HI Podiatrist Wins Third Place in Start-up Competition
 
Start-up Weekend is an international movement inspiring business development in places like London, Berlin, Paris, New York, and Chicago, to name a few. But Maui? Why not! Start-up weekend was held May 16th to 18th at the Maui Research & Technology Park in Kihei. Kihei podiatrist Dr. Steven King and his team won third place with a plan to develop shoes and boots that reduce stress fractures.

King, a former Army officer, said he has been working on his advanced composite insoles idea for about eight years. His company, Kingetics, LLC, has received federal grants from the Department of Defense and the U.S. Army to develop the product.

Olga Luepschen, DPM, has joined Highlands Regional Wound Center (HRMC)a service of Highlands Regional Medical Center. Dr. Luepschen also practices at Gentle Foot Care Center in Sebring and is on staff at Highlands Regional Medical Center.

Podiatry Gets Vote as Medi-Cal Benefit in CA
 
In a floor vote yesterday, the Assembly unanimously agreed to make podiatric services a Medi-Cal benefit. Podiatry had been an optional Medi-Cal benefit until 2009 when it was cut because of budget constraints along with a number of other services, including optometry and adult dental services. Some of those services, including podiatry, were reinstated at rural clinics and rural federally qualified health centers after a court challenge, but most areas of California are not allowed to provide podiatry as a Medi-Cal benefit. That would change if AB 1868 by Assembly member Jimmy Gomez (D-Los Angeles) becomes law.
 
"This bill would reinstate full foot and ankle care by podiatrists in the Medi-Cal system," Gomez said on the Assembly floor yesterday. "Eliminating podiatric services from Medi-Cal has further exacerbated an already dire access problem for low-income and disabled communities."

Two Gloves Are Better Than One
 
Despite pages upon pages of clinical studies that have proven the effectiveness of double-gloving in protecting surgical team members from sharps injuries, needlesticks, and infectious diseases such as HIV, and hepatitis B and C, the practice isn't yet routine. Time and again we hear that clinical proof is needed to drive change in the operating room. Well, here it is. A survey published in the journal Infection Control and Hospital Epidemiology (tinyurl.com/pdjoesh) reveals that 99% of polled surgeons suffered at least one needlestick in their careers. The problem, note the researchers, is that surgical glove punctures often go unnoticed during cases, meaning surgeons might be exposed to blood and associated infection risks without knowing it.
 
The researchers say glove puncture rates vary, although risks increase to as high as 70% during longer procedures as well as during surgeries requiring maximum effort in deep cavities and around bones. They further note that research shows the risk of blood contact is decreased from 70% with single gloves to as low as 2% with double gloves, likely because the inner glove was shown to remain intact in up to 82% of cases.

Query: Bunion Surgery on a Down's Patient
 
The patient is an otherwise healthy and high-functioning 17 year-old young man with Down's Syndrome.  His chief complaint involves painful bilateral bunion deformities. Clinically, they are quite severe and his mother has found it difficult to find any shoes that fit and do not produce pain. There is a mild pes planus foot type.

I reviewed all of the standard conservative options. Surgery seems to be a reasonable choice, however, I question the patient's ability to be compliant with the necessary requirements for post-operative recovery - in this case, non-weight-bearing with cast and crutches seems likely. Based on the x-ray (IMA 22, HAA 55), what procedure would my colleagues recommend for this patient? What can be done to make post-operative management easier for this nice young man and his parents?

My office uses Zixcorp to receive HIPAA-secure encrypted email with PHI (e.g., progress note transcriptions). I heard that that all gmail are now are "HIPAA secure encrypted email." Is it true that gmail can be a replacement for using Zixcorp?

With ACA & ICD-10, now more than ever, podiatrists need to start changing the way they practice. Ten years ago, I walked away from the limitations of insurance and started conducting clinical trials. I have never looked back! The majority of clinical trials in the field of foot disorders, foot care, and treatment of lower extremity disorders are being conducted by MDs and DOs. Podiatrists are highly qualified and can be considered as experts in the disorders and treatments related to the lower extremity. However, the majority of podiatrists are not taking advantage of the large number of clinical trials available to them.
 
By incorporating clinical research into your private practice, you increase the value of your business. By that, I mean increasing your revenues, profits, and equity should you decide to sell your practice or exit in some way. It's time for podiatrists to discover the lucrative world of clinical research.
