PODIATRISTS AND HUMANITARIAN EFFORTS
PA Podiatrists Return from Medical Mission in Honduras
 
Drs. Neal Kramer and Lee J. Sanders recently returned from a medical mission to the Manos Amigas clinic, La Entrada, Honduras. The mission was sponsored by Serving at the Crossroads (Media, PA) and First Presbyterian Church of Allentown, PA. The medical and dental brigade consisted of two podiatrists, one ER physician, one dentist, two nurse practitioners, three RNs, and nine non-medical volunteers. They supported the clinic’s full-time medical staff of two physicians, including Dr. Alex Garcia.

The unique focus of this mission was diabetes, diabetic foot disease and wound care. Medical and surgical care was provided for a diverse population of patients with acute, chronic, developmental, and parasitic disorders. Prevalence of diabetes in Honduras is 7.2%, with 53% of the patients undiagnosed until they present with complications. Both podiatrists reported that the experience was memorable and incredibly rewarding.

Canadian Podiatrist Returns to Live and Practice in BC
 
A decade ago, the site of the Central Green residential development was an ugly hodge podge of old school buildings a few blocks from Kelowna’s downtown core. Today, it is set to become the Okanagan city’s first master-planned urban community constructed according to one of the world’s most popular green building certification programs. A major attraction for Dr. Neil Brown, who recently returned to Canada from the U.S. after practicing podiatry in San Francisco and Washington, D.C. for several years, is having the large city park and easy access to recreation.

“To be on a piece of real estate that has a lot of outdoor space available for walking and exercise is really appealing,” said Brown. “Kelowna is coming of age and you still feel you are connected to the major cities, but you can do all the recreational activities like golfing and skiing and hiking without having to drive a half an hour to get there.”

Steven Latter, DPM: When capturing casting for orthotics, should you bill a casting fee? 
 
Harry Goldsmith, DPM: If the patient’s insurance policy includes a custom foot orthotic benefit, I certainly would recommend billing for orthotic impression casting. Unlike Medicare that bundles the evaluation for AFOs, therapeutic shoes, and inserts with any casting, manufacturing, fitting, and dispensing performed by the supplier, commercial payers vary in their payment policies. While non-Medicare payers typically follow this reimbursement policy for orthotists, it is not unusual for the same commercial payers to separately pay for impression casting and the supply (custom foot orthotics) to DPMs (or MDs/DOs). 
 
In my experience as a claims reviewer, the differences between reimbursement to the orthotist and the podiatrist (for example) for the same devices is...

Email Does Not Toll the Statute of Limitations for Malpractice: NY Court 
 
In Caesar v. Brookman, 800004/15, NYLJ 120275458584903, at 1 (Sup. Ct. New York County March 30, 2016), Justice Alice Schlesinger was faced with the sole question of whether advice contained within emails constituted "continuous treatment" as applicable to the statute of limitations in podiatric malpractice actions. She answered this question in the affirmative.
 
Plaintiff presented to defendant, a podiatrist, with a possible foreign body in his right heel; defendant found a shard of glass in the heel which he removed, and then cleansed and dressed the resulting wound. Two days later, plaintiff sent defendant an email detailing various symptoms he experienced since his office visit. Defendant responded by email that same day advising that "plaintiff could leave the wound uncovered, it was unlikely his fever and chills were from the wound, and recommended he take antibiotics." Two days later, plaintiff was admitted to the hospital with an MRSA infection. Justice Schlesinger's holding in Caesar clearly conveys to physicians aiming to decongest their waiting rooms by treating patients in cyberspace that there will be consequences of doing so.

Children With No Shoes On 'Do Better in Classroom', Major Study Finds
 
Researchers at the University of Bournemouth found that pupils who leave their shoes outside the classroom are more likely to arrive to school earlier, leave later, and read more widely – ultimately resulting in better academic achievement overall. From observing thousands of children from 25 countries over ten years, academics say they want to encourage the practice to be adopted in UK primary and secondary schools
 
Experts believe having children remove their shoes for the classroom improves their learning because it makes them relax and “feel at home”, and have called on teachers in the UK to apply similar “shoeless” policies “to give children the best possible chance of performing in their exams.” Professor Stephen Heppell, who led the research with the Centre for Excellence in Media Practice at Bournemouth University, said: “Children are much more willing to sit on the floor and relax if they have no shoes on.”

RE: The Passing of Norman Klombers, DPM
 
It is with regret and sadness that I learned that Dr. Norman Klombers passed away on May 26, 2016 in Virginia. Dr. Klombers was a graduate of the New York College of Podiatric Medicine in 1944, served for a time on the NYCPM Faculty, and became Assistant Director for the New York Podiatric Medical Association. He then became Director of Scientific Affairs for APMA in 1978. In 1980, he was selected as Executive Director of APMA after the death of Dr. Nyman, a post he held until his retirement. He led the transition of the Association's headquarters in Washington, DC to Bethesda. MD.
 
Dr. Klombers had a wealth of clinical knowledge that significantly enhanced his position. During his tenure, Health Policy was initiated, advocacy enhanced, governmental relations expanded, as well as expanding the scope of practice. He led by example. He was knowledgeable, professional, and maintained the highest level of integrity.
 
On a personal side, we remained in close contact, speaking weekly, and I have lost a friend. His record is a part of the history of APMA and the profession. Rest in peace, my friend, and know that the world is a better place because you were here.
