PODIATRIC EDUCATION |
Cost of Podiatric Education is Biggest Stumbling Block: IL Podiatrist
The biggest stumbling block to becoming a podiatrist is the cost of schooling. Dr. Laura Pickard said that when she finished school, she was $125,000 in debt. For today's students, the figure is closer to $200,000, and that doesn't include debt incurred as an undergraduate. So what would make someone want to assume that kind of financial burden?
|
Dr. Laura Pickard |
"Honestly, you have to have a passion for going into the field," Pickard said. "You know what you're going up against. The field has really progressed; it's comparable to MDs (medical doctors) and DOs (osteopaths). You don't go into medicine in general because you want to make a lot of money. You have to have a passion to take care of people. You've got to be motivated by the right reasons."

Athletic Shoes Need to be Sport-Specific: IL Podiatrist
Whether it's running, biking, or walking, selecting the proper athletic shoes for your activities can have a significant impact on preventing injuries. Dr. Bruce Williams, sports medicine and gait analysis podiatrist at the Weil Foot & Ankle Institute, says athletic shoes should be specific to the activity and individual feet as well.
|
Dr. Bruce Williams |
"While exercising, the way we use our muscles, joints, and tendons is key to a successful workout. In order to facilitate proper body mechanics, your shoes need to be appropriate for the type of exercise and your specific feet," says Dr. Williams. "Many common injuries, including plantar fasciitis, shin splints, or stress fractures, can be prevented and helped with the proper athletic shoes."

STATE PODIATRY NEWS |
Illinois Senate Approves Medicaid Expansion Restoring Podiatry
The Illinois Senate has advanced a plan to restore Medicaid cuts made in 2012. Senators voted 46-10 on Thursday to expand Medicaid to include funding for adult dental and podiatry services. Those were cut two years ago to save money. Republicans have expressed concerns about sustaining services during a difficult budget year. The Illinois House approved the measure on Wednesday. Democratic Rep. Greg Harris is a bill sponsor. He has said that the 2012 cuts didn't save the state money because people wound up in the emergency room, which costs more.

HEALTHCARE NEWS |
Most Medical Groups Are in ACA Exchanges and Dislike It
Group practices are participating in new insurance plans created by healthcare reform by and large, but they are grumbling about it, mostly because of administrative hassles and narrow provider networks, according to a new survey by the Medical Group Management Association (MGMA). Almost 77% of MGMA members have agreed to be network providers in one or more private insurance plans sold through state marketplaces, or exchanges, under the Affordable Care Act (ACA). The leading reason for signing up, cited by 58% of participants, was to remain competitive in their local market.
But what do group practices expect to gain from the ACA exchanges? Not much, according to the survey, released on May 20. Fifty-nine percent of all practices, participating or not, said the exchanges will have either an unfavorable (44.3%) or very unfavorable (15.1%) effect on them. Only 13.8% of the 700-plus groups polled predicted a favorable or very favorable effect.

CLINICAL CASE OF THE DAY |
RE: Photosensitivity Reaction from Lovenox Combined with Extreme Sun Exposure
|
Photo sensitivity reaction to Lovenox |
This 38 year old male presented with a rare reaction to Lovenox combined with photosensitivity. The literature reports ten other cases. He is on daily Lovenox because of a hematological disorder which causes him to develop blood clots. He spent the day on the beach over the weekend. This is the second time this has happened to him in the past three years of being on daily Lovenox. The past event was also caused by sun exposure.

RESPONSES/COMMENTS (CLINICAL) |
From: Don Peacock, DPM
This would be a great case to utilize minimally invasive surgical techniques. Down’s syndrome patients have diminished muscle tone and, therefore, hypermobility. It is likely this patient would benefit greatly with stabilizing the subtalar joint, utilizing a HyproCure stabilization device. This would most likely reduce the IM angle, theoretically to 16 - 18°.
Following the subtalar joint stabilization, you can perform a percutaneous head metatarsal osteotomy along with a lateral capsule release and Akin osteotomy. This can be done under light sedation, and if the patient is amendable to it, under complete local.

Pre-operative and immediate post-op views |
Here are pre-operative and immediate post-op views of a patient on whom I utilized MIS techniques to reduce to an IM of 18 degrees.

RESPONSES/COMMENTS (DME) - PART 1 |
From: Connie Lee Bills, DPM, Paul Kesselman, DPM
Yes, I too have been amazed at this policy since we discovered it about 3 years ago.
Anthem's policy, like many other health policies, are cost-driven only (for the immediate or near term). They often don't address long-term or preventative solutions. I agree that this policy makes little sense. I am assuming this is for an active employee and not a retiree or Medicare eligible patient. For these patients, Anthem usually has policies which parallel that of FFS traditional Medicare.
However, be careful what you ask for. Many major FFS non-Medicare or Medicare advantage plans which do cover shoes have fee schedules which are far below a reasonable retail rate.

RESPONSES/COMMENTS (DME) - PART 1B |
From: Greg Mangum, DPM
In Texas, there is a "mandate in force" to cover orthotics(shoes) for at-risk diabetics for Blue Shield. I have been denied as well as non-covered, but on review I have been successful.
Ask for a "Right of Review"- cite that there may a "Violation of Title 29 of the United States Code of Federal Regulations-section 2560.503-1(f)(2)(iii)" , which mandates the coverage for diabetics at risk. Ask for the documentation or articles used in the denial and the credentials of the person denying the claim. I assume the Fed Regs are the same for Ohio.
