Reasoning Behind Vibram Shoes is Faulty: PA Podiatrist
 
The idea behind FiveFingers was that they'd allow for a natural barefoot running style, with a rubber sole for a bit of protection against sticks, stones, and the like. But a 2012 lawsuit filed by a Florida woman claimed Vibram promised health benefits without scientific proof, including that the shoe would improve posture, strength, and balance. Now, Vibram has agreed to settle the class-action lawsuit for $3.75 million, but doesn't admit wrongdoing. It's still selling the shoes.

Dr. Maria Kasper, a podiatrist at Martin Foot and Ankle, said she has seen patients with overuse issues from FiveFingers, mainly things like metatarsal stress fractures and tendonitis. The reasoning behind the Vibrams — running "naturally," the way nature intended, instead of shoe-bound — is not unreasonable, she said. "The problem is, that reasoning falls in line with a Kenyan running," instead of someone who has been in shoes his or her whole life, she said.

Over Half of Prescriptions Go Online for First Time in 2013
 
For the first time, the majority of U.S. prescriptions — not including those for controlled substances — were routed electronically to pharmacies through Surescripts in 2013, according to the company's National Progress Report on health information technology (IT). That's more than one billion prescriptions, or 58% of all non-controlled substance, eligible prescriptions written last year.
 
The total volume of prescriptions routed online increased 32% to 1.04 billion in 2013 from 788 million in 2012, according to Surescripts. By comparison, the growth rate in the number of e-prescribers was much lower. Seventy-three percent of office-based physicians prescribed electronically last year versus 69% in 2012. In previous years, adoption increased much more rapidly.

Integrated Cuff Testing and Alarms Make Tourniquets Safer 
 
You'll find these features in some of today's most modern tourniquet systems. It lets you test cuffs, tubing, and connectors for potentially hazardous leakage. By pressing a single button in the system, the cuff will inflate by itself and perform a test to see if there is a leak. The system will find even a pinhole leak that's not easy for the naked eye to detect. This technology can be used during surgery, as well as before and after procedures to let you know whether the cuff is working or will work properly. While a slow leak itself isn't likely to harm patients, it can indicate a more serious potential failure of the cuff, which would cause blood to suddenly enter the surgical field and create challenges for the operative team.
 
It's also extremely important for a cuff to depressurize entirely when deflation is intended. But this doesn't always happen. To ensure cuff depressurization, some tourniquet instruments now include an alarm that sounds when the technology detects failure, informing staff of the need to investigate and remove the cuff.

Query: Diabetic Shoe Coverage
 
I just finished speaking with BCBS, Anthem Federal Employee program. I was calling on behalf of my patient. He called his insurance company today to find out about coverage for his shoes. After he pays his deductible, he is covered for A5512, the diabetic liners, but not for the shoes, A5500. So, I called myself and got the same answer. When I asked the person I was speaking to, what he was supposed to put the liners in, she laughed. Has anyone else heard of such madness?

How do I code the procedures all done on the left foot/ankle?: 
 
- Excision of an exostosis located across the dorsal aspect of the talar neck; 
- Removal of ossicle, anterior aspect of the ankle; and 
- Excision of exostosis, anterior distal fibula

Response: These procedures that were performed are all related to peri-articular structures of the ankle joint. I recommend one code, CPT 27700, arthroplasty, ankle joint as the best coding that would be inclusive of all three procedures that were done. I don't think it would be reasonable to unbundle these into three separate codes for each anatomical location.

I had a very similar case to this, involving a Down patient. The only difference was that his IM was almost 45° with a short metatarsal, and made anything but custom shoes impossible and still a trip hazard. I corrected this to an 8º IM with a normal parabola via a Fowler procedure with an Akin. He was compliant enough to heal everything and have the other side done some months later. As he was told by several other people in town that "nothing could be done," the mother was very happy. The patient had a naturally sunny disposition and I'm not sure of his satisfaction level. Being able to wear OTC shoes didn't hurt either.

Procedural choice should consider post-op ability to comply. As far as what surgery to do—you must identify the etiology. Was this congenital or acquired? If acquired—why? What is the biomechanical reason? Then address the biomechanical/structural abnormality with a surgical solution. This young man needs a procedural choice which will give him long-term stability.
 
Regarding post-op management: Having had similar cases in the past, my advice would be to have a heart-to-heart with the parents and impress upon them the need to comply. Include that as an extra risk in the consent; the goal being to protect the patient from himself. Use locking plates and screws. Put him in an ABOVE KNEE cast and have the patient use a wheelchair. DO NOT USE CRUTCHES, if you can avoid it.
