AT THE COLLEGES |
NYCPM’s 107th Graduation Celebrates High-Achieving Class of 2018
At the 107th commencement of NYCPM, 85 members of the Class of 2018 proudly accepted their diplomas, signed the Book of Physicians, took the Hippocratic Oath en masse and marched out of Alice Tully Hall as new doctors of podiatric medicine. The class that had achieved 100% pass rate on Part II of the APMLE Boards and 100% residency matches in the first round smiled broadly throughout the proceedings; keynote speaker and deputy board chairman Rock G. Positano, DPM (’88), MSc, MPH, DSc, called the class “the best trained, highly educated class at the College.”

NYCPM 2018 graduates recite the Hippocratic Oath |
Both Dr. Positano and Bijan Safai, MD, DSc, recipient of NYCPM’s honorary Doctor of Science degree, Honoris Causa, made the point that the graduates have been and still are supported by networks of their parents and friends; “you have not done this alone,” Dr. Safai told them. After the Class of 2018 had received their diplomas, three graduating PGY III residents received their certificates, as did the five graduates of the dually sponsored DPM-MPH degrees between NYCPM and the Icahn School of Medicine at Mount Sinai.

PRACTICE MANAGEMENT TIP OF THE DAY |
Staff Roles Practices Should Consider Adding - Part 4
Whether it be clinical, financial, or technical, today’s physician practices have seen many changes. These changes have resulted in adding new roles within the practices as well as expanding roles for some employees already on staff.
4. Medical Scribes - “Medical scribes are the fastest growing medical profession in the United States. The scribe is a way to untether the provider from the EHR so the physician can concentrate on the patient. Due to the current variance in scribe training and frequent lack of specific certifications, physician practices should establish clear guidelines on how scribes are to deal with a myriad of likely scenarios to promote safe, effective use of the EHR.
Recently, The Joint Commission stated that at a minimum, all scribe-generated orders must be signed by a provider prior to implementation and that organizations must document the competency of scribes for the functions the organization deems appropriate.” - Jeffrey A. Gold, MD, professor of medicine and director of simulation at Oregon Health and Science University, Portland, OR

CODINGLINE CORNER |
Query: Federal BCBS Initial Consult Code
What is the correct code that should be billed for an in-patient, initial consult? Federal BCBS is telling me we cannot bill a CPT 99221-99223 and we need to bill CPT 99251-99255 or CPT 99231-99233.
Charles Chapel, DPM, Brooksville, FL
Response: That is correct…all consulting CPT codes were discontinued a number of years ago.
Joseph Borreggine, DPM, Charleston, IL

QUERIES (MEDICAL/LEGAL) |
Query: Potential Kickback Scheme
A rep came to my office asking me to start prescribing her company's compression devices after surgery for DVT prevention. She says the company will pay me $$$ for each prescribed device delivered to the patient. They even have a “legal contract.” But...it strikes me as a possible kickback violation (not to mention paying doctors for prescriptions seems like a huge conflict of interest for the doctor). I am sure you can’t do that with Medicare or Medicaid (as it is a Stark Law violation), but I can’t find definitive answers for private carriers. I could be wrong, but it smells a bit fishy. Is this kind of thing legal? I am sure I am not the only podiatrist that has come across this kind of situation.

QUERIES (NON-CLINICAL) |
RE: Is BNA Burz Still in Business?
From: Jerome E. Reeves, DPM
I brought some bur machines from BNA Burz, and have one unit in need of repair. I was unable to reach this company via telephone or email addresses. If anyone can tell me if this company has gone out of business, or how to contact them, it would be greatly appreciated.
Jerome E. Reeves, DPM, Hollis, NY
Editor's Note: It appears that BNA Burz is no longer in business. If any PM News subscriber has an alternate source for repairing these machines, please post that info on this site.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS) |
From: Joel Lang, DPM
It is interesting that the current poll concerns Saturday hours. When I first started out, I did have hours on Saturday morning 9 AM to 1 PM. I think this is helpful for any new practitioner with lots of debts and bills and not so many patients.
However, as my practice progressed, I gradually cut down on Saturdays. After a while, I worked only every other Saturday. Then the following year, I worked only one Saturday a month. The following year, I cut out Saturday hours completely. I think this is a good plan for any new practitioner in private practice as his/her Saturday patients gradually become adjusted to the new scheduling.
The best advice I got going into private practice was to "Practice on your first day the way you would like to be practicing 20 years from now, because it is hard to change once practice habits are formed."
Joel Lang, DPM (retired), Cheverly, MD
