Fire Destroys CT Podiatrist's Office
 
A well-known doctor was “devastated” when he woke up early Tuesday morning to realize his downtown office was gutted by what officials have called a “suspicious” fire. Dr. Jeremy Bier, director of podiatry at Stamford Hospital, said he was alerted by his alarm company of the fire at the two-and-a-half-story house on Bedford Street about 5:45 a.m. “This is my life,” Bier said as he looked at the remnants of his former office. “And my house was demolished, gone.”

Bier said the home, which he bought in 2001 before moving his office into the converted 3,190-square-foot space two years later, is likely a complete loss. “I’ve dedicated my life to my practice,” he said. “It’s like a calling for me being a foot and ankle specialist.”

IPED Welcomes OH Podiatrist as New Trustee
 
The Institute for Podiatric Excellence and Development (IPED) welcomes Gina Tomsho, DPM to its Board of Trustees. Dr. Tomsho is owner and CEO of Barberton Podiatry, Inc. and is in her third year of private practice in Akron, OH. She is a 2011 graduate of KSUCPM. Following graduation, Dr. Tomsho completed a 3-year foot & ankle surgical residency at Alliance Community Hospital.

During her residency, Dr. Tomsho was vice-president of the Mideast Academy of the Ohio Foot and Ankle Medical Association and is currently serving her third year as president. She also satisfies her desire to teach with office rotations and practice management lectures for podiatry residents and medical students and serves as an educator for her local community by volunteering her time for free foot health education lunch & learn lectures and community health fairs.

PA Podiatrist Memorialized with Playground Tree
 
A tree planted in memory of Dr. Terry Musselman last fall was dedicated recently during a ceremony organized by the Greencastle Lions Club and attended by 50 people at the Jerome R. King Playground. Lion Joann Williams noted that the club had previously placed a plaque at the former office of the late podiatrist stating that Musselman was a man who chose Greencastle as a place to live, work, and serve others.

“It’s fitting that we honor Terry’s memory with the planting of a tree that will continue serving the community that Terry loved,” Williams said. The merlot redbud tree is planted near equipment donated to the playground by the Lions Club last year.

Lack of Parking Hurts FL Podiatrist's Practice
 
Finding a place to park at many businesses in South Florida can be difficult. But a group of doctors said they're dealing with a parking emergency at a local hospital. "The parking situation has been critical," said Dr. Sandra Gotman, a podiatrist who works in the medical office building on Kendall Regional Medical Center campus. She said she receives at least two calls a day from patients saying they can't find a place to park. "I'm already coming in to work thinking how many patients will call today to cancel because of the parking," Dr. Gotman said.

She's one of about a dozen doctors who work in the medical office building who are meeting to try to find ways to fix the problem. They said their medical practices are in jeopardy due to the number of patients cancelling services due to the parking situation. They're now consulting with an attorney to figure out what their legal options are to get more parking for patients.

HEALTHCARE LEGISLATION
House Republicans Offer Obamacare Alternative
 
Two congressional Republicans presented a new bill for a free marketplace alternative to the Affordable Care Act (ACA), which they modestly christened the "World's Greatest Healthcare Plan Act of 2016," on Monday. Rep. Pete Sessions (R-TX), who co-authored the new bill along with Sen. Bill Cassidy, MD (R-LA), argued their proposed plan is more sustainable than the ACA, and better suited to patients' individual needs.
 
Their proposed plan would not repeal or replace the ACA -- people can keep their current coverage if they choose to -- but would be an alternative option. It would scrap the employer and the individual mandates, and instead provide tax benefits to every U.S. citizen of $2,500, plus a $1,500 tax benefit for every dependent minor, as long as the minors are also U.S. citizens. A family of four would receive a total of $8,000 tax credit.

My original post stated the benefits of using an ointment, including helping to loosen eschar, thereby keeping the wound intact. There is essentially one caveat. We know that blood is a cultural media and blood serves as "food" for bacterial growth. Do not allow eschar to remain more than 3 days without a topical antibiotic ointment.
 
There are benefits of blood-related products (platelets) in the clotted blood and any allowed eschar formation. The balance of proteins and enzymes must remain the art of medicine and requires your practice judgment. The bio-burden of determinant products in the healing process is more of a risk than the benefits of maintaining a viable wound environment with the benefits of applying copious amounts of ointment 2-3 time/day. It is not the antibiotic substance but the property of ointment's viscosity which is most beneficial. 
 
The wound can exhibit autolysis (self-debridement) to remove wound products and debris (dried blood and byproducts) at times when ointment is essentially absorbed or wasted away. Ointment occludes in a similar way as an eshar. Yet, ointment provides antibiotics and a long-term moist environment for viable tissue growth.

Our office has used both Navinet and Availity for several years for eligibility and benefit status, claim status, payment estimators, and ERAs/EPS. Both sites are free (although Navinet does have an "all payer" access for a fee), and are HUGE time savers. The insurance companies you can access through them vary, but generally include Medicare, BCBS, Aetna, Cigna, Humana, and many others at no cost. I have seen no downside to either website as of yet, and recommend signing up for them.

Dr. Moglia's suggestion that podiatry should change tactics in combating the outrageous letter from the AAOS regarding podiatrists' position in the VA is correct, we should change tactics to deal with this obstacle. However his strategy is flawed. Dentists' domain has no similar medical specialty that treats teeth. Oral surgeons, on the other hand, do meet resistance from several medical specialties. The MD/DMD change has helped mitigate this problem, but not resolve it. I can't speak to reimbursements, but I wouldn't be surprised if oral surgeons and ENTs or plastic surgeons were reimbursed at different rates. 
 
If podiatry intends to remain autonomous, then it must be prepared to take on this battle and many more to come. To this point, the strategy and tactics used should be carefully planned with the appropriate amount of resources and flexibility to be applied to future obstacles. Addressing the common core issue of each of them successfully will prevent continually and repetitively organizing methods to deal with the each of the multiple individual inequities facing podiatry.  
 
I think this option will result in a generational struggle that can be won, but will be successful only when the opposition is compelled to address unfair treatment of podiatrists. In my opinion, the success of this option will be in the legislature and/or the courts. Congressman Brad Wenstrup, DPM is an example of what it will take at the federal level, and other podiatrists seeking office at the state levels, to make meaningful changes.

In response to Dr. Smith's inquiry, the Council for Medical Education and Testing was formed in 2007 and has been certifying physicians since 2008. It is a physician-specific organization formed by physicians for physicians and certifies all prescribing physicians, MDs, DPMs, and DOs. With more than 700 physicians now certified, it is by far the largest physician-specific certification in United States. It provides a seven year certificate and offers five pathways for recertification.
