GA Podiatrist Uses Stem Cells to Save Foot With Necrotizing Fasciitis

Bobby Rice says it all started when he cut his foot after stepping on a piece of glass last summer. Not thinking anything of it, he Superglued the cut and went on. A few weeks later, it was badly infected and his doctor diagnosed him with necrotizing faciitis, which is also known as a flesh eating bacteria. When his doctor first removed the flesh-eating bacteria from his foot and up his leg, there wasn't much left but bone, and amputation was likely. Then his podiatrist, Dr. Spencer Misner, thought of an experiment using stem cells from placenta from umbilical cords. "Why not? I have nothing to lose and everything to gain," Rice said. 

Dr. Misner contacted a facility in California. They told him stem cells had never been used to re-grow a foot, but donated the tissue so he could try. "Kind of flying by my boot straps. I just was kind of guessing along the way," Dr. Spencer Misner said. "Once I started seeing healing of the tendons take place, I started getting a little goosebumpy," Dr. Misner said. After a few more treatments, Dr. Misner says Bobby will be as healed as possible and will eventually be able to lose the walker.

Flop-Flops are Aptly Named: Canadian Podiatrist

Are we such slaves to fashion that we opt for sparkly flip-flops and six-inch wedges over ultra-comfortable sports sandals? Sorry, ladies, but when it comes to footwear, beauty shouldn’t have to mean pain. As the weather gets warmer, toes are coming out of hibernation. David Greenberg, a doctor of podiatric medicine, says he doesn’t recommend specific brands because everybody’s feet are different. He says you’re better to experiment and try on lots of different shoes.

As a rule, though, flip-flops should be avoided if you’re doing a lot of walking, he says. “They’re called flip-flops for a reason. People flip and flop,” he says.

MI Podiatrist Loses on Cell Phones, But Wins on Conflicts

Farmington School Board members during their Tuesday meeting at North Farmington High School approved a Code of Ethics for the board, but the vote was not unanimous. Dr. Murray Kahn, who is new to the board this year, said he voted against the 16-point policy because of a new section regarding use of cell phones. He initially brought up concerns about the proposed policy during a May 8 study session.

At that time, Kahn explained that he occasionally needs to access his cell phone during meetings because of his position as division head of podiatry with Henry Ford Health System. He explained that his phone has a security code that requires him to type in numbers in order to access his messages. A section of the code that did change deals with conflicts of interest, board member Frank Reid said Tuesday. Kahn suggested the change, which requires board members to disclose any "actual or potential" conflicts of interest and to refrain from voting on issues in order to "avoid even the appearance of impropriety."

Podiatrists Among the Lowest in Divorce

Looking for a happy-ever-after partner, one who is willing to put in the hard yards for the long haul? Forget searching in bars or online; you should head to your local dentist, podiatrist, or optometrist instead.

According to research that correlated occupations with divorce and separation rates published in the Journal of Police and Criminal Psychology, optometrists were found to be among the most loyal of spouses with just four percent separating or divorcing, which is only a little less than the marital breakdown rate for podiatrists (6.8 percent) and dentists (7.75 percent).

Challenges to Peer Review Confidentiality Rising

Physicians historically have enjoyed state protections when discussing a colleague’s behavior or analyzing an adverse event in peer review committees. At least 45 states prevent disclosure of what is said during such meetings to facilitate open communication and foster better healthcare. But a recent rise in legal challenges against peer review protections is putting doctors’ confidentiality — and the process itself — at risk, legal experts and physicians say.

Courts that have allowed peer review files to be discoverable seem to follow one of two lines of reasoning. In some rulings, judges have cited the disclosure as appropriate because of conflicting state laws or overriding federal regulations. In other decisions, courts have said the documents are not privileged because of how or why they were created.

QUERIES (CLINICAL)

Query: Tuberous Sclerosis

A dermatologist referred a 25 y/o patient with tuberous sclerosis (TS) (mild genetic expression) and associated periungual fibromas (Koenen’s tumors). The periungual lesions have been trouble for him with regard to bleeding, and his nails have some secondary deformity as well as onychomycosis. A decision was made to surgically remove the lesions which will include involved matrix tissue, and treat the fungal infection topically (due to potential drug/drug interaction).

The literature includes several techniques of tumor removal: sharp dissection of the entire tumor with borders, curette and phenol, application of silver nitrate, cryosurgery, and CO2 laser ablation. I am inclined more toward surgical excision or CO2 laser. I am open to any ideas from those with more experience in treating this condition.

Fellow DPMs Beware: As a biomechanics watchdog for our profession, I must warn you about the recent lauding of Foot Solutions by Cam White, CPed that avoids documenting a major conflict of interest as he is the national sales manager for The Novascarpa Group that has a partnership agreement with, you guessed it, Foot Solutions.
 
There is also a YouTube video that Mr. White taped at a Foot Solutions store promoting his Chung Shi Shoes, the ones mentioned in the less than professional ad posted yesterday on this site distributed by, you guessed it, NovaScarpa.
 
Cam’s posting, without any disclaimer of his economic ties with Foot Solutions should be counted as further evidence of unethical behavior that is rooted at Foot Solutions and its partners.

My dear friend Dr. Soave accuses me of an injustice regarding Vision 2015 by my stating that the profession has chosen to imitate orthopedic foot and ankle surgeons. Unfortunately, he interpreted and restated my remark as "imitating foot and ankle surgeons."

This, of course, is NOT what I meant at all. I am well aware that podiatric surgeons are indeed prominent in the art of foot and ankle surgery. I applaud and encourage all who so desire to become the best foot and ankle surgeons they can become. But my original comments were based on the indisputable fact that our schools lure in students by telling them that...

RE: OH Podiatrist Discusses Heel Fissures

The Podiatrists in the News section on 5-29-12 highlighted Dr. Gregory Blasko's explanation of the serious complication of fissured and xerotic heels, especially in populations such as diabetics, patients with PVD and neuropathy, etc.

CMS, in its questionable wisdom, stopped paying for topical urea products through the Medicare Part D drug program, and many commercial drug plans have followed suit. There is now a 42% urea product - Kera 42, available from CTS (Clinical Therapeutic Solutions). This is a new supplier of in-office podiatrist-dispensed only products. In the case of Kera -42, you can make available to your patients a great product at a very affordable price.

I would like to thank Dr. Sharit for her diligent Internet research. The aluminum-Alzheimers theory was indeed a popular theory at one time, and was based on scientific research. This research has been subsequently "explained" as a theory and not an actual cause and effect relationship. My Alzheimer's disease info was apparently not updated.

Interestingly, the following quote can be found on the Mayo Clinic website: "Aluminum-containing antacids should not be used by elderly persons with bone problems or with Alzheimer's disease. The aluminum may cause their condition to get worse." If indeed the theory was completely debunked, why make such a statement?
