GA Podiatrist Recommends Shoes with Good Traction for Safety 
 
According to the National Institute on Aging (NIA), foot problems are more common in older adults, and these can lead to reduced mobility, increased frailty, and a higher incidence of injury. The good news? Gregory Alvarez, DPM, a podiatrist at the Ankle & Foot Centers of America, says there are simple ways to make your shoes safer if you're over the age of 60.

Aussie Podiatrists Will Start Clinical Trial of Inhalable Anti-Anxiety Drug
 
Podiatrists at Noarlunga Hospital will trial a new model of care for anxious patients, including children, to help them get rapid access to some minor surgical procedures. The study is an Australian first, which allows public podiatry patients who experience needle-phobia or anxiety to access inhalable medicine, methoxyflurane, known as the ‘green whistle’ to enable the pain-free removal of an ingrown toenail.
 
Methoxyflurane has been available for use in private podiatry practice since 2010 and is now on the list of approved SA Health medicines for use for podiatry outpatients at Southern Adelaide Local Health Network (SALHN). Previously, SALHN patients experiencing needle phobia or anxiety about the procedure would be required to have the procedure performed in the operating room under general anaesthetic. This can result in increased use of healthcare resources and longer waiting times.

Tips for Easing Patient Collections in Your Practice
 
Starting in 2023, some credit-reporting firms will remove unpaid medical debt of less than $500. Asking patients to pay their medical bill may become more difficult when there are fewer financial consequences. Here are some strategies to ease the burden of patient collections and improve cashflow and revenue:
 
5. Use caution with collection agencies. Collection agencies are always an option, but they should be a last resort, advises Krisi Hutson, senior director of product development at Availity. “If you’re going to spend extra dollars, do it on pre-service collection,” she emphasizes. “With collections, you’re going to spend a minimum of 35% to 45% the minute you send it out the door. If you take that money that you would lose on collections and invest in one or two financial counselors, over the course of a few years you’re going to break even — and you’re also going to improve your patient satisfaction scores.”

Regarding Dr. Rosenblatt's post, "Pot May be a Boon to Podiatry," he states some reasons for a low recommendation rate for podiatry as a career choice based on "decreased reimbursements and competition from other specialties, especially nursing." He then cites a study recently presented at the Society for Cardiovascular Angiography & Interventions concluding that "Marijuana users are at a markedly increased risk of being diagnosed with PAD." He then appears to claim that the increasing trend in marijuana use and its presumed associated increase in the incidence of PAD will counter the ostensible low recommendation rate for a podiatry career.
 
How does more PAD equate to increased reimbursements and less competition from other specialties, especially nursing? This appears to be a non sequitur. It then seems that Dr. Rosenblatt goes on to reduce the strength of his own argument, stating the increased incidence of diabetes, obesity, and the increased size of the aging population as other trends favorable to podiatry as a career choice, however these trends have largely occurred over the past two decades while the ostensible negative podiatry recommendation trend has occurred. Am I missing something? Please note, my questions are not meant to be interpreted against podiatry as a career choice.

My experience with daily pot users has been just the opposite. This is an educational pearl for young podiatrists. If you have a patient in their forties or fifties with type 2 diabetes and neuropathy that has good skin texture, normal hair distribution, palpable pulses, and no history of chemo treatment, then this is diagnostic of a daily pot user. You don't even have to ask them, you can tell them.
 
Daily pot use causes neuropathy, impotence, and diabetes. If they keep using it, they most likely will die from kidney failure in their sixties. This has been my extensive experience with pot users. If you have a patient who has complete neuropathy, good skin, good pulses, and no chemo and they deny using pot, they are lying.

CMS attempts to facilitate access for therapeutic shoes to patients who only see PAs or an NPs has been a flop. It also has confused the auditors at every level, created a larger paperwork trail. Even my fellow DME council members continue to flood my phone and email box just having reviewed this issue during a council meeting. So to try to give a straight answer covering every scenario in this format would be nearly impossible. This is what a PA can do:

PAs and NPs are permitted to do foot exams, documenting qualifying risk factors for therapeutic shoes and to complete a Statement of Therapeutic Necessity. Medicare still requires an MD/DO to be the Certifying Physician. It is okay if they provide "general supervision (versus direct)" of the NP/PA.
 
It is still necessary for the certifying physician to co-sign the certifying statement and foot exam. Often, this is a challenge to execute. It is NOT okay to get the NP/PA signatures without a MD/DO co-signing it.
