MD Podiatrist Educates College Students About Podiatry
 
David Engorn, DPM recently provided a lecture to the University of Maryland, College Park pre-medical society students about a career in podiatric medicine and surgery. Dr. Engorn is an alumnus of the University of Maryland, College Park. He volunteers his time and allows local undergraduate students to shadow him as they explore the various medical fields. The talk focused on the podiatric medical school curriculum and its parallels to the allopathic and osteopathic education, residency requirements and experience, fellowship opportunities, and practice types for foot and ankle specialists today.

On Sunday, May 28th, David Weiss, DPM of Richmond, Virginia partnered with local restaurant, Rare Olde Times, to raise money in support of a very worthy cause, The Wounded Warrior Project. It was an enjoyable evening as Dr. Weiss sang and played the guitar to a full house.

VA Podiatrist Discusses Youth Sports With IL Sports Podiatrist
 
Ben Pearl, DPM discussed challenges with youth sports with Bob Weil. DPM. on his podcast Fit Foot U. Dr. Weil has co-written a book, #HeySportsParents!, which is featured in a segment in an upcoming documentary, Where Our Children Play -The Challenges of Youth Sports. The pressures parents and coaches place on children can lead to overuse injuries and eating disorders. These problems are especially prevalent in sports such as gymnastics, figure skating, and tennis.

Currently, CMS is doing massive audits on diabetic shoes and are very picky about what they allow. An NP may conduct the exam but it has to be signed by the MD or DO overseeing the patient's diabetic plan of service, and not just signed. There must be a statement such as, "I agree with the above findings" or CMS will reject it.  
 
Also, the auditors will claim that the MD or DO is not truly overseeing the patient if they have not personally seen the patient in the past six months. In other words, people in rural areas who go to a clinic manned by an NP who has their practice overseen by an MD or DO are flat out of luck because they never actually see that doctor.

It was important to provide an additional response to the letter I provided in a recent issue of PM News. I purposely did not expand on the issue of when the NP/PA does the foot exam and prescribes the shoes (as they have the legal right to) and thus is the prescribing entity. This circumstance was purposefully omitted out of an abundance of creating more confusion. But since Dr. White brought this up, I feel compelled now to clear that issue up as well.
 
Dr. White is correct in that PAs and NPs under Medicare are eligible prescribers for therapeutic shoes and inserts, as well as ordering many other tests and DMEPOS. That is NPs/PAs can both prescribe shoes and inserts working incident to (under direct supervision with the MD/DO in the office) or under general supervision (when the MD/DO is not in the office but...

I have been following the discussion of the various medications that are being used by some in their office for pre-operative sedation. I fondly remember discussing the selection of drugs for use in office cases with the chief of anesthesia at NYC’s St. Barnabas Hospital while I was a resident there in 1984. At the time, an entire issue of Clinics in Plastic Surgery was devoted to office-based surgery since plastic surgeons, like podiatrists, were among the pioneers in the field. One article, “The Selection of Drugs in Office Surgery” discussed the use of the various classes of drugs in the office setting.
 
I asked the question of him, if he were going to operate in an office, what would be the one thing he would use. His pithy answer was that the one thing he would use would be an anesthesiologist. He punctuated his advice by informing me that if you put it in, you had better be able to take it out. To this day, if my patient is overly anxious or fearful and might need pharmacologic assistance, they go to the surgery center. The results of the reader poll showing over 70% of podiatrists steering clear of in-office sedation appears to be consistent with that decades old advice. I have no desire to manage a potential adrenergic response to anxiety, fear, pain, or hyperstimulation.
