NC/SC Podiatrists Collaborating With CDC on an Infection Prevention Project
 
InStride Foot and Ankle Specialists (IFAS) is one of ten medical practices in the United States selected to work with the Centers for Disease Control and Prevention (the CDC) and the Joint Commission on a research project designed to prevent and control infections in ambulatory healthcare settings. The goal of this three-year program will be to create model infection control plans to be used to prevent infections across the country. Gary Laio, DPM is the leading investigator for InStride Foot and Ankle.

IFAS is based in Concord, North Carolina but has seventy-six podiatrists and over sixty locations spread across North and South Carolina. IFAS President Kevin McDonald, DPM stated, "We are grateful for the opportunity to be part of this important initiative to decrease the incidence of infections in outpatient settings in America. The access to the latest protocols and procedures by working with the CDC fits in perfectly with our best practices program."

Feet become both longer and wider as you age because the tendons and ligaments that link tiny bones lose elasticity. Get your feet measured at least once a year so you know your true size, suggests Dr. Timothy Swartz, chief of podiatry of Kaiser Permanente Mid-Atlantic region.

Robert Snyder: If you look globally at a place like Spain or Italy, more often than not, the generalist sees the patient first. With insurance changes here in the U.S., generalists are now taking up this role. For example, if a generalist sees a person with an abnormal mammogram, she will be sent to a breast surgeon. if the patient in the next room has a high PSA, he will be sent to the urologist, but the patient in the third room with a terrible wound is given an Rx for an antibiotic (they might not even need) and told to return in 4 weeks.

A story like this should raise a red flag about the patient's integrity and/or emotional stabilty. Sure idiosyncratic reactions can occur with any medication but a story like this raises doubts about the patient, not the medication. The patient may be "testing the waters" to see how the doctor handles a complaint. It may be wise to limit treatment for this patient to very conservative management and see how she reacts to further interventions. In a nutshell, be the last on line to surgically remove that patient's neuroma.

I think Dr. Bergman gave some sage advice "use your clinical judgment" regarding dry eschars. While the fact "dead tissue doesn't get better," removing it can also potentially open a can of worms. Hence, I like the sage advice better.

Since parity with MDs/DOs has been met with obstruction, why not change tactics and pursue parity with the DDSs/DMDs. The DDSs/DMDs perform dental care and work with parity along side the MD/DO performing reconstructive surgery of the jaw as oral surgeons. Why not accept the limited license and work towards parity payscale with the dentists?

Summary
 
52-year-old diabetic man with a right diabetic foot ulcer and cellulitis, possible osteomyelitis.
 
Plan and recommendations:
1.  Alginate to the wound for now
2.  Check MRI and ABIs
3.  Continue to off-load pressure
4.  Glycemic control
5.  Continue nutritional and supportive care
6.  Podiatry has been constipated and plans debridement after MRI
