NYCPM’s 108th Graduation Celebrates Class of 2019
At the 108th commencement of NYCPM, 93 members of the Class of 2019 were hooded and proudly accepted their diplomas, signed the Book of Physicians, and took the Hippocratic Oath. The class achieved 100% pass rate on Part II of the APMLE Boards and 100% residency matches in the first round. Families and friends in the audience cheered this high-achieving class. NYSPMA President Paul Liswood, DPM, bestowed greetings and congratulations from the association. Keynote speaker and honoree New York State Assemblymember Robert J. Rodriguez reminded the students of their important role in helping diabetics maintain foot health as they manage their disease.

Honoree Alina Moran, CEO of NYC Health + Hospitals/Metropolitan, said she hoped that the graduates would carry the values of “integrity, compassion, accountability, respect, and excellence” with them as they enter the next phase of their medical careers. Both Ms. Moran and Mr. Rodriguez received the honorary degree, Doctor of Humane Letters, Honoris Causa, at the ceremony. Among the graduates, Michelle A. Diaz, DPM, MPH had participated in the dually sponsored DPM-MPH degree program with the Icahn School of Medicine at Mount Sinai. Three graduating PGY III residents received their certificates.

Western University of Health Sciences’ College of Podiatric Medicine awarded diplomas to 34 graduates at its Commencement ceremony May 24, 2019 in Pasadena, CA. CPM Assistant Professor of Podiatric Medicine, Surgery and Biomechanics Adrienne M. Estes, DPM ’13, MBS, a member of CPM’s first graduating class, gave the WesternU Alumni Association welcome.

College of Podiatric Medicine Professor of Podiatric Medicine, Surgery and Biomechanics Emeritus Lester J. Jones, DPM, MSEd, who served as interim dean of the college for nearly two years and retired in April 2019, administered the Podiatric Physician’s Oath and reminded the graduates to listen to their patients, think with their heads, act with their hearts, and use their hands to help others.

How to Renegotiate Payer Contracts - Part 3
If you feel stuck in a lousy deal with your payers, you can renegotiate. Here are some steps to renegotiate your contracts and improve your revenue.
3. Let the games begin - Once you have assembled all of your data, you can compare each payer to one another. Now you have the information you need to begin the process of renegotiating your contract with a specific payer. Initiate the process according to the terms of your agreement but do so with a formal notice. Pick a payer that you think you’ll be able to get the desired changes from. It might be based upon the fee schedule, the time to the end of the contract, or the volume of business you do with them. I recommend starting with payers that offer the lowest rates and those with the least amount of time left on the contract. Research who you should send the notice to and ask for confirmation of receipt. Send your notice to as many people at the payer’s organization as possible.

In the discussion about current residents preferring hospital based employment versus private practice, versus group practice, the following comment is often stated: "Hospital-employed physician medical care is certainly driving up healthcare costs." Of course the decision is largely based on personal and economic realities where anyone ends up practicing. Although very costly, starting a private solo practice is becoming the least desirable, but it is my belief that the emerging healthcare changes will make some solo practices golden and in extremely high demand.

Dystrophic nail changes may be secondary to repetitive micro-trauma. Mallet toe and hallux hyperextension deformity are common examples of such etiologic factors. Fungal infection may occur as a comorbidity or without associated trauma. Dermatopathology laboratory testing can, in many cases, establish the fungal and or traumatic etiology and therefore suggest treatment options. I would suggest that such testing would be considered prior to EHL tenotomy for "spoon toe". Perhaps dermoscopy or other studies in the future will prove helpful.
Additionally, whether performed distal to the extensor hood or not, the development of flexion deformity of the hallux IPJ and hallux hammertoe are possible over the long-term. It seems appropriate to provide long-term clinical outcomes showing the absence of such iatrogenic deformity. Ultrasound, MRI, or other studies demonstrating re-establishment of the continuity and function of the EHL would also be appropriate. Long-term study of the eventual result with reference to the toenail appearance, texture, associated pain, etc. would also be helpful.
Dr. Katzen correctly notes that treatment of onychomycosis with laser therapy or the use of oral antifungals is not typically appropriate absent confirmatory testing. Furthermore, as noted by Dr. Katzen, any traumatically induced contribution to the observed nail dystrophy should be recognized and appropriate intervention offered to the patient. The induction period for iatrogenic deformity is not always immediate. Short-term success will not guarantee long-term success.

Clinical practice guidelines can also be used to determine what is considered appropriate covered services. In 2000, the federal government's expert referenced the "Practice Enhancement Manual for the Podiatric Physicians" in its case against me. Included with this was the so-called Global Surgical Task Force Report (1996-99), which was a report from the APMA. The expert did not directly refer to the AMA's CPT ® code book. I did not find this out until the trial against me.
Thus, the authoritative source was not the AMA's code book. Instead, it was opinions from these other resources that went back to the jury room. Despite the jury request for the AMA CPT code book, it did not go back to the jury room. In short, what the jury saw was a paid for (commercial) report, with advertising and sponsors, which was presented to the court and grand jury instead of the authoritative source, the AMA's code book.
