CSPM Graduate Prepares for Harvard Medical School Residency
 
Monique G. Rodriguez, DPM became a doctor of podiatric medicine on Saturday and after completing her residency, plans to return to the Rio Grande Valley to start her practice as a podiatrist and podiatric surgeon to give back to her community. She will graduate from the California School of Podiatric Medicine (CSPM) at Samuel Merritt University in Oakland and attend the Harvard Medical School’s podiatric surgery residency program at Mount Auburn Hospital in Cambridge, MA, becoming the first Latina and only the fourth woman to be in the program.

Podiatrist Erin Nelson, DPM is happy to be working in Ames again after she recently opened her own practice, Foot and Ankle Specialists of Ames. Nelson worked at McFarland Clinic for eight years and left to work at Des Moines University (DMU) for four years, where she was an assistant professor and the clinical chair of the DMU Foot and Ankle Clinic.

UK Podiatrist Recommends Bi-annual Podiatric Foot Check-ups
 
When it comes to your feet, "There’s an element of ‘out of sight, out of mind’," says Emma Price, podiatrist and co-founder of PodiPedia.co.uk. "If you don’t have any foot pain, you don’t necessarily think of your feet. It’s not until you get some nice weather and put sandals or flip-flops on, and then go: ‘Oh, what’s that?!'" For Price, feet can "absolutely" affect your confidence. "Personally, it makes me feel great knowing that I’ve got good looking feet," she says.

Price suggests many are reluctant to visit a podiatrist, saying: "People are embarrassed by their feet, but if you had a dodgy looking tooth or your tooth was going a bit brown or yellow, you’d get yourself straight to the dentist. You wouldn’t paint your tooth and carry on – that’s obviously a bit of a silly example, but it’s true – that’s effectively what you’re doing." She recommends annual or biannual check-ups with a podiatrist, like you do with a dentist. "I’m very much about prevention being better than a cure," Price adds, emphasizing that podiatrists can spot early signs of serious medical conditions such as rheumatoid arthritis.

PODIATRIC PRODUCTS AND SERVICES IN THE NEWS
VA Podiatrist Makes “Shark Tank”-Like Presentation at GWU Health Summit
 
Ben Pearl, DPM presented his hallux limitus/turf toe splint in a “Shark Tank” format to students at the recent George Washington University (GWU) Health Summit. Dr. Pearl, who has been working closely with the premed student group, developed the segment to ignite interest in creating medically related devices and/or apps to be used in the medical world. The splint is on pace to sell several thousand units by the end of the year. The sharks’ role was to provide critique and suggestions.

Pearl reached out to Doug Richie, DPM (inventor of the Richie Brace) and Richard Stess, DPM (inventor of the STS Casting sock) to play the role of “sharks”. The segment followed the keynote address by Dr. Richard Carmona, former surgeon general. Richie offered that the information on the website should include more simple language on what hallux limitus is. Drs. Stess and Richie commented that more education has to happen one-on-one with medical personnel.

RELEVANT RESEARCH
Descriptive Study of Ulcers Treated in a Diabetic Foot Unit
 
Diabetic foot (DF) is one of the most prevalent complications in diabetic patients and an important cause of morbidity, mortality, and health costs. Multidisciplinary diabetic foot units (DFU) aim to improve the management of DF and reduce its complications. The objective of this report is to describe the type of patient received by location, severity of their pathology, and the main results of treatment.
 
Results: 288 patients were included, 239 of whom had a total of 404 ulcers. Of them, 58.4% were digital ulcers, 20.5% metatarsal, 5.4% midfoot, and 15.6% heel ulcers. Regarding Texas Classification, 18.2% of the lesions were group A, 9.7% group B, 52.2% group C, and 20% group D. At follow-up, the healing rate was 77.2% and re-ulceration occurred in 35.1% instances. The minor amputation rate was 36.1%, major amputation 6.3%, and mortality 5.9%.
 
Conclusion: Our experience in our DFU suggests that this multidisciplinary team can improve the results of DF ulcer management, reduce major amputation rates, and increase survival of our patients.

What Practices Must do to Assure Payment for Telehealth in 2021 - Part 2
 
The flourishing of telehealth in 2020 was driven not just by necessity, but also by a significant loosening of governmental regulations and reimbursement requirements by payors. To ensure that you will continue to be paid for your telehealth services, here are three best practices you should be aware of:
 
2. Closely monitor bulletins and announcements regarding telehealth from any insurance payors your practice accepts.
 
Payor policies are changing rapidly and unexpectedly. Do not assume that any policies that applied in 2020 will continue. Make monitoring of payor policies at least a monthly discipline.
 
When you verify that a particular patient’s insurance accepts telehealth, determine if the coverage specifies the use of the payor’s approved telehealth vendor only. In that case, appointments conducted on the practice’s telehealth platform may not be covered. In some cases, state telehealth regulations might specify that the telehealth appointment must be allowed to be conducted with the patient’s regular provider, which could supersede the payor’s requirements. However, even if your state has such a requirement, be extra careful in 2021. There may be a gap between the implementation of state requirements and the provider contracts and policies that would postpone application of the state requirements until 2022.

Query: Appropriate Hospice Care
 
I biopsied a suspicious lesion from the lower leg of a hospice patient. Pathology reported a squamous cell carcinoma. Is it appropriate from a Medicare billing perspective to excise this lesion on a hospice patient using the -GW modifier if the patient’s original hospice qualifying condition is unrelated to the carcinoma?

Response: When a person goes into hospice, they are no longer treated for the underlying condition that created the need for hospice care. They are maintained peacefully. However, if an unrelated situation should arise such as a “suspicious lesion” that needed to be biopsied, shouldn’t this lesion be biopsied and the physician that performed the procedure be reimbursed? Of course. That is what led to the creation of the -GW modifier which is defined as the following: HOSPICE SERVICES – Use this Medicare modifier when you need to indicate that the provided services are not related to the hospice patient’s terminal condition.

RE: Outrageous Joke of the Day
 
A man went to the doctor. He said, "Doc, you gotta check my leg. Something's wrong. Just put your ear up to my thigh, you'll hear it!" The doctor cautiously placed his ear to the man's thigh only to hear "Gimme 20 bucks, I really need 20 bucks". "I've never seen or heard anything like this before, how long has this been going on?" the doctor asked. "That's nothing Doc, put your ear to my knee."
 
The doctor put his ear to the man's knee and heard it say, "Man, I really need 10 bucks, just lend me 10 bucks!" "Sir, I really don't know what to tell you. I've never seen anything like this." The doctor was dumbfounded. "Wait Doc, that's not it. There's more, just put your ear up to my ankle," the man urged him.
 
The doctor did as the man said and was blown away to hear his ankle plead, "Please, I just need 5 bucks. Lend me 5 bucks please if you can." I have no idea what to tell you," the doctor said. "There's nothing about it in my books," he said as he frantically searched all his medical reference books. "I can make a well-educated guess though. Based on life and all my previous experience, I can tell you that your leg appears to be broke in three places."
