting shoe" is enough to send many professional women over the footwear-edge, haunted by visions of modern day Working Girl troops in laced-up New Balances or pedicure-exposing Havaianas.

Dr. Robin Ross

While the silhouette of a stiletto looks infinitely more stylish and commanding than a running shoe or thong sandal, professional women making their way into office-buildings in cities and suburbs across the country in comfortable shoes have the right idea. "Wearing heels will change a person's center of gravity," says Dr. Robin Ross, a practicing podiatrist and spokeswoman for the New York State Podiatric Medical Association. Altering the balanced position of your body can lead to falls, sprains and breaks, not to mention back pain. The many effects of tight high-heels read like a laundry list of unsightliness: bursitis, bunions, and corns included.

E-HEALTH NEWS

Physician Internet Use Is Growing Rapidly, but Mobile Is the Trend to Watch

As health plans search for ways to reach and motivate members to become more engaged in their health, they say that one lesson stands out loud and clear: In today's world, people won't come to you on your terms. Health plans that reach out to their members on their terms using tools that reach them wherever they happen to be with tailored, focused messages stand the best chance of success. And one effective way to do this is with cell phones and other mobile devices.

William Reed, CEO of AllOne Health says that the cell phone is quickly becoming the least common denominator for all generations, and that "the mind and the imagination are the only limits to how mobile devices can be used" in healthcare. Over the next several months, AllOne Mobile will be launching a program for the U.S. Army using mobile devices to help its "wounded warriors" better manage their post-injury lives. The mCare Project rollout eventually will link 10,000 soldiers who have returned from the war zone with significant injuries with their case managers, who will use the soldiers' mobile devices to track their rehabilitation progress, personalize and monitor daily care and stay in touch through personalized encrypted messages.

PRACTICE MANAGEMENT TIPS FROM AAPPM

Ways to Organize Your Physician Marketing

In podiatry and other medical specialties, one of the most powerful ways you can market your practice is by visiting potential referring physicians’ offices. To help develop a system for doing this, stop by your local hospital(s) and visit the medical staff office. It is here that you can request a copy of a medical staff directory giving you a “road map of sorts” on whom to visit in your area.

Typically, these directories are broken down by specialty, so you have the ability, for example, to visit all the local primary care/internal medicine doctors, followed by all of the pediatricians.  After you complete this, you can then focus on more niche marketing areas such as endocrinology or infectious disease if this is an area you are looking to develop. As you visit each office, keep a record of whom you visited in a smart phone or day runner, then cross out the names in the medical staff directory. This will give you some structure to a task that seems a bit large to begin.

QUERIES (CLINICAL)

RE: Terminating a Podiatry License

I have a license in a state that I will never practice in and never have, and it is up for renewal. I want to know what advice can be given regarding a license in a state that a podiatrist has never practiced in. Is it any different than a state that a physician does practice in? Is there a "right" way to give up the license? Any input would be greatly appreciated.

Philip E. Larkins, DPM, Escondido, CA

Editor’s comment: PM News does not provide legal advice. You should inquire with the licensing agency as to the state’s policy. In many states, you can change your license to “inactive” or “retired” status. In other instances, you simply inform the state that you do not intend to renew your license.

RE: Prepare for the Unexpected
From: Bret M. Ribotsky, DPM

Each and every day, will fill our lives to the point that we don't have any time to a lot of extra stuff. On Thursday morning, I received a phone call that my house was on fire. Within in a few minutes, I left my office to find seven fire trucks on my block. End result: All my stuff (everything) at my home is gone. The house was empty and the cause is to be determined.

Dr. Ribotsky's House Post-Fire

I have learned a lot dealing with adjusters, inspectors, etc. Fortunately, I had n videotaped all my belongings. I had inventoried each shirt and each electronic item (including reading off the serial numbers). I advise that you do the same. Your tape may be several hours in length, but when you look at the pictures of my room, you will be happy you did.

I want to thank all those who have contacted me with their kind words. Computer back-ups off-site work well. I can say that my Carbonite web-based backup, which had over 200 gigs of my digital records was a great investment. Caution to all - Be prepared.

Bret M. Ribotsky, DPM, Boca Raton, FL

RE: Post-Op Care for Alzheimer's Disease Patient
From: Daniel Cohen, DPM

For clarification on my post regarding my patient with Alzheimer's, I would like to add a few points. The procedure was not performed as an "ambulatory surgical case." The patient was seen in consultation as an in-patient with a diabetic foot infection with exposed bone and acute osteomyelitis. His amputation was preceded and followed by an in-patient hospital stay and also included consultations with vascular surgery and infectious disease, all who agreed that an amputation was the best option. He was then sent to a skilled nursing facility and subsequently is residing in a facility that specializes in caring for Alzheimer's patients.

Since we can't restrain a patient for 24 hours a day indefinitely, and my patient is self-mutilating at night when he is left alone, I was wondering if anyone knew of devices to allow a patient to not be fully restrained while still preventing them from pulling off a bandage  (Like boxing gloves, but that's really not an option.)  I appreciate the suggestions I have received via e-mail.
