Excess Body Weight Detrimental to Foot Health: IN Podiatrist
 
The pounds we carry unquestionably affect our foot health, says Alex Kor, DPM, a spokesperson for the American Podiatric Medical Association. The APMA found that 74% of overweight Americans surveyed say they experience foot pain. A little over half of U.S. adults surveyed confess that discomfort due to excess weight limits their everyday activities; which means physical fitness also falls by the wayside.

Instead of giving up, Dr. Kor suggests modification. Habitual exercisers can switch to running or walking every other day instead of on consecutive days. Or, log the same miles but spread them out more over the week. Non-weight-bearing workouts — biking, swimming, water aerobics, and elliptical machines — are alternatives.

OH Podiatrist Turns 100 Years Old
 
On Wednesday, Dr. Thomas Maloney’s 100th birthday, a parade, organized by American Legion Post 587, will ride past the veteran’s Toledo home at 7 p.m. to honor his service and sacrifice. Maloney, who had graduated from podiatry school before his service, acted as a pharmacist’s mate during the war. When a soldier was wounded, it was Maloney's responsibility to meet them on the field and address any life threats and then transport them back to the regiment’s hospital which operated out of a boat, he said.

After returning from the war, Dr. Maloney became a podiatrist. He and his wife were married in October, 1947. Dr. Maloney now has 27 grandchildren, and 15 great-grandchildren, most of whom still live in the Toledo area.

CA Podiatrist Discusses How to Test Shoes for Stability and Torsion
 
When you’re shoe shopping, Peter Redko, DPM, a doctor of podiatry at the Northbay Foot and Ankle Center in Petaluma, California, recommends testing for stability and torsion. First, hold the shoe from heel to toe in your hands and push your hands together. “The shoe should only bend where your toes bend,” he says. “If it bends anywhere else, it is not stable enough to protect your plantar fascia.” Then, hold the shoe from heel to toe and attempt to twist the shoe at opposite ends, like wringing out a wet shirt.

“This tests the structural stability of the shoe,” he explains. “If it twists very easily, it does not have the correct amount of stability to support and protect your foot. ACFAS Breaks the Mold with 79th Annual Scientific Conference
 
The 2021 American College of Foot and Ankle Surgeon’s (ACFAS) Annual Scientific Conference is in the books as perhaps a new hybrid in-person/virtual model for future meetings. While the 79th annual conference looked a bit different this year in response to the ongoing pandemic, in-person and virtual attendees received the highest-quality foot and ankle surgery education they come to expect from ACFAS.

In order to hold the conference, the College made sure new safety measures and guidelines were implemented to make every aspect of the meeting safe and accessible whether attendees were traveling or staying home. Surgeons who attended this conference will advance the profession of foot and ankle surgery world-wide, and the hybrid model made it possible for over 800 surgeons from around the country to attend virtually.

Query: Excision of Pressure Ulcer
 
What code would you recommend for excision of pressure ulcer? Here is an excerpt of the operative report: “Attention was directed to the left plantar medical foot at the level of the arch where an approximately 3 cm round chronic ulceration with underlying bursa formation was noted. At this time, an elliptical incision was made in a 3:1 fashion running in line with the foot from toe to heel. This incision was deepened through subcutaneous tissues with care being taken to identify and retract all vital neurovascular structures. At this time, dissection scissors were utilized in order to remove the chronic ulceration with underlying bursa. At this time, the skin margins were then separated in order to free up and allow for skin closure.”

Response: The ulcer statement is not the primary major procedure. The primary issue here is the subcutaneous issue and the diagnosis should be “bursa” or in ICD-10 speak: “M71.372 Other bursal cyst, left ankle and foot”. This diagnosis is paired as the only diagnosis to be billed with CPT 28090 -Excision of lesion, tendon, tendon sheath, or capsule (including synovectomy) (e.g. cyst or ganglion); foot.

RE: Trajectories to Melanoma on the Hands and Feet

Acral melanoma is a subtype of melanoma that occurs on the glabrous (non–hair bearing) skin of the hands and feet, the incidence similar across people of different races and ethnicities. At the genetic level, acral melanomas have diverse mutations known to activate the mitogen-activated protein kinase (MAPK) pathway. Their overall point mutation burdens are relatively low, but they are riddled with complex copy number alterations, recently termed tyfonas. 
 
Tyfonas can be present in pre-cancerous cells up to 3 mm beyond the clinical or histological boundary of acral melanomas, suggesting tyfonas arise early during tumorigenesis that can progress into melanoma. A small subset of melanomas on glabrous skin of the hands and feet have...

When I was a "wet behind the ears" delegate at my first APMA HOD, Simon greeted me with a hearty handshake and a friendly smile. He introduced me to people he thought I should know and showed me how the process worked at the HOD. Thereafter, he always was happy to answer questions and provide advice when asked. Simon was, like so many of our colleagues, a behind the scenes kind of guy who worked hard to make advances in our profession. I am in debt to him for the guidance he showed me and will be making a memorial donation to the APMA Educational Foundation in his name.

I see no reason why we can't or shouldn't as a profession provide sound recommendations on what may be a 'better' flip-flop. There are many out there. I happen to like Olu Kai.

I had a recent patient who provided me with a most unique reply as to why she did not need to obtain the COVID-19 vaccine. She simply stated that when everyone else has been vaccinated-then I won't need it. I bit my tongue to keep from laughing but did enjoy a unique older patient's sense of humor.
