NC Podiatrist Provides Tips on Shoe-Buying for Toddlers
Toddler shoes can be tricky. You want footwear that looks good, for sure, but when it comes to toddler shoes for boys and girls, it’s what on the inside that counts. Jane Andersen, DPM, a board-certified podiatrist based in North Carolina, explained what to look for when buying toddler shoes. The primary goal is to protect their little feet. She told Fatherly, “They’re learning how to walk and when you put on a shoe, it prevents them from slipping.”
|
|
Dr. Jane Andersen |
The key elements to look for make total sense. “You want a flexible shoe without a rigid sole. You want the foot to develop on its own,” she says. “In a perfect world, toddlers could walk barefoot, but in this case a thin sole protects their feet.” When it comes to sizing, “Make sure you ask if there’s any pain or if the shoe rubs anything. You want a little bit of room, a thumb’s width, in the toe box so there’s room to grow. It’s important to have them try on the shoe and be able to test them and wear them around,” says Anderson.

Close to Half of Last-Year Residents Would Prefer Hospital Employment: Survey
Forty-three percent of final-year medical residents said they would prefer being employed by a hospital over any other practice option, according to a survey from healthcare consulting firm Merritt Hawkins. The 2019 Survey of final-year medical residents consisted of an emailed questionnaire that gauged the demand of final-year medical students. About 400 final-year residents responded to the survey.
When considering an employment opportunity, geographic location was cited as a top priority for final-year residents. A good financial package and availability of personal time were second and third, respectively. Just 2 percent of last-year residents said they would consider opening their own practice. If offered a do-over, nearly 20 percent of residents said they wouldn't choose medicine as a career.

Query: Location Code for Nursing Home Billing
There appear to be two location codes for billing at nursing homes, 31 and 32. One is for skilled nursing facility and other is for nursing home. There is a distinct difference in reimbursement between the two codes. Does anyone know how to distinguish one from the other? We were told that long-term care residents versus sub-acute care patients might be the distinguishing factor. Additionally, we were told that both codes might be able to be used in certain facilities depending on the level of care.

Response: The only way to differentiate between Place of Service 31 and 32 is to ask, “Is this patient currently under a skilled stay.” Many facilities have patients that are there under both skilled and non-skilled stays. Some separate them into different parts of the building, but not all do. A patient is normally, but not always, under a skilled stay for the first 100 days following a hospital admission. You cannot assume they are under a skilled stay if it is the first 100 days following hospital admission because there are exceptions. An administrator in an office at the facility will know, and that is the only way for you to know for sure. Check with that person. If they are under a skilled stay at the facility, use POS 31. If they are under a non-skilled stay at the facility, use POS 32.

From: Don Peacock DPM, MS
I agree with the post written by Dr. Allen Jacobs, DPM that some corporate entities can unduly influence where and how money is exchanged. However, he seems to be guilty of the same in his post. He is giving opinion and assessment not based on facts, but merely a flawed view.
He calls the “ambulatory group”, now known as the Academy of Minimally Invasive Foot and Ankle Surgery, a quasi-authoritative group. This is unfair. He degrades the use of EHL tenotomy for most uses. Patients with "spoon toe deformity" complain of pain in footgear. A percutaneous EHL tenotomy for extensus hallux toe deformity is used by many practitioners today. It’s effective and has research back-up. Here is the abstract of one such study. Dr. Monroe LaBorde’s many publications on soft tissue correction of foot and ankle deformities are a great resource.
The Academy is composed of many members including board certified ABFAS members like myself. I find many of the ideas taught by the “ambulatory society” to be very useful. The Academy strengthened me as a foot surgeon. I hope all examine their own biases based in proven research. Dr. Jacobs, I respect your position but strongly disagree with your opinion.

From: Joel A Feder, DPM
Upon close examination, this seemingly beneficial concept will prove to be fuel and fodder for malpractice attorneys. The proper practice of medicine is not a “cookbook”. It is art and not an exact science due to the myriad of differences among patients.

From: Joel Lang, DPM
I am surprised and disappointed that over 1/3 of responding podiatrists are not CPR certified. It only takes a couple of hours and it could save a life. Why would anyone not want this training? Patients trust us with their health and lives. I had to administer CPR once outside of the office and was glad I had that training. It could save the life of a family member as well. Why would any practitioner of any specialty not take responsibility and acquire this skill. Go get certified.
Joel Lang, DPM (retired), Cheverly, MD

RE: Procrastinating Podiatrists
If you make a doctor’s appointment, experts advise booking early in the day. Data show doctors request fewer tests and follow-up procedures in the afternoon compared to the morning. Apparently they make so many choices in the morning that they have decision fatigue.
Peter Podiatrist picked plenty of procedures pre-noon but poor Pete prefers to procrastinate on his PM-prognostications.
