That Dark Spot on Your Foot May Be Deadly: MI Podiatrist
 
According to Conway McLean, DPM, "If you are having pain from a new spot on the foot, don’t assume it is benign and will disappear on its own. If it is callusing, this will most likely be a progressive problem as the skin learns to make callus better over time. As with many medical conditions, lesions on the bottom of the foot get harder to treat as time passes by. Treatment starts with an accurate diagnosis, typically aided by consulting a specialist who will be acquainted with the optimal therapies."

PA Podiatrist Discusses When to Replace Workout Shoes
 
Depending on the shoes you're wearing, with each step in your favorite workout, you'll either feel a cloud of comfort — or discomfort. This begs the question: Is it time for new workout shoes? The usual lifespan for a running or walking shoe is about 300 to 500 miles, according to Alicia Canzanese, DPM, Vice president of the American Academy of Podiatric Sports Medicine.

Dr. Canzanese says for gym or "court-sport" sneakers such as pickleball shoes, tennis shoes, or footwear for basketball, the gauge is different. "The general rule of thumb is 40 to 60 hours of playing a sport in the shoe." That means if you're playing in those shoes more than a couple of times a month, the answer to how often you should replace your shoes is around every 9 to 12 months, she says.

Tips for Easing Patient Collections in Your Practice
 
Starting in 2023, some credit-reporting firms will remove unpaid medical debt of less than $500. Asking patients to pay their medical bill may become more difficult when there are fewer financial consequences. Here are some strategies to ease the burden of patient collections and improve cashflow and revenue:
 
4. Create a written financial policy (or update your existing one) to include language about up-front payments. Let patients know that full or partial payment is due at the time of service. Provide the policy to patients at check-in and ask them to sign it, says Rozmin Bapat, president of CodeRite Healthcare Consulting. Front staff and physicians themselves should also be prepared to answer questions, such as “Why has the policy changed?” or “What if I’m not prepared to pay today?”
 
For example, in terms of why the policy has changed, Bapat suggests the following response: “Patients are contractually bound to pay this amount when they sign up for insurance coverage, and now our practice is enforcing it on the front end. We’re asking you to honor it.”

Query: Arthroplasty of a Toe
 
In many cases requiring and arthroplasty of the proximal interphalangeal joint (PIPJ) 5th digit (especially when associated with a heloma molle), I often do an arthroplasty PIPJ, flexor lengthening and a partial syndactylization (advancing the webspace but differently than the description of Ruiz-Mora). I have always simply billed for an arthroplasty PIPJ 5th toe. Now that I have been forced to pay attention to coding because of my EMR program, should I have been billing this differently for all these years?

Response: The arthroplasty is coded using CPT 28285 which is defined as the following: Correction, hammertoe (e.g. interphalangeal fusion, partial or total phalangectomy).
 
The flexor tendon lengthening is coded using CPT 28232 which is defined as the following: Tenotomy, open, tendon flexor; toe, single tendon (separate procedure).
 
The partial syndactylization, advancing the webspace is best coded using CPT 14040 which is defined as the following: Adjacent tissue transfer or rearrangement, feet; defect 10 sq. cm or less.
 
The first step that needs to be taken is to access the NCCI edits. CPT 28232 and CPT 14040 are both Column 2 codes to CPT 28285 the Column 1 code within the NCCI edits. Therefore, based upon the posted scenario, neither CPT 28232 or CPT 14040 are separately reimbursable when billed in conjunction with CPT 28285. CPT 28286 is not an option. It is defined as the following: Correction, cock-up fifth toe, with plastic skin closure (e.g. Ruiz-Mora type procedure). Clearly, this was not the pathology that needed to be corrected, nor was it the procedure that was actually performed. The only option for the correct coding for the surgical correction that was performed on the 5th toe is the following: CPT 28285 – T4/T9.

I believe I have had requests from Episource in the past. They never paid my invoice and were never given charts. Name Withheld said they approved payment, but the big question is whether they actually paid. There have been many times that these companies agree to pay invoices but never send payment. I cannot imagine how much money 66 charts will bring in from Medicare if Episource gets those charts! Even if they pay Name Withheld the $3,300, which I highly doubt, Episource will clean up!

Would you still consider recommending podiatry as a  career? We have had a chorus of no’s in answer to that question, with decreased reimbursements and competition from other specialties, especially nursing. But there appears to be a hidden gem which is not being reported that could reverse that trend: Cannabis use and legalization. Some of the news is creeping out: “Marijuana Linked to Higher PAD Risk” Medscape reported a study of over 600,000 “chronic” users that points to a direct connection between PAD and pot. The study population is mainly under 37 years of age, when you might not expect pedal pulses to start diminishing. But we can all generally assume that marijuana use will increase as more states legalize it.
 
Podiatry concentrates on older people, which is not surprising since...
