PODIATRISTS AND DIABETES
Cushioning is a Critical Component for Diabetic Shoes: NC Podiatrist

Since many people with diabetes don’t have a lot of feeling in their feet, they may often be tolerating more pressure than they should, which can lead to ulcers. If they choose a shoe with a breadth of cushioning, the pressure is better distributed across the entirety of the foot.

Dr. Elizabeth Daughtry

“Cushioning is an important characteristic for diabetic patients who may have neuropathy or lack of feeling in the feet from nerve damage,” Elizabeth Daughtry, DPM  explains. “This problem becomes an issue, as the protective sensation is lost in the feet, putting the skin at risk of developing pressure, friction sores, or blisters that could progress to nonhealing ulcers or infection. Lack of protective sensation can lead to a simple callus getting too thick over a bony prominence, leading to skin breakdown from additional pressure or shearing force from the function of gait.”

PODIATRISTS AND FOOTWEAR
MD Podiatrist Discusses How to Choose Sandals

Priya Parthasarathy, DPM, a podiatrist at U.S. Foot & Ankle Specialists, explained that wearing the wrong sandals can cause serious problems, including Achilles tendonitis, plantar fasciitis, pain in the big toe and even stress fractures. Finding the right sandal can prevent foot pain in the long run. She told HuffPost that she sees the most people coming into her office in late August and September, which she calls “plantar faciitis season.” It’s the result of wearing unsupportive sandals, usually flexible flip-flops, all summer long. It’s imperative to have shoes that are supportive during the active summer season.

Dr. Priya Parthasarathy

Avoiding soft, flimsy sandals is key, Dr. Parthasarathy noted, even if they feel comfortable: “A completely flat sandal is equally as bad as a high heel. Another red flag is a sandal that you can bend in half, or one that is not wide enough for your feet. The width of the sandal is important to accommodate your foot and any ailments such as bunions, neuromas and hammertoes.” Unfortunately, there are certain people who should avoid sandals in general. If you are actively treating a foot injury, tendinitis or a fracture, then now is not the time to be slipping on a pair. If you aren’t sure, Parthasarathy recommended speaking to your podiatrist.

PODIATRISTS IN SPORTS
MI Podiatrist's Bowling Team Wins B’nai B’rith Bowling League Title

Andy Rosenfeld says his team has the most fun of any team in the Brotherhood-Eddie Jacobson B’nai B’rith Bowling League. That proclamation may be up for debate among the weekly league’s 23 other teams. But no one can debate that Rosenfeld’s team, named Uncle Miltie in honor the late Jewish actor and comedian Milton Berle, couldn’t be stopped this season and won the league championship.

Dr. Eric Foreman (second from right)

The team includes Eric Foreman, DPM, who averaged 140 this season, lives in West Bloomfield, and is a podiatrist. His team beat the defending champion 600 club which had a combined regular season average of nearly 208!

HEALTHCARE NEWS
Hospitals Are Refusing to Do Surgeries Unless You Pay in Full First

For years, hospitals and surgery centers waited to perform procedures before sending bills to patients. That often left them chasing after patients for payment, repeatedly sending invoices and enlisting debt collectors. Now, more hospitals and surgery centers are demanding patients pay in advance. Advance billing helps the facilities avoid hounding patients to settle up. Yet it is distressing patients who must come up with thousands of dollars while struggling with serious conditions.

Those who can’t come up with the sums have been forced to put off procedures. Some who paid up discovered later they were overcharged, then had to fight for refunds. Among the procedures that hospitals and surgery centers are seeking pre-payments for are knee replacements, CT scans, and births.

PRACTICE MANAGEMENT TIP OF THE DAY
7 Ways Automated Coding Can Help Capture Reimbursement

Capturing correct reimbursement is crucial for any physician’s practice. Unfortunately, coding staff shortages, chart backlogs, and guideline changes often lead to denied claims and revenue leakage. While a myriad of solutions have been developed, only coding automation can truly solve the problem. Here are seven ways this AI technology improves revenue capture. But first, it’s important to understand the core reason that coding is a weight on many practices today.

6. Adapts to guideline changes

With new guideline changes in effect this year, your coding team may still be adapting. AI ensures that none of the common 2023 E/M errors and changes slip through the cracks. For example, according to the new guidelines, the effort it takes a physician to consider multiple diagnoses, treatments, and procedures should be accounted for in the E/M code. As AI technology instantly incorporates guideline changes, it will catch this update to ensure you’re properly reimbursed.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Ivar Roth, DPM, MPH

After reading Dr. Kornfeld’s response, I can again add some context to why my income dropped for such a long time. As a rare third-year residency-trained podiatrist back when I went full direct pay, my practice was mostly surgical. When patients called, they were not ready to pay out-of-pocket for surgery, and so my income took a major hit. The good thing was I had saved up for this transition. I agree with Dr. Kornfeld that the transition, if properly planned, can be shorter but as the original direct pay podiatrist, I had no guidance on how to make the transition.

I do remember calling Dr. Kornfeld and asking him how he was doing, and I remember that he was off private insurance companies but still taking Medicare, so that may be a way to transition; but I had decided to go for it and delisted/ opted out from Medicare completely. I still maintain that this type of practice is only cut out for those relatively few podiatrists who have the full spectrum of skills required to go direct pay and have the full love of our profession at heart. This type of practice requires diligence and hard work above and beyond the typical practice.
