NC Podiatrist Discusses Prevention of Ingrown Toenails
 
According to Wagner Santiago, DPM, "There are various reasons why an ingrown toenail can occur. Tight footgear, improper cutting of nails, trauma to the nail, thickening of the nail, or sometimes there is a hereditary component that can cause a painful ingrown nail. The problem is the nail grows into the side of the skin which creates a red, inflamed, draining toe. It is painful to just have the sheets on your bed touch it, much less try to stuff it in a shoe. Occasionally there is pus accumulated under the skin and if left for long periods of time, can affect the bone leading to bone infection."

"Since the nail grows from the base of the nail bed, cutting a “V” at the end of the nail only makes the nail look “fancy”, but does nothing for the ingrown. However, trying to cut the nail straight across and not tearing the nails off in the corner (or “in the quick”) can prevent the start of the problem. Avoiding tight shoes can also be a benefit. When those options are no longer providing relief, it is time to call a podiatrist who treats foot and ankle problems and has performed ingrown toenail procedures," says Dr. Santiago.

Lori J Hillman, DPM won the Gold Medal in Platform Diving at the 2018 USA Masters Diving Spring Nationals in Ft Lauderdale, FL on 5/20/18. Dr. Hillman practices in The Woodlands, TX and treats several Olympic divers.

Dr. Hillman received her medical training at Des Moines University College of Podiatric Medicine and Surgery in Des Moines, Iowa. She completed her residency in foot surgery at Harris County Podiatric Surgical Residency in Houston and Liberty Medical Center Podiatric Surgical Residency in Baltimore, Maryland. This was followed by a fellowship in foot orthopedic surgery at University of Maryland Hospital in Baltimore.

PA Podiatrist Uses Neurogenx to Treat Diabetic Neuropathy
 
UPMC Pinnacle says it's treating patients with an innovative machine that repairs nerve damage and relieves pain. Seth Steber, DPM says Neurogenx is becoming increasingly popular. After living with diabetes for 10 years, Ron Gilbert's pain kept getting worse. After  Neurogenx treatments, Gilbert said, "It's a lot better. I don't have the stinging pain."

Dr. Steber says Neurogenx is the only FDA-approved device in the country that goes to a certain frequency of electricity. Steber says the key is repairing the nerve instead of suppressing symptoms like many medicines do. He admits nothing is a cure-all for everyone, but he says studies prove the machine's effectiveness. Steber says that after about 12 treatments that last 40 minutes each, many of his patients don't need to come back for more.

Staff Roles Practices Should Consider Adding - Part 2

Whether it be clinical, financial, or technical, today’s physician practices have seen many changes. These changes have resulted in adding new roles within the practices as well as expanding roles for some employees already on staff.

2. Host/Hostess - “In most restaurants, there is a host or hostess that immediately greets patrons. They are responsible for ensuring the restaurant customer enjoys an excellent experience. We adapted the roles and responsibilities of a ‘restaurant’ host/hostess to fit our practice. They immediately greet patients as they arrive at the practice and take the patient to an examination room. They also answer any immediate questions the patient might have. Query: Automatic Level 3?
 
A friend of mine was discussing E/M coding. He tells me that if the new visit includes prescribing a medicine or taking an x-ray, the visit will be enough medical decision-making that it must be a level 3 or higher. This does not seem to make sense to me. Any guidance on this would be helpful.

Response: There is no automatic coding for any scenario, An initial visit requires that the history, examination, AND decision-making all be at the same complexity level. So DOCUMENTING a brief history and examination will not generate a level three E/M service just because your decision-making resulted in medication being dispensed or an x-ray taken. Furthermore, what was the medication: topical antifungal cream, systemic antibiotic? Gout medicine? It does matter as this will impact the complexity level. Some doctors believe (mistakenly) it is allowable to take x-rays (often bilateral) on all new patients regardless of the diagnosis. On audit, was taking the x-ray medically necessary and reasonable? So your x-ray could potentially be disallowed.
 
The key is documentation of the E/M components, making sure everything is medically necessary and reasonable. With EHRs, the templates can be auto-populated and the information may not necessarily be relevant to the particular problem - so it may not count towards bullet points. Bottomline: never assume a certain diagnosis or writing an RX qualifies for a certain E/M level.
