ABPM NEWS
MT Podiatrist Named Interim Executive Director of ABPM

The American Board of Podiatric Medicine’s (ABPM) Board of Directors (BOD) is pleased to announce the appointment of the interim Executive Director position recently vacated by the resignation of James W. Stavosky, DPM after 25 years of service to the board. Gina M. Painter, DPM has  agreed to step into this role while the BOD launches a search for our new Executive Director. Dr. Painter has recently been appointed as the Medical Director at the Benjamin Charles Steele Clinic in Montana VA Health Care System in Billings, Montana.

Dr. Gina Painter

Dr. Painter served on the BOD from 2012- 2020 and was president of the board from 2016-2018. She has been involved in the examination process since 2003 and is currently the examination chair. William E. Chagares, DPM as immediate past president of the ABPM, has been selected to chair the ad hoc search committee for ABPM's new Executive Director.

RELEVANT RESEARCH
Utility of PCR for Assessment of Onychomycosis During Topical Therapy

A total of 28 patients with culture-proven, mild to moderate onychomycosis were treated topically with efinaconazole for 24 months. While culture/KOH and polymerase chain reaction (PCR) had high positive predictive values, PCR had a much higher sensitivity (91.3%) at completion of therapy than culture/KOH (43.5%) with respect to visual clearance. In addition, PCR was more sensitive for detection of non-dermatophyte molds; however, relevance of this needs further evaluation.

Findings from this study suggest that PCR should be considered for a more accurate prediction of treatment outcome of topical onychomycosis therapies.

PRACTICE MANAGEMENT TIP OF THE DAY
Digital Front Doors Improve Patient Health, Remove Barriers to Physicians

While the term front door to some means only a physical door—digital in this case—that is missing a much larger opportunity. Some digital front doors enable a more comprehensive solution. Combining the right technology, people and process allows for whole-person care to be delivered. For example, digital front door solutions that create readmission programs, health risk assessment/Gap closure programs, integrated after-hours telehealth solutions, and seamless connections between payers and providers are all ways that digital front doors can provide a more complete solution.

Some benefits include:

6. Enhances the patient’s experience as a whole

Digital front doors offer patients a space to experience independence, empowerment, and autonomy to access personalized care at their convenience. This emancipation from rigidity and structured agency, allows patients to be proactive in their care, they will be more likely to pay attention to potential warnings and give attention to potential health risks and hazards. Most important, early detection of possible health threats gives space for treatment and remedy before it can become a serious issue.

CODINGLINE CORNER
Query: Lisfranc ORIF

What would the proper surgical coding be for a Lisfranc ORIF?

Codingline Archive

Response: I assume you are doing an open repair of a dislocation since you have asked for surgical coding. The code that would best describe your intention is CPT 28555, open repair dislocation, with or without internal or external fixation.

If, however, you are performing this repair under anesthesia but closed technique, then use CPT 28546, percutaneous skeletal fixation of tarsal bone dislocation, with manipulation.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Thomas E. Silver, DPM

A practice valuation is essential for selling a practice. Like selling a home, the price has to be based on specifics and not just an arbitrary amount. A certified practice valuator can give you real numbers as a starting point for selling a practice. I have worked with Dr. Jon Hultman in California. He does an amazing job in valuing a practice and at a reasonable price for the amount of work he puts into it.

RESPONSES/COMMENTS (MEDICAL-LEGAL)
From: Paul Kesselman, DPM

I applaud all those who have posted their opinions, but alas, it appears there are two different issues being presented here. The first is whether or not podiatrists should be treating the systemic issues associated with gout. I don't believe that many orthopedists would do that and would likely refer the patient to their internist or other PCP for this. They are too damn busy fixing knees, hips, etc.

I offer this opinion not as one who is suggesting doing this will give us parity, but as one who has worked with a variety of orthopedists, who simply don't have the time nor do they feel it is within their skill set to start managing gout. I don't believe that any podiatrist out there should...

RESPONSES/COMMENTS (VA NEWS)
From: Allen Jacobs, DPM

I believe Lee Saunders, DPM was chief at Lebanon VA hospital many years ago, prior to any APMA or legislative fiat. There has never been an administrative inhibition to a podiatrist as COS. There have likely been others. Dr. Kittra Owens worked hard and earned this position on her accomplished merits.
