MI Podiatrist Discusses Diabetic Neuropathy and Fall Risk
According to Conway McLean, DPM, "The interactions between the nerves in our feet, the brain, and the muscles utilized to stand are complex and multifaceted. Although it seems logical, the research clearly demonstrates impaired balance and stability in the presence of neuropathy. Those individuals afflicted with adult-onset diabetes will typically have some degree of the nerve dysfunction, making them prone to instability. The studies indicate that even relatively young people with diabetes may have impaired balance but certainly the greatest dangers occur to seniors."

"The combination of reduced muscle tone, nerve dysfunction, and impaired reflexes means that the risk of falling for an older person with type 2 diabetes is dramatically increased when compared to a healthy adult of similar age. The biggest issue is recognition since few individuals will seek out assistance for reducing their risk of falls, especially if the presence of neuropathy has not been made," says Dr. McLean.

NY Podiatrist Discusses Running Shoes and Overpronation
"Overpronation is a timing issue in which one's foot remains in the shock-absorbing phase much longer than it should,” says Samantha Landau, DPM, podiatrist and assistant professor of orthopedics at New York College of Podiatric Medicine. And while the condition may be common among runners, Dr. Landau stresses that “common does not mean normal.” The best running shoes for overpronation have proper cushioning to distribute impact and reduce the risk of injuries.

Landau cautions that the thickness of the midsole, aka the stack height, should not be too far off the ground. Too much cushioning “makes the foot work harder, as it is sinking rather than progressing forward throughout the gait cycle,” she says. A stack height of 30 mm, or approximately one inch, is ideal. Once you find your ideal shoe, Landau recommends pairing it with custom orthotic inserts prescribed by a healthcare professional for optimal stability. “It's really important to wear prefabricated devices along with running shoes that have qualities that match that particular foot and patient's needs to neutralize the deforming forces that are causing the extremity and foot to overpronate,” she says.

Pay the dues, reimburse course fees, and schedule study time so coders stay current instead of scrambling just before renewal dates.

Dr. Tomczak, I am very sorry to read your negative impression of our great profession. I am a 3rd generation DPM and my son will be proudly applying to begin his DPM degree next year. This profession has given me more pride, professional fulfillment, and financial security than I could have ever asked for. I am a professor at Georgetown University, and I have the joy of being part of a team that spends our days saving limbs, saving lives, and helping people find a solution for their foot and ankle conditions.
Perhaps your DPM path did not take you where you wanted…but please don’t feel like you need to spread misinformation and work against those of us who would like to put in the effort and lift this profession to even higher heights. The branding campaign that we are embarking on will be of a scale well beyond anything this profession has ever seen before and will serve as a force to unify our national organizations. We need visibility so that students and patients know who we are and what we do.
A fact check for you: MGMA is the respected authority for group practice compensation and is based on verified compensation for all specialties. The 2024 MGMA puts podiatric foot and ankle surgeons at a median compensation of $361,472.

I hesitate to dignify your comments with a response, but I am offended. You asked a direct question, so I’ll give you a direct answer: No, I don’t feel guilt. What I do feel is a deep responsibility to the profession and to the next generation considering it. I know this field isn’t perfect—far from it. But I’ve also experienced firsthand how meaningful and rewarding a career in podiatry can be. That’s what I try to share. Not myths. Not self-congratulation. Just the truth as I’ve lived it, flaws and all.
You’re right that prospective students deserve honesty. But honesty doesn’t have to be drenched in cynicism. I believe it’s better to light a candle in the dark than to curse the darkness.
We may differ in tone and approach, but I know we both care about this profession. I’ll keep doing what I believe helps move it forward.

I have a very easy solution regarding a reliable metric for podiatric compensation. Since we all essentially "work" for insurance companies now, show prospective students several EOBs covering some of the most common procedures that we perform and point out the billed amount and the allowed amount.
Then mention the fact most practices operate with at least 50-60% overhead and subtract that from the allowed amount and that is exactly what we get paid. And just to be completely transparent, also mention the AMA statistics that over the last 21 years physician income has decreased by 29%. Now any prospective student can make an informed decision regarding their future career.

RE: Do We Really Have a Medical Degree (Allen M. Jacobs, DPM)

I agree with Dr. Jacobs that a 3-year residency can make a huge difference in surgical skills. The problem is that according to PM News surveys, the majority of podiatrists don't do ORIF of ankle fractures and don't want to. Many people go into podiatry to practice podiatric medicine and not orthopedic surgery. This is no longer an option to them. With the mandatory 3-year program, it's just not logical to do the same time in training of MDs and DOs with unlimited specialty options when podiatrists only have one.
The 3-year residency, once a premiere program ensuring excellence in training is now like a high school diploma. Everybody has one. I have seen 3-year graduates who could not do a hammertoe arthroplasty or even a matrixectomy with competence. I would also like to point out that because of this generic requirement, podiatrists like Dr. Jacobs cannot get privileges at many hospitals today.
