When Should Running Shoes Be Replaced? DPM's Comment

Your kicks will cushion for only about 200 miles, says Lance D. Barry, D.P.M., a Georgia podiatrist who has completed 21 marathons. "After 200 miles, the midsole has lost enough impact cushioning to make you more prone to heel pain and carpal tunnel-like symptoms."

Don' t track miles? Pinch the sole with your thumb on the outside and fingers inside, says Richard Braver, D.P.M., a science advisor to Runner's World. If your fingers can't compress the sole, it's time for a new pair.

Armstrong Delivers Grand Rounds Simultaneously in Canadian Hospitals

It is becoming common for podiatrists to deliver medicine grand rounds at academic medical centers around the world. Doing so at three major university medical centers simultaneously, however, is indeed rather unusual. On Tuesday David G. Armstrong, DPM, PhD, Professor of Surgery at Scholl’s Center for Lower Extremity Ambulatory Research (CLEAR) at Rosalind Franklin University of Medicine and Science delivered a lecture at Toronto’s Women’s College Hospital. With the assistance of telemedicine, his presentation was broadcast to lecture theatres at Toronto’s St. Michaels Medical Center, Women’s College Hospital, and Sunnybrook Medical Center.

The subject matter was infectious diseases and the discussion was lively. Armstrong concluded: “The fact that internal medicine, surgery and other general medical disciplines are focusing on what was once an orphan specialty speaks volumes to the solid work that is being done in this area. I believe that podiatry is really making a difference both in patient care and in meaningful academics and scholarship. We are beginning to look outward instead of inward, and that is hugely healthy for the future of foot care.”

We have recently converted our 2 offices to an EMR system. We researched options for nearly 9 months and chose eClinicalWorks. It is a completely integrated system that includes practice management, EMR, and billing. We have been running on it for about 4 months now and are very pleased with its usability and its potential.

We use it on a completely wireless system with tablets in the office for staff. Using the ASP model (running it over the Internet) kept the cost of the software down (about $400 a month per doctor) and lets me access the information from any computer. Our biggest expenses were hardware and labor to have an IT specialist get us up and going. We did underestimate the hardware and IT set-up costs.

My recommendation is to choose an integrated system and talk to other podiatrists who use the system you are looking at. You also must have someone in the office who understands enough about computers, networks and software who can bridge your office needs with your hardware, IT, and software needs. Looking back, I would make the same choice, we are very happy with the end product.

RE: Periodic Medical Exams of Diabetics By Podiatric Physicians (Ken Malkin, DPM)

I complete agree with Dr. Malkin. Diabetics ARE entitled to medical exams of the feet which have nothing to do with at-risk foot care. In fact, as one of several podiatrist within our local county hospital system, I am seeing an increasing number of diabetic referrals from the primary care clinics. Many are seen with pre-ulcerative lesions and undocumented peripheral neuropathy. These MDs know what we do and expect an appropriate podiatric evaluation and assessment.
