NYCPM’s Class of 2024 Displays Research Posters
 
New York College of Podiatric Medicine's (NYCPM) Class of 2024 Research Poster Day, on May 22, was an impressive display of the research and explanatory skills that this class gained and honed in a year-long effort in which teams of students work with faculty mentors. In his introduction to the compilation of the posters, Michael J. Trepal, DPM, Vice President for Academic Affairs and Dean, noted that these skills “are not necessarily easy ones to learn and master,” and Dean of Pre-Clinical Studies and Chair of the Steering Committee, Eileen Chusid, PhD, wrote, “Our hope is that they [the students] will continue to advance research and knowledge in podiatric medicine.”

Seventeen posters were presented and judged by D. Scot Malay, DPM, director of Podiatric Research at the Penn Presbyterian Medical Center and Editor Emeritus of the Journal of Foot & Ankle Surgery. The first-prize winning poster was “A Comparative Study of Semmes-Weinstein Monofilament and Dynamic Neuro-Screening Device in Detection of Early Diabetic Neuropathy,” researched by 2024 students Diana Yoo, Thu Truong, Mina Weisen, Karin Messiha, and Iqra Safarish, who were mentored by Anthony R. Iorio, DPM, MPH and Drs. Jean Chen-Vitulli, DPM. Drs. Kenneth H. Astrin and Ronald Soave rounded out the Steering Committee, and there were also second- and third-prize winners.

CA Podiatrist Discusses Compression Socks for Pregnant Women
 
When shopping for compression socks, podiatrist Salma Aziz, DPM, MS, says pregnant women should start with a mild compression of 15-20 millimeters of mercury (mmHg). This is the same scale of measurement used for blood pressure and indicates the level of compression at different parts of the sock.

You should also consider how comfortable the socks are and whether they provide measurable relief. “Listen to your body,” says Dr. Aziz. “If you’re feeling discomfort while wearing your compression socks, it’s time to remove them, elevate your feet, then put them back on once your swelling has gone down.”

UK Podiatrist Raises Thousands for Strathcarron Hospice at Kilt Walk
 
For podiatrist Douglas Brodie, it was his first time taking on the grueling 23 mile route for charity. His friend Lyn Donaldson, a former Miss Falkirk Herald, has taken part before. This time, they joined forces, putting their love of all things fitness to a very good cause, raising money for the hospice in Denny, and finishing the challenge in five and a half hours, which put them among the earlier finishers.

Tips for Easing Patient Collections in Your Practice
 
Starting in 2023, some credit-reporting firms will remove unpaid medical debt of less than $500. Asking patients to pay their medical bill may become more difficult when there are fewer financial consequences. Here are some strategies to ease the burden of patient collections and improve cashflow and revenue:
 
3. Focus on staff training. - Up-front collections or those taken before service has been provided may be a significant departure from what patients have always known. This is especially the case if a practice’s standard procedure has been to bill the insurance company first and then balance bill the patient, explains Krisi Hutson, MBA, senior director of product development at Availity.
 
Staff should also be able to explain concepts such as deductible, co-payment, co-insurance, and out-of-pocket maximum to patients, says Rozmin Bapat, president of CodeRite Healthcare Consulting. In addition, they should understand how to interpret insurance benefits. For example, a patient might have a deductible that doesn’t apply to an office visit, only to a hospital visit. As a result, the patient may only owe a co-payment.

QUERIES (MEDICAL-LEGAL)
Query: Episource Record Request Review
 
Does anyone have experience with an Episource record request review? They advise that their chart review will include an audit of clinical documentation and a diagnostic coding review. It sounds like they may be critically reviewing doctor notes as well as combing for data extraction. I am concerned that if my notes don't pass muster by their "certified" coders, they will request a return of fees paid! I requested a fee of $50 per chart for 66 charts to discourage the request, but they agreed to it.

I agree with Ivar Roth, DPM, that nitrous oxide should be our top choice for office "sedation." One issue that has yet to be mentioned in the discussion of drugs used to relax patients for office procedure is that the patient taking such a drug cannot drive themselves to and from the office. Failure to inform patients of that issue can be a liability to the practitioner.
 
Nitrous oxide effects are not in effect prior to a procedure and have worn off by the time the patient drives home so this is a significant advantage. Nitrous oxide, used in isolation, is not classified as anesthesia (although it is often called anesthesia) because it cannot be administered in sufficiently high concentrations to create general anesthesia. The safety profile of nitrous oxide is high, especially due to modern equipment that mixes the gas with oxygen. Issues had occurred in the past when the gas mixers on the equipment allowed a user to turn off or reduce oxygen to achieve a high concentration of nitrous oxide but current mixers will not allow that.
 
There used to be a podiatric course on nitrous decades ago. I used nitrous oxide for a number of years but did not continue that in my current state of Texas due to need for additional certification. Regulations for its use vary by state with some states having no rules on this. Nitrous oxide has traditionally been classified as "analgesia" as opposed to anesthesia or sedation which is a reason that its use has been under less strict regulation. This would be a good area to reconsider as the trend toward more office-based procedures continues.

I also used to purchase 10% NaOH from a supplier. As things started getting more and more expensive, I decided to look elsewhere as well. My office is in a complex that has a pharmacy with a compounding center as well. They now compound 10% NaOH for me and it is perfect every time, and is MUCH cheaper. They have also compounded Catharadin for me for verrucae and I have been very happy with their customer service and products. The pharmacy is Cornerstone Pharmacy in Little Rock, Arkansas. Their Rodney Parham location is the compounding pharmacy. Ask for Allison. She is the compounding pharmacist.
